Phonatory vocal tract stability in stuttering children before and after fluency--enhancing therapy.

BACKGROUND: Stuttering is a complex disorder. Essentially, it is a neuromuscular disorder whose core consists of tiny lags and disruptions in the timing of the complicated movements required for speech. OBJECTIVE: The purpose of the current study was to collec and comparg jitters and shimmer values in children who stutter before and after fluency--enhancing therapy. METHODS: Subjects consisted of 15 Iranian preschool girls with stutterg, and 15 Iranial preschool girls without afflictions, matched according to age. Vocal jittering and shimmer measurements of thesphonation of the children were…

Long- and short-term results of children's and adolescents' therapy courses for stuttering.

BACKGROUND: It is widely known that most young children recover from stuttering. Evaluations of this spontaneous recovery vary from 40 to 80% of stuttering children. However, if the child is already older than 6 years of age, the spontaneous recovery will be less likely. The effectiveness of stuttering treatment, and especially fluency shaping methods, has been frequently studied. Good results in the treatment of stuttering for school-aged children have been accomplished by several treatment methods. However, stuttering modification treatments have not been studied so intensively. AIMS: This …

Telehealth adaptation of the Lidcombe Program of Early Stuttering Intervention: five case studies.

This article describes and reports data-based outcomes of a low-tech telehealth adaptation of the Lidcombe Program of Early Stuttering Intervention. Participants were 5 children with early stuttering, ranging in age from 3;5 (years;months) to 5;7, and their families. All children met the speech criteria for completion of Stage 1 of the Lidcombe Program. Data suggested that the treatment method may be viable and that favorable outcomes may be achievable. Mean posttreatment stuttering rates in everyday speaking situations were available 12 months posttreatment for 4 children. Two children scored…

Evidence-based treatment of stuttering: III. Evidence-based practice in a clinical setting.

UNLABELLED: At the heart of evidence-based practice in stuttering treatment are four issues: (1) the collection of data to inform treatment; (2) the long standing concern with maintenance of treatment gains; (3) the need to demonstrate accountability to clients, payers and our profession as service providers; and (4) the desire to advance theoretical knowledge. This article addresses the first three of these issues from a practical point of view, illustrating how data collection for stuttering treatment outcome research in a clinical setting is intimately blended with that required for clinica…

Comparison of diadochokinesis in Kannada-speaking adults who do and do not stutter.

PURPOSE: In the context of limited understanding of the objective measurement of diadochokinesis in adults with stuttering, the study aimed to compare the oral diadochokinetic and laryngeal diadochokinetic rates between adults who stutter and adults who do not stutter. The study further examined the effect of lexicality (real words vs. non-words) in oral diadochokinetic tasks. METHOD: Using Praat software, oral (oral diadochokinetic rate, consonant-vowel duration, and inter-syllabic duration) and laryngeal diadochokinetic rates were examined among 35 adults who stutter and 35 age- and gender-…

A Proof-of-Concept Study of Gamified Rhythmic Training in Preadolescents Who Stutter.

Stuttering is a developmental speech fluency disorder linked to timing deficits in speech motor control. Given the shared neural mechanisms between rhythmic timing and speech production, rhythm-based interventions may hold promise for stuttering. This proof-of-concept study evaluated the feasibility and potential benefits of a gamified rhythmic training program, Rhythm Workers (RW), in preadolescents who stutter. Twenty-one children (aged 9-12) were randomly assigned to RW or an active control game, which they played at home for 3 weeks. We assessed feasibility and potential training effects o…

Brain anatomy differences in Chinese children who stutter: a preliminary study.

BACKGROUND AND PURPOSE: It is unknown the neural mechanisms of developmental stuttering (DS). The aim of this study was to investigate the changes in the structural morphology of the brain in Chinese children who stutter. METHODS: A case-control study was conducted to collect magnetic resonance imaging data from stuttering and non-stuttering children, thereby analyzing whole-brain gray matter volume and cortical morphological changes in stuttering children. RESULTS: A total of 108 subjects were recruited (stuttering group: control group = 1:1). Comparing to healthy controls, the gray matter …

Behind the mask: Stuttering, anxiety, and communication dynamics in the era of COVID-19.

BACKGROUND: While wearing masks during the pandemic poses communication and social challenges for people in everyday life, those with social anxiety might find them plausible, aligning with contemporary cognitive theories. Social anxiety involves fearing negative assessments and holding a negative self-image. Concealing anxiety symptoms during mask use may contribute to a more positive self-perception. AIMS: Given that up to 60% of adults seeking stuttering treatment also meet criteria for social anxiety disorder, this study aims to investigate the complex relationship between communication f…

Designing a Module on Stuttering and Cluttering: A Guide for Speech-Language Pathology Educators.

PURPOSE: Due to the scope of practice of speech-language pathology (SLP) expanding considerably in recent times, there is reduced emphasis on certain communication conditions within the curricula of SLP university programs. Stuttering and cluttering are neglected components of such curricula, despite the complex clinical skill set required to work with these client groups. Evaluation of the content and quality of modules on stuttering and cluttering is warranted to ensure that SLP students are graduating with adequate competence and confidence for supporting people with these conditions. This …

Clinical and Psychosocial Predictors of Post-Event Processing in Adults Who Stutter.

PURPOSE: Post-event processing, defined by self-critical rumination following a stressful communication event, is significantly associated with reduced quality of life. However, despite its relevance to the stuttering experience, to date, only a few studies have investigated post-event processing among adults who stutter, and no study has identified clinical and psychosocial predictors of post-event processing. The purpose of this study was to determine the contributions of clinical markers of stuttering and psychosocial variables to post-event processing. METHOD: Adults who stutter (N = 96) …

The Expansion of the Spectrum in Stuttering Disorders to a Novel ARMC Gene Family (ARMC3).

Stuttering is a common neurodevelopment speech disorder that negatively affects the socio-psychological dimensions of people with disability. It displays many attributes of a complex genetic trait, and a few genetic loci have been identified through linkage studies. Stuttering is highly variable regarding its phenotypes and molecular etiology. However, all stutters have some common features, including blocks in speech, prolongation, and repetition of sounds, syllables, and words. The involuntary actions associated with stuttering often involve increased eye blinking, tremors of the lips or jaw…

Neuroanatomical anomalies associated with rare AP4E1 mutations in people who stutter.

Developmental stuttering is a common speech disorder with strong genetic underpinnings. Recently, stuttering has been associated with mutations in genes involved in lysosomal enzyme trafficking. However, how these mutations affect the brains of people who stutter remains largely unknown. In this study, we compared grey matter volume and white matter fractional anisotropy between a unique group of seven subjects who stutter and carry the same rare heterozygous AP4E1 coding mutations and seven unrelated controls without such variants. The carriers of the AP4E1 mutations are members of a large Ca…

Successful Management of Psychotropics Induced Stuttering Priapism with Pseudoephedrine in a Patient with Schizophrenia.

Stuttering Priapism is a recurrent, persistent penile erection in the absence of sexual desire due to altered genital hemodynamics, affecting the arterial component (high flow, non-ischemic) or the veno-occlusive mechanism (low flow, ischemic). Both typical and atypical antipsychotics increase the risk for priapism with greater implications in typicals than atypicals. Prompt recognition and treatment are important as 40% to 50% of patients with stuttering priapism may develop an erectile dysfunction if left untreated. There are several case reports in the literature about the association betwe…

An investigation of the effects of a speech-restructuring treatment for stuttering on the distribution of intervals of phonation.

PURPOSE: The purpose of this study was to investigate whether stuttering reductions following the instatement phase of a speech-restructuring treatment for adults were accompanied by reductions in the frequency of short intervals of phonation (PIs). The study was prompted by the possibility that reductions in the frequency of short PIs is the mechanism underlying such reductions in stuttering. METHOD: The distribution of PIs was determined for seven adults who stutter, before and immediately after the intensive phase of a speech-restructuring treatment program. Audiovisual recordings of conve…

When a school-age child stutters, let's focus on the primary problem.

In this reply to Yaruss, Coleman, and Quesal (2012), I explain why it is reasonable for speech-language pathologists to focus their attention on helping school-age children who stutter (CWS) achieve natural-sounding fluent speech. I also emphasize the necessity for clinical researchers to conduct high-quality treatment efficacy studies of school-age CWS.

Right-shift for non-speech motor processing in adults who stutter.

INTRODUCTION: In adults who do not stutter (AWNS), the control of hand movement timing is assumed to be lateralized to the left dorsolateral premotor cortex (PMd). In adults who stutter (AWS), the network of speech motor control is abnormally shifted to the right hemisphere. Motor impairments in AWS are not restricted to speech, but extend to non-speech orofacial and finger movements. We here investigated the lateralization of finger movement timing control in AWS. METHODS: We explored PMd function in 14 right-handed AWS and 15 age matched AWNS. In separate sessions, they received subthreshol
