Arthroscopic treatment of pigmented villonodular synovitis of the proximal tibiofibular joint.

Disorders of the proximal tibiofibular joint should be kept in mind in the evaluation of lateral knee pain. They include osteoarthrosis, rheumatic disease, traumatic subluxation or dislocation, ganglion or synovial cysts, synostosis, synovial chondromatosis, pigmented villonodular synovitis and hypomobility of the joint. Peroneal nerve can be at risk with pathologies of the joint either by compressive effect or formation of intra-neural ganglion. A case of pigmented villonodular synovitis of the proximal tibiofibular joint was reported which presented with lateral knee pain. It was successfull…

Synovitis with pitting edema as the presenting manifestation of systemic lupus erythematosus.

Rheumatologists are increasingly aware of the entity synovitis with pitting edema. The remitting seronegative symmetrical synovitis with pitting edema (RS3PE) syndrome has been reported with an array of conditions that include polymyalgia rheumatica, rheumatoid arthritis, Sjögren's syndrome and psoriatic arthropathy. Synovitis with pitting edema is now being increasingly recognized with systemic lupus erythematosus (SLE). We report a patient who presented with edema of hands and feet and was diagnosed eventually with definite SLE. With magnetic resonance imaging, joint effusions and tenosynovi…

[Effectiveness of arthroscopic synovectomy in treatment of pigmented villonodular synovitis of knee].

OBJECTIVE: To explore the effectivness of arthroscopic synovectomy in the treatment of pigmented villonodular synovitis (PVNS) of the knee. METHODS: A retrospective analysis was conducted on 13 patients with PVNS of the knee treated with arthroscopic synovectomy between June 2008 and December 2011, including 8 left knees and 5 right knees. There were 9 males and 4 females, aged 25-45 years (mean, 33 years). Of 13 patients, 5 had a history of trauma, and 8 had no history of trauma. The disease duration ranged from 4 months to 80 months (mean, 44 months). The preoperative Lysholm score was 45.3…

Haemophilic synovitis of the elbow: radiosynovectomy, open synovectomy or arthroscopic synovectomy?

Arthroscopic synovectomy (AS) of the elbow, while providing similar pain relief to open synovectomy (OS), may place patients at higher risk for recurrence. The primary predictor of outcome is degree of pre-existing degenerative changes within the joint. Regarding haemophilia patients, radiosynovectomy (RS) is the best choice for patients with persistent synovitis of the elbow. In the elbow we recommend a dose of 30-40 megabecquerels (mBq) in children and a dose of 56-74 mBq in adults. If three consecutive RSs with 6 months intervals are ineffective, an AS or OS must be indicated. Synovectomy (…

Fine needle aspiration of pigmented villonodular synovitis of the temporomandibular joint.

Temporomandibular joint (TMJ) problems are usually the result of functional disorders. Tumors of the TMJ are rarer entities, the diagnosis of which may be delayed due to a clinical impression of a functional ailment. We report a case of a patient with a recurrent TMJ area lesion diagnosed cytologically as pigmented villonodular synovitis (PVNS), an entity that very rarely involves the TMJ and is usually diagnosed by histology. The differential diagnosis of PVNS in the context of its location in and around the TMJ is also discussed.

Exceptionally aggressive pigmented villonodular synovitis of the hip unresponsive to radiotherapy.

Pigmented villonodular synovitis (PVNS) is a rare benign neoplastic proliferation of synovial tissue which is typically localised and usually responds well to surgery and/or radiotherapy. We present a case of unusually aggressive of PVNS of the hip in a 73-year-old woman.

Pharmacokinetics of intra-articular morphine in horses with lipopolysaccharide-induced synovitis.

OBJECTIVE: To describe the pharmacokinetics of intra-articularly (IA) administered morphine. STUDY DESIGN: Experimental randomized, cross-over study. ANIMALS: Eight adult healthy mixed breed horses aged 6.5 +/- 2.3 (mean +/- SD) years and weighing 535 +/- 86 kg. METHODS: Unilateral radiocarpal synovitis was induced by IA injection of 3 microg lipopolysaccharide (LPS) on two occasions (right and left radiocarpal joint, respectively) separated by a 3-week wash-out period. Treatments were administered 4 hours post-LPS-injection: Treatment IA; preservative free morphine IA (0.05 mg kg(-1)) plus…

Flexor tendon synovitis of the hand as first manifestation of atypical tuberculosis.

We present the case of a 73-year old patient suffering from chronic flexor tendon synovitis of the wrist with carpal tunnel syndrome. He underwent synovectomy and median nerve release. Primary bacteriology was negative. Histology of the excised synovia revelead non-caseating granuloma as typical for sarcoidosis. Further screening for sarcoidosis was negative. Culture of a sample harvested from the poorly healing wound was finally positive for Mycobacterium tuberculosis. Tuberculostatic treatment was started and the wound gradually healed. To the best of our knowledge, this is the first reporte…

Immune features of seronegative and seropositive arthritis in early synovitis studies.

Synovitis of recent onset is a challenging problem, both from a diagnostic and a mechanistic point of view. The role of the immune system in mediating the systemic and synovial inflammatory response remains an area of active investigation. Studies in early synovitis cohorts have confirmed the relatively specific association of rheumatoid factor positive polyarthritis with a number of autoantibodies, particularly anticyclical citrullinated peptide (CCP) antibodies, antifilaggarin antibodies (AFA), and anti-Sa antibodies. Immunopathologic studies of synovial tissue samples from patients with ear…

Localized pigmented villonodular synovitis presenting as recurrent dislocation of the patella.

A 31-year-old woman presented with a 1-year history of recurrent dislocation of the patella. On examination, she was found to have a mass at the medial patellofemoral joint line. Arthroscopy showed this to be a vascular synovial tumor extending into the medial patellofemoral joint and causing lateral deviation of the patella. The mass was resected with resolution of her symptoms and histologic examination showed this to be pigmented villonodular synovitis. There are an increasing number of case reports of these lesions causing varying mechanical symptoms in the knee and they should be consider…

Dynamic MR-imaging in the diagnosis of pigmented villonodular synovitis of the knee.

The aim of this report was to study the rate of contrast enhancement at MRI in pigmented villonodular synovitis (PVNS). Dynamic MRI was performed in a patient with PVNS just before and 1 week and 3 years following synovectomy of the knee. As opposed to patients with rheumatic disease the patient with PVNS showed a slower enhancement rate before synovectomy than 1 week after. At 3 years follow-up the patient with PVNS showed an enhancement rate just above that of the normal controls. The results indicate that PVNS has features more common to hyperplastic or neoplastic lesions than to inflammato…

[Pigmented villonodular synovitis of the knee. Treatment results in 22 cases].

PURPOSE OF THE STUDY: The authors report their experience about 22 cases of pigmented villonodular synovitis of the knee observed between 1970 and 1997. The authors studied the results of treatments according to localization, evolutionary stage and condition of an early diagnosis. MATERIAL AND METHODS: Among thirty three cases of pigmented villonodular synovitis treated during the same period the knee was involved in twenty two cases. Diagnosis was done or highly suspected in 13 cases because of significant symptoms (iterative hemarthrosis, bony gap in both articular components of the joint) …

Distribution of immunoglobulin classes in plasma cells in various kinds of synovitis.

Sixty-five cases of synovitis corresponding to eight groups (seropositive and seronegative rheumatoid arthritis (RA), ankylosing spondylitis, psoriatic arthritis, post-traumatic arthritis, osteoarthritis, osteonecrosis and unclassified synovitis) were investigated in specimens stored as paraffin blocks. Immunoglobulin class content (heavy chains: G, A, M and their associated light chains: K, L) of the plasma cells was determined by the immunoperoxidase method. Plasma cells were observed in all cases in both RA groups but not in all cases of the other groups. All three classes of immunoglobulin…
