[Longer medical certificates do not improve the quality of physicians' assessments].

Sickness certification is a common and demanding task in Swedish primary care, particularly for patients with mental health conditions, for whom assessments of work ability and functional impairment are often associated with considerable uncertainty. Drawing on the findings of Falk et al., the authors argue that longer or more detailed sickness certificates do not improve the consistency or quality of physicians' assessments but do increase the administrative burden. The authors advocate shifting the focus from documentation towards active rehabilitation. Suggested strategies include earlier a

Mitigating the HIV and Viral Hepatitis Workforce Crisis Through Development of an HIV/Hepatitis C Coinfection Mobile Application.

A decline in the HIV workforce has led to a crisis of insufficient expertise to manage people with HIV (PWH), roughly a quarter of whom are coinfected with hepatitis C. Task shifting to nonspecialist providers can contribute to solving the HIV workforce shortage problem, but nonspecialist providers require sufficient training and support to acquire and retain the necessary knowledge and skills. Digital tools including mobile applications (apps) and telementoring which utilizes telecommunication technology for education and skill acquisition can be used for professional development. Described i…

Internal switching and backward inhibition in depression and rumination.

Prior research has suggested that impairments of switching abilities are associated with depression as well as rumination. Backward inhibition (BI) refers to the ability to inhibit the processing of previously relevant information and is demonstrated to be one of the key mechanisms underlying switching abilities. However, the association between BI in internal switching and depression/rumination remains uninvestigated. To examine this association, a modified Internal Shifting Task (IST) was administered to a sample of dysphoric and healthy control undergraduates. Results showed that depressive

The scaling of postural adjustments during bimanual load-lifting in traumatic brain-injured adults.

Previous postural studies of traumatic brain injury (TBI) patients have been limited to identifying deficits in static and quasi-dynamic postural control tasks such as weight shifting. In this study, we examined whether or not patients with TBI are able to scale adequately their postural adjustments during the performance of the dynamic task of bimanual load-lifting. An age matched group of healthy adults served as controls. We used the Tetrax posturography system that calculates a stability score (ST) based on fluctuations in vertical ground reaction forces, normalized for body weight. During

Assessment of formaldehyde exposures under contemporary embalming conditions in U.S. funeral homes.

The funeral service profession has used formaldehyde-containing embalming solutions for the preparation of decedents since the early 1900s. The available literature regarding funeral director exposure to formaldehyde largely consists of data collected prior to 2000, with most studies reporting task-length exposure concentrations rather than full-shift time-weighted average concentrations. As formaldehyde undergoes review in the U.S. Environmental Protection Agency Toxic Substances Control Act (TSCA) risk evaluation process, accurately characterizing long-term exposure potential in this profess

Quantifying distribution shifts in single-cell data with scXMatch.

MOTIVATION: A basic task that frequently arises when analyzing single-cell data is to assess if there is a global distribution shift between the data profiles of cells from two different conditions. Widely used approaches to address this task such as visual inspection of two-dimensional representations or clustering-based workflows lack a solid statistical underpinning and are notoriously unstable and prone to confirmation bias. RESULTS: To promote more rigorous analysis, we present scXMatch. scXMatch is based on a non-parametric graph-based test to quantify distribution shifts in arbitrary d

Perceptual training: learning versus attentional shift.

Prior research has demonstrated that perceptual training can improve the ability of healthcare trainees in identifying abnormalities on medical images, but it is unclear if the improved performance is due to learning or attentional shift-the diversion of perceptional resources away from other activities to a specified task. Our objective is to determine if research subject performance in perceiving the central venous catheter position on radiographs is improved after perceptional training and if improved performance is due to learning or an attentional shift. Attention dysfunction subtypes of developmental dyslexia.

BACKGROUND: Previous studies indicate that many different aspects of attention are impaired in children diagnosed with developmental dyslexia (DD). The objective of the present study was to identify cognitive profiles of DD on the basis of attentional test performance. MATERIAL/METHODS: 78 children with DD (30 girls, 48 boys, mean age of 12 years ±8 months) and 32 age- and sex-matched non-dyslexic children (14 girls, 18 boys) were examined using a battery of standardized tests of reading, phonological and attentional processes (alertness, covert shift of attention, divided attention, inhibiti

Muscle synergies involved in shifts of the center of pressure while standing on a narrow support.

We investigated multi-muscle synergies during preparation to push a load forward and their changes with different support conditions. We hypothesized that the subjects show unchanged mode structure and would be able to form multi-mode COP stabilizing synergies while standing on an unstable board. Eight healthy subjects participated in the study. Standing subjects performed load-pushing tasks under two conditions, "normal support" and "ML narrow support". Electromyographic (EMG) signals of 12 postural muscles were recorded and analyzed. The participants also performed standard tasks associated

Qualitative Exploration of factors affecting Uptake and Demand for Contraception and other Family Planning Services in North-West Nigeria.

In spite of the improvements in knowledge about family planning (FP), the contraceptive prevalence rate and unmet need for FP remain poor in most parts of Northern Nigeria. This study sought to explore specific factors that influence contraceptive uptake and demand in North-West Nigeria. Key Informant and In-depth Interviews were conducted using guides among stakeholders in two selected states in North-West Nigeria, Kebbi and Sokoto States. Interviewees were selected purposively to include Reproductive Health Focal Persons at the local government level, service providers, Women of Reproductive

Pharmacological effects on accommodative adaptation.

We review the research which has attempted to determine whether the characteristics of autonomic innervation of ciliary smooth muscle are relevant to the process of accommodative adaptation. The effect of various topical autonomic drugs on the three phases of adaptation were analyzed: pre-task tonic accommodation (open-loop); within-task accommodative response (closed-loop); and post-task regression of accommodation to pre-task tonic levels. Although it is clear that parasympathetic innervation predominates, there is evidence that some individuals utilize supplementary inhibitory sympathetic i

The Effects of Short-Lasting Anti-Saccade Training in Homonymous Hemianopia with and without Saccadic Adaptation.

Homonymous Visual Field Defects (HVFD) are common following stroke and can be highly debilitating for visual perception and higher level cognitive functions such as exploring visual scene or reading a text. Rehabilitation using oculomotor compensatory methods with automatic training over a short duration (~15 days) have been shown as efficient as longer voluntary training methods (>1 month). Here, we propose to evaluate and compare the effect of an original HVFD rehabilitation method based on a single 15 min voluntary anti-saccades task (AS) toward the blind hemifield, with automatic sensorimo

Instructing to ignore a color does not promote its suppression but its facilitation in human early visual cortex.

Attentional shifts during feature-based color selection trigger a bi-phasic activation sequence in early visual cortex: rapid facilitation of the to-be-attended followed by delayed suppression of to-be-ignored color(s). Does this sequence reflect a priority bias induced by instructions "to attend" to a certain task-relevant color instead of instructions "to ignore" task-irrelevant colors? In two EEG recording sessions, we instructed 20 participants either "to attend" to two colors (explicit instruction) or "to ignore" one color (implicit instruction) out of three differently colored and flicke

A registered re-examination of the effects of leftward prism adaptation on landmark judgements in healthy people.

It has long been known that active adaptation to a shift of the visual field, caused by laterally-displacing prisms, induces short-term sensorimotor aftereffects. More recent evidence suggests that prism adaptation may also stimulate higher-level changes in spatial cognition, which can modify the spatial biases of healthy people. The first reported, and most replicated, higher-level aftereffect is a rightward shift in the point of subjective equality (PSE) for a perceptual bisection task (the landmark task), following adaptation to leftward prisms. A recent meta-analysis suggests that this vis

Slowing is slowing: Delayed neural responses to words are linked to abnormally slow resting state activity in primary progressive aphasia.

Neurodegenerative disorders are often characterized by neuronal "slowing," which may be assessed in different ways. In the present study, we examined the latency of neural responses to linguistic stimuli in participants diagnosed with primary progressive aphasia (PPA), as well as changes in the power spectra of resting state activity, both measured with MEG. Compared to both age-matched and younger controls, patients with PPA showed a delayed latency of 8-30 Hz event-related desynchronization (ERD) in response to semantic anomalies. In addition, resting-state MEG revealed increased power in th

Assessing visual performance with progressive addition lenses.

PURPOSE: As a result of lens design limitations, progressive addition lenses (PAL's) present a limited field of view for tasks at intermediate distances, such as with computers. To assess whether this limitation results in diminished visual performance, PAL's were compared with single-vision lenses in four different types of reading tasks in a computer workplace environment. METHODS: Adult subjects performed four computer-based reading tasks using both single-vision lenses and PAL's at an intermediate distance of 64 cm. RESULTS: Single-vision lenses performed significantly better than PAL's

Subjective and objective workload of critical care nurses in a tertiary hospital in Switzerland: a retrospective analysis.

BACKGROUND: Nursing workload in intensive care units affects outcomes and staff well-being. Objective measures such as Nine Equivalents of Nursing Manpower Score (NEMS) capture task demands, whereas subjective ratings reflect nurses' workload; how these relate is unclear (1-4). METHODS: All shifts over 6 months in an adult ICU of a tertiary hospital were analyzed. For each shift, staffing (number of nurses, patients, patient-to-nurse ratio PNR), number of interventions, admissions and discharges, cumulative Richmond agitation sedation scale RASS score and intensivist identity were recorded. Cognitive functioning of female nurses during the night shift: The impact of age, clock time, time awake and subjective sleepiness.

Decline in cognitive functioning in the workplace is a major concern for health care systems. Understanding factors associated with nighttime functioning is imperative for instituting organizational risk management policies and developing personalized countermeasures. The present study aims to identify individual factors associated with cognitive functioning during the night shift of hospital nurses working on irregular rotating-shift schedules. Ninety-two female nurses were recruited from 17 wards in two general hospitals, using convenience sampling by clusters. Inclusion criteria were workin
