CT of the pacchionian body.

The CT appearances of the pacchionian body, confirmed at autopsy, are reported. The pacchionian body appears as a low density mass in the dural venous sinus, of almost the same density as the subarachnoid space.

Computer tomography as the primary radiologic procedure in acute subarachnoid hemorrhage.

CT was performed in 149 patients with acute subarachnoid hemorrhage and clinical findings consistent with an aneurysm rupture, and was found informative in most cases when performed within one week after the hemorrhage. CT disclosed in 29 per cent of cases other causes for the hemorrhage than a ruptured aneurysm (intracerebral hemorrhage, tumor, trauma, infarct) and an extensive angiographic evaluation could be omitted. In those patients where the hemorrhage was caused by an aneurysm rupture, the distribution of extravasated blood in the subarachnoid space and the brain parenchyma usually indi…

Retrolaminar Migration of Intraocular Silicone Oil.

BACKGROUND: Migration of intravitreal silicone to the retrolaminar optic nerve was detected pathologically in 1983, symptomatic migration to the subarachnoid space of the optic nerve was reported in 1994, and asymptomatic intraventricular silicone was first seen radiographically in 1999. Since then, little advance has been made in understanding this phenomenon despite numerous case reports. Although some authors have restricted their attention to cases of intraventricular silicone, we believe that these represent part of a clinical spectrum and that all cases with retrolaminar silicone should …

Ligation of cervical lymphatic vessels decelerates blood clearance and worsens outcomes after experimental subarachnoid hemorrhage.

Subarachnoid hemorrhage (SAH) is characterized by the extravasation of blood into the subarachnoid space, in which erythrocyte lysis is the primary contributor to cell death and brain injuries. New evidence has indicated that meningeal lymphatic vessels (mLVs) are essential in guiding fluid and macromolecular waste from cerebrospinal fluid (CSF) into deep cervical lymph nodes (dCLNs). However, the role of mLVs in clearing erythrocytes after SAH has not been completely elucidated. Hence, we conducted a cross-species study. Autologous blood was injected into the subarachnoid space of rabbits and…

Effect of thrombin injection on cerebral vascular in rats with subarachnoid hemorrhage.

OBJECTIVE: To evaluate the effect of thrombin (TM) injection via the cerebellomedullary cistern on cerebral vessels in rats with subarachnoid hemorrhage (SAH). METHODS: Eighteen rats were randomly divided into three groups. In the A1 group, physiological saline was injected via the cerebellomedullary cistern; in the A2 group, 3 U of TM was injected into the subarachnoid space; and in the A3 group, SAH models were established and 3 U of TM was injected with the first injection of whole blood. Three days later, basilar artery specimens were collected for pathological examination. RESULTS: The …

Pseudo-subarachnoid hemorrhage in a patient with acute cerebellar infarction.

OBJECTIVE: Hyperdensity in the basal cisterns on computed tomography (CT) brain scan is a diagnostic feature in subarachnoid hemorrhage. However, false enhancement of the subarachnoid space on CT scan may be seen in diffuse neurological conditions with acute presentation. This is the first reported case of a 'focal lesion' (acute cerebellar infarct) producing a 'pseudo-subarachnoid hemorrhage' secondary to increased intracranial pressure. CLINICAL PRESENTATION: A 42-year-old man presented with acute onset of left hemiataxia and neck pain. CT brain scan showed a left cerebellar infarct. During…

Constrictive endarteropathy following experimental subarachnoid hemorrhage.

Rhesus monkeys were subjected to subarachnoid hemorrhage by either the introduction of blood into the subarachnoid space or by mechanically rupturing the middle cerebral artery. Local cerebral blood flow, vascular compliance studies, and histological studies (light and electron microscope) were made of the vessels of all animals. In animals hemorrhaged via vascular rupture, subintimal swelling and thickening was minimally present by 3 days, becoming progressively more severe by one month. Such changes consisted of severe subintimal proliferation, fibrosis of the medial smooth muscle layer and …

Intracranial pressure elevation alters CSF clearance pathways.

BACKGROUND: Infusion testing is a common procedure to determine whether shunting will be beneficial in patients with normal pressure hydrocephalus. The method has a well-developed theoretical foundation and corresponding mathematical models that describe the CSF circulation from the choroid plexus to the arachnoid granulations. Here, we investigate to what extent the proposed glymphatic or paravascular pathway (or similar pathways) modifies the results of the traditional mathematical models. METHODS: We used a compartment model to estimate pressure in the subarachnoid space and the paravascul…

Evaluation of volumetric changes in differential diagnosis of brain atrophy and active hydrocephalus.

Despite a variety of diagnostic methods, differentiation of symptoms of normal pressure hydrocephalus from those of atrophic processes of the brain is still a difficult task. In the present study an attempt of non-invasive differential diagnosis of normal pressure hydrocephalus (NPH) and brain atrophy (BA) was presented using volumetric analysis of CT images of the head by means of VisNow proprietary software. The analysis was based on the number of voxels converted to the amount of cerebrospinal fluid (CSF) in the subarachnoid space, skull base casters, and the ventricular system. The results…

Death associated with an indwelling orbital catheter.

We report a case of a 38-year-old woman who underwent revision of an orbital implant. A flexible indwelling orbital catheter was placed for postoperative anesthesia. Marcaine, administered to the patient through the 5.25-inch catheter while the patient was at home, led to respiratory arrest. After unsuccessful resuscitation, the patient died. At autopsy, it was noted that the catheter passed through the superior orbital fissure, with evidence of injection of marcaine in the subarachnoid space. This patient had signs and symptoms of Stickler syndrome. This leads us to believe that deficiencies …

Delayed respiratory arrest in combined spinal-epidural anesthesia. Case report.

BACKGROUND AND OBJECTIVES: A case of delayed respiratory arrest in the combined spinal-epidural anesthesia (CSEA) is described. This event was likely due to morphine injected through the epidural catheter, unintentionally entering into the subarachnoid space through the hole in the dura that was made previously by the spinal needle in the needle-through-needle technique. METHODS: The CSEA is a popular new regional anesthesia that combines the benefits of the spinal and epidural injections. The needle-through-needle technique, which is only one of the various techniques to accomplish CSEA, may…

Interdural cyst of the lumbosacral region.

The purpose of this report is to call attention to the clinical, myelographic, and computed tomographic appearance of a very rare type of dural cyst within the lumbosacral spinal canal. We report this condition in three unrelated boys who presented with symptoms of cauda equina compression. Our experience suggests that these cysts are congenital in origin. Anatomically, the cyst wall consisted of a dura-like layer without arachnoid. There was a small ventral defect allowing incomplete communication with the compressed subarachnoid space. Cerebrospinal fluid-like fluid accumulated within the in…

Cerebrospinal fluid ferritin in chronic hydrocephalus after aneurysmal subarachnoid hemorrhage.

OBJECTIVES: Subarachnoid hemorrhage (SAH) is a common cause of chronic hydrocephalus. Blood in the subarachnoid space is intracranially metabolized to bilirubin and iron, and free iron is thereafter detoxified by ferritin. However, no studies have reported the relationship between intracranial heme metabolism and chronic hydrocephalus after SAH. The goal of this prospective study was to clarify the relationship between intracranial heme metabolism and chronic hydrocephalus after SAH. METHODS: The authors measured the levels of bilirubin, iron and ferritin in the cerebrospinal fluid (CSF) of 7…

A large arachnoid cyst of the lateral ventricle extending from the supracerebellar cistern--case report.

BACKGROUND: The pathogenetic mechanism of intraventricular arachnoid cyst development is still controversial, but is believed to originate from the vascular mesenchyme or as an extension of the arachnoid cyst in the subarachnoid space into the ventricle through the choroidal fissure. We report a case supporting the extension hypothesis and suggest differential points between an intraventricular arachnoid cyst that extended from the supracerebellar space and a lateral ventricular diverticulum that extended into the supracerebellar cistern. CASE DESCRIPTION: A 12-month-old girl presented with m…

Cerebral intraventricular haemorrhage in a young adult.

A case of a 26-year-old man who suffered a fatal intraventricular cerebral haemorrhage following an episode of trauma is described. The initial appearance at necropsy suggested a traumatic subarachnoid haemorrhage and initial investigation was directed towards the anterior neck structures and the vertebral arteries with negative results. Dissection of the fixed brain showed a massive intraventricular bleed with secondary involvement of the subarachnoid space and dissection into the cerebral parenchyma. No bleeding points or natural disease of the cerebral vessels could be identified. The pract…

The potential of disulfiram as a drug to improve the prognosis after the onset of subarachnoid hemorrhage.

Subarachnoid hemorrhage due to rupture of intracranial aneurysms has a poor outcome, making this disease being the social problem. Inflammation evoked by the increase in intracranial pressure and the clot in the subarachnoid space after the onset of SAH exacerbates neuronal death and vasospasm, resulting in the poor outcome and severe aftereffects. Here, FROUNT mediates CCR2 and CCR5 signaling as an intracellular molecule binding to these chemoattractant receptors which facilitate the migration of inflammatory cells, such as macrophages, in situ to trigger inflammation there. Animal model of s…

Noninvasive MR cisternography with fluid-attenuated inversion recovery and 100% supplemental O(2) in the evaluation of neurocysticercosis.

We describe an MR protocol for the noninvasive imaging of the subarachnoid space, which we use in patients with suspected neurocysticercosis in this space. It consists of a fluid-attenuated inversion recovery sequence performed 5 minutes after the continuous inhalation of 100% O(2) with a resultant increase in the signal intensity of the CSF that leads to a greater conspicuity of cyst walls in relation to the cortex and the extraventricular CSF.

Chronic idiopathic inflammation of the retropharyngeal space presenting with sequential abducens palsies.

We describe a patient who presented with sequential, bilateral abducens palsies associated with a mass of the nasopharynx. Biopsy of the mass showed chronic non-specific inflammation and fibrosis. The diagnosis of idiopathic inflammatory pseudotumor was arrived at by exclusion of other known causes of inflammation of the retropharyngeal space. Magnetic resonance imaging suggested that injury to the sixth cranial nerves probably occurred as they traversed the dura and subarachnoid space overlying the clivus.

Craniocervical Junction Morphology and Cerebrospinal Fluid Spaces at the Foramen Magnum in Idiopathic Intracranial Hypertension: Implications for Pathophysiology.

Background: Although numerous magnetic resonance imaging (MRI) findings have been described in idiopathic intracranial hypertension (IIH), the potential contribution of craniocervical junction morphology to disease pathophysiology and diagnosis remains largely unexplored. This study aimed to evaluate foramen magnum morphology and cerebrospinal fluid (CSF) spaces at the craniocervical junction in patients with IIH and to investigate their associations with disease severity and diagnostic performance. Methods: In this retrospective case-control study, 70 patients diagnosed with IIH according to
