[Swallowing magnets: a dangerous game. A case report].

Complications related to the ingestion of magnetic foreign bodies by children represents an affirmed health hazard in the United States. In France, an alert has been announced. We report the 1st case in France. Our aim is to alert pediatricians and emergency physicians and to draw attention to the particularities of this type of foreign body. Responsible for complications is the ingestion of at least 2 magnets, or 1 magnet and a metallic foreign body, with a time interval between ingestions. In these cases, it is strongly recommended to extract the foreign bodies with endoscopy if they have no

Evaluation of swallowing in children with vomiting after feeding.

Vomiting after feeding is a symptom of gastroesophageal reflux (GER) and of eosinophilic esophagitis (EE), which are considered to be a cause of infant feeding disorder. The objective of the present study was to evaluate swallowing in children with feeding disorder manifested by vomiting after feeding. Using clinical and videofluoroscopic methods we studied the swallowing of 37 children with vomiting after feeding (mean age=15.4 months), and of 15 healthy children (mean age=20.5 months). In the videofluoroscopic examination the children swallowed a free volume of milk and 5 ml of mashed banana

The effect of taste and palatability on lingual swallowing pressure.

There is evidence that a strong, unpalatable, sour bolus improves swallowing in neurogenic dysphagia. It is not known whether other tastes may alter swallowing physiology. This study investigated the effect of moderate versus high taste concentrations (sweet, sour, salty, bitter) and barium taste samples on lingual swallowing pressure in ten healthy young adults, using a three-bulb lingual pressure array secured to the hard palate. Palatability of the samples was analyzed using the nine-point hedonic scale. Results showed that moderate sucrose, high salt, and high citric acid elicited signific

A swallowed fishbone penetrating the oesophagus into the sternomastoid muscle.

A 40-year-old woman presented with the history of swallowing a fishbone one hour earlier. Since the patient was swallowing normally and the X-ray results were negative, she was reassured and sent home. Twenty days later, when the patient again presented with pain and tenderness in her neck, the fishbone was detected in the left sternomastoid muscle, and removed under local anaesthesia.

Videofluoroscopic swallow studies in unilateral cricopharyngeal dysfunction.

OBJECTIVES/HYPOTHESIS: Although the cricopharyngeus muscle is a ring-like structure, unilateral cricopharyngeal dysfunction can produce significant dysphagia. This entity has not been well described in the literature. The aims of the study were to identify the characteristic findings on videofluoroscopic swallow studies in patients with dysphagia secondary to unilateral cricopharyngeal dysfunction, to note the associated vagal nerve injury, and to evaluate patient outcomes following ipsilateral cricopharyngeal myotomy. STUDY DESIGN: Retrospective clinical investigation. METHODS: The clinic c

Swallow syncope associated with paroxysmal atrial fibrillation.

Swallow or deglutition syncope is a very unusual potentially lethal but treatable disorder. We report the case of a 26-year-old woman, who presented with a history of recurrent, multiple fainting episodes precipitated by swallowing. Twenty-four-hour manometry and pH recording together with continuous 24-h ECG monitoring revealed multiple episodes of symptomatic and asymptomatic paroxysmal atrial fibrillation, and significant gastro-oesophageal reflux associated with swallowing. Oesophageal function tests and continuous electrocardiographic evaluation is important in the diagnosis of this rare

Pulse oximetry monitoring for the evaluation of swallowing function.

The aim of this study was to investigate the relationship between aspiration and oxygen desaturation and to assess the efficacy of using pulse oximetry monitoring as a clinical tool to predict or detect aspiration. The 204 subjects were divided into four groups: 63 control subjects, 110 dysphagics, 9 dysphagics with a cuffed tracheostomy tube and 22 laryngectomized subjects. Arterial oxygen saturation (SpO2) was continuously measured throughout fluorographic examination until an adequate post-examination period afterward. Then, fluorographic findings (aspiration) and SpO2 levels were compared.

Has aphasia therapy been swallowed up?

OBJECTIVE: To review the change in number of referrals to speech and language therapy for dysphasia and dysphagia over one decade. DESIGN: This retrospective study analyses data collected on an information system developed specifically for the use of speech and language therapists in the management of their services. SETTING: Eleven health care providers used the system between 1985 and 1995 and recorded details of 73758 patients referred to speech and language therapy during that period. RESULTS: Whilst there were 12 times as many dysphasic patients as dysphagic patients referred to speech

Zinc-induced copper deficiency in a coin swallower.

In humans, acquired copper deficiency anemia is rare. This report describes a 58-yr-old man with metal pica, especially coins, who presented with symptomatic anemia. His workup led to the diagnosis of zinc-induced copper deficiency. We believe that, in this man, leaching of zinc from pennies explained the clinical and laboratory findings. This case demonstrates that health care workers should consider the possibility of zinc-induced copper deficiency when confronted with patients with unexplained anemia who have ingested coins or other zinc-containing metals.

Timing of events in normal swallowing: a videofluoroscopic study.

Dynamic videofluoroscopic swallow studies were performed on 60 normal adult volunteers to establish normative data for clinically useful timing measures. The relation of swallowing gesture timing to the timing of actual bolus transit was of particular interest because it provides insight into the physiology of larger bolus volume accommodation. Parameters evaluated include the timing of bolus pharyngeal transit, soft palate elevation, aryepiglottic fold elevation and supraglottic closure, arrival of the bolus in the vallecula, hyoid bone displacement onset and duration, arrival of the bolus at

Safety of sublingual-swallow immunotherapy in children and adults.

BACKGROUND: Immunotherapy is an established treatment of allergic diseases. The safety of this treatment, particularly when administered without direct medical surveillance, as in the case of the sublingual-swallow route needs to be established. The aim of this paper is to review the safety of the sublingual-swallow immunotherapy as reported in eight double-blind, placebo-controlled trials carried out in France, Italy and Greece. METHODS: Six hundred and ninety subjects, 472 adults and 218 children, took part in trials of specific immunotherapy (SIT) for the treatment of rhinoconjunctivitis a

[Effects of wearing complete dentures on swallowing in the elderly].

The purpose of this study was to evaluate effects of wearing complete dentures on swallowing in the elderly. Sixteen nondysphagic edentulous volunteers (mean age: 75.0 yrs) were studied in terms of surface electromyography of the anterior belly of digastrics and masseter muscles, tongue pressure toward the hard palate, and the laryngeal movement. The subjects swallowed 2 ml water under four conditions, with upper and lower dentures, without lower denture, without upper denture, and without upper and lower dentures. Dipper-type of swallowing was prescribed. A repeated-measures ANOVA was used fo

Normal swallowing physiology as viewed by videofluoroscopy and videoendoscopy.

BACKGROUND: Neonatal mortality remains one of the most pressing global health challenges, disproportionately affecting low- and middle-income countries. In Ethiopia, complex interrelated factors contribute to high neonatal mortality rates, creating a multifaceted public health concern that demands comprehensive investigation. Despite the magnitude of this challenge, there exists a notable lack of longitudinal studies that examine the temporal patterns of neonatal deaths, survival, and associated risk factors throughout the complete neonatal period. This study addresses this gap by evaluating s

Quantitative aspects of swallowing in an elderly nondysphagic population.

The prevalence of swallowing impairment increases with age and is a major health care problem in the elderly. It has been assumed that age-related changes in nerves and muscles hamper muscle strength and coordination of swallowing. However, it is unclear what impairment is related to primary aging and what is the consequence of diseases prevalent in the elderly (secondary aging). In order to quantify swallowing in nondysphagic elderly we used the noninvasive ROSS (Repetitive Oral Suction Swallow) test. A total of 53 individuals aged 76 +/- 5 years (mean +/- SD) were examined. We found that the

The effects of brain stem stroke on communication and swallowing.

A brain stem stroke frequently results in impairment of speech and/or swallowing. Symptoms may range from mild to severe, depending on the location and extent of the stroke. This article describes the brain stem's role in communication and swallowing, as well as evaluation and treatment of disorders caused by brain stem stroke. A number of brain stem syndromes that commonly impair speech and swallowing, including Wallenberg's syndrome and locked-in syndrome, are also discussed.
