Surgical treatment of congenital pseudarthrosis of the clavicle: A series of 10 cases.

INTRODUCTION: Congenital pseudarthrosis of the clavicle (CPC) is a rare congenital malformation. We describe the outcomes of surgical treatment of CPC in a series of 10 patients. The hypothesis was that surgical treatment is an acceptable solution that will lead to satisfactory healing in asymptomatic patients. The aims were to determine if 1) surgical treatment of CPC will produce a satisfactory union rate, 2) surgical treatment causes complications and impacts the postoperative outcomes and 3) risk factors can be identified that affect the success of the surgical care. METHODS: Ten patients

Optimising the tip-apex-distance in trochanteric femoral fracture fixation using the ADAPT-navigated technique, a longitudinal matched cohort study.

INTRODUCTION: The annual incidence of proximal femoral fractures is 100-150/100,000 and continues to increase with an aging population. Cut-out of hip screws after fracture fixation has been quoted as 8% in the literature. The tip-apex distance (TAD) is the strongest predictor for cut-out after operative fracture stabilisation. The aim of this study was to evaluate the novel ADAPT system (Adaptive Positioning Technology, Stryker, USA), a navigation device for intramedullary nailing of trochanteric fractures and its effect on optimising the TAD. This is the first clinical study to evaluate this

Chronic instrumentation with model microstimulators in an animal model of the lower urinary tract.

BACKGROUND: Microstimulators are a new type of neuroprosthetic device that should be considered for applications such as micturition control after spinal cord injury (SCI). These devices are small (less than 25 mm by 5 mm) and the electrodes are located on the ends of the stimulator. The aim of the current study was to develop methods for chronic implantation of model microstimulators (M-Micro) on the bladder wall and pelvic plexus of female cats. A postmortem evaluation of the effects of 3 months of implantation is reported. METHODS: Techniques to produce the M-Micro are described. Four of t

Distal femoral malunion: Biomechanical drivers, fixation strategies and the role of fixator assisted correction.

Distal femoral malunion can have significant consequences because deformity at this level alters knee loading via its longest lever arm. Even relatively small magnitudes of malunion may lead to varus or valgus shift of the mechanical axis, creating abnormal compartmental contact pressures and external moments. This may lead to activity related pain and accelerated joint degeneration. Although distal femoral fractures account for only 0.5% of all skeletal injuries, they include high energy injuries in young patients, low energy mechanisms in older patients and a subgroup of periprosthetic injur

How we do an anterior sacrospinous ligament fixation for vaginal vault prolapse.

Since 2019, all vaginal mesh implants were removed from the market. Since, surgeons have only the autologous technique left to treat prolapse by vaginal route. The anterior sacrospinofixation is an alternative technique to treat vaginally apical prolapses. We have divided the anterior sacrospinous ligament fixation technique into 10 surgical steps: exposure, infiltration, vaginal incision, vesico-vaginal dissection, paravesical dissection, sacrospinous ligament suture (two passage in the SSL on each side, using a suture-capturing device), vaginal fixation, vaginal closure, sacrospinous ligamen

Percutaneous Endoscopic Sigmoidopexy for Recurrent Sigmoid Volvulus: A Systematic Review of Efficacy, Safety, and Clinical Outcomes.

Sigmoid volvulus represents a significant clinical challenge, particularly in elderly patients with multiple comorbidities who experience recurrent episodes following initial endoscopic decompression. While definitive surgical resection remains the gold standard for preventing recurrence, many patients are unsuitable for major abdominal surgery due to advanced age, medical frailty, or elevated surgical risk. Percutaneous endoscopic sigmoidopexy has emerged as a minimally invasive alternative that involves endoscopically guided fixation of the sigmoid colon to the anterior abdominal wall, there

Valgus intertrochanteric osteotomy for non-union of femoral neck fracture.

In spite of improved surgical techniques and fixation devices, non-union still reportedly occurs in 10-30% of cases of femoral neck fracture. A variety of methods of treatment that preserve the femoral head have been described, yet there are few reports on the results of valgus intertrochanteric osteotomy in cases of femoral neck non-union. We treated 11 such cases with valgus intertrochanteric osteotomy performed at one centre and using a single surgical technique. The cases were evaluated clinically and radiographically at a mean of 4.9 years. All of the non-unions were shown radiographicall

Application of a shape-memory alloy internal fixator for treatment of acetabular fractures with a follow-up of two to nine years in China.

Displaced acetabular fractures should be treated surgically. Over the past decade, surgical approaches to the acetabulum and the surgical technique for repair of common fracture patterns have advanced. Excellent outcomes after repair of these injuries can be achieved. The aim of this study was to assess the medium-term results of reconstruction of acetabular fractures by using shape-memory alloy designed by the authors. This is a retrospective review conducted at a level 1 trauma centre. From October 1999 to July 2009, 19 patients with acetabular fractures were treated with our patented Ni-Ti

Biomechanical properties of double-headed pedicle screws and parallel connectors in thoracolumbar extension revision fixation: a finite element analysis.

BACKGROUND: Revision surgery of the spine frequently requires removal of the original instrumentation, which can cause substantial iatrogenic tissue injury. To address this issue, we developed a revision construct utilizing double-headed pedicle screws and parallel connectors, which can obviate the need for removal of the original internal fixation. This study aimed to investigate the biomechanical properties of these constructs. METHODS: An intact finite element model of T12-L5 was established. Then, four surgical models were developed. Constructs A and C simulated the traditional long and sh

Which Technique is the Optimal Option for the Treatment of Displaced Far-Lateral (Cho Type IIC) Distal Clavicle Fractures? A Comparative Analysis of Coracoclavicular Fixation Versus Anatomical Locking Plate Fixation.

BACKGROUND: As achieving stable fixation in unstable far-lateral distal clavicle fractures is challenging, the optimal surgical treatment remains controversial. The aim of this study was to compare the clinical and radiological results of coracoclavicular fixation (CCF) versus anatomical locking plate fixation (ALPF) in patients with Cho type IIC fractures. METHODS: Thirty-eight patients who underwent surgical treatment for Cho type IIC fracture were included. Open CCF using a suture button device was performed in 25 patients and ALPF was performed in 13 patients. The mean follow-up period wa

Durability of cervical disc arthroplasties and its influence factors: A systematic review and a network meta-analysis.

BACKGROUND: The durability of cervical disc arthroplasties (CDA) may vary significantly because of different designs and implanting techniques of the devices. Nevertheless, the comparative durability remains unknown. OBJECTIVES: We aimed to assess the durability of CDAs in at least 2-year follow-up. We analyzed the classifications and causes of secondary surgical procedures, as well as the structural designs of the devices that might influence the durability. METHODS: PubMed, Medline, Embase, and Cochrane Central Register of Controlled Trials were searched from the inception of each database

Extreme Lateral Interbody Fusion (XLIF) with Lateral Modular Plate Fixation: Preliminary Report on Clinical and Radiological Outcomes.

The lateral transpsoas approach (extreme lateral interbody fusion, or XLIF) allows surgeons to use various lordotic cage sizes to help restore intervertebral disk height, correct sagittal alignment, and improve fusion rates. The use of standalone devices has consistently raised doubts due to the high risk of complications and inadequate functional recovery that a circumferential arthrodesis can support. The recent introduction of a novel XLIF cage with adapted lateral plate fixation (XLPF) may further enhance the structural rigidity, consolidating the cage and plate into a singular modular ent

Hardware complications related to the surgical fixation of slipped capital femoral epiphyses.

Slipped upper femoral epiphysis is a disabling condition with an annual incidence of 2-13 per 100,000. In situ surgical fixation is the preferred initial treatment for both stable and unstable slipped capital femoral epiphysis (SCFE) cases. The main aim is to avoid further slippage and complications such as osteonecrosis and chondrolysis. The choice of medical implants in managing this condition has changed along the years from large nail-like devices to cannulated screws. The biomechanical properties of different fixation techniques have been studied. All implants have been associated with co

Clinical application of instant 3D printed cast versus polymer orthosis in the treatment of colles fracture: a randomized controlled trial.

BACKGROUND: The shortcomings of plaster in water resistance, air permeability, skin comfort, fixed stability and weight of wearing are still to be solved. 3D printed cast can overcome the above shortcomings. At present, there is a relative lack of data on the clinical application of 3D printed cast, probably due to its complexity, relatively long operating time, and high price. We aimed to compare and evaluate the short-term effectiveness, safety and advantages of 3D printed wrist cast versus polymer orthosis in the treatment of Colles fracture. METHODS: Forty patients with Colles fracture in
