Ruptured temporal lobe arachnoid cyst presenting with severe back pain.

Arachnoid cyst is a common congenital anomaly in the pediatric population. The cysts are often asymptomatic, but they can cause headache and other symptoms. Occasionally a cyst may rupture after head trauma producing a subdural hematoma. The authors present the case of an 11-year-old boy who presented after a week of progressive and severe back pain radiating to the back of his thighs. Imaging revealed a spinal subdural blood collection at the L4-S1 level. This finding prompted further cephalad imaging of the spine and the brain, which revealed a sylvian fissure arachnoid cyst with intracystic

Seizure precipitants and inhibiting factors in mesial temporal lobe epilepsy.

Epileptic seizures may be triggered by both nonspecific facilitating factors and specific reflex epileptic mechanisms. These consist of sensory or cognitive inputs activating neural networks that, due to some functional instability, may respond with an epileptic discharge. The aim of this study was to determine the prevalence and nature of self-perceived seizure-inducing and -inhibiting factors in patients with mesial temporal lobe epilepsy (MTLE) followed from March 3rd to December 8th, 2009 at the Centro de Epilepsia de Santa Catarina Outpatient Clinic of the Hospital Governador Celso Ramos

Temporal lobe epilepsy confused with panic disorder: implications for treatment.

A 34-year-old woman with a 9-year history of unprovoked attacks of anxiety and dyspnea associated with symptoms of depersonalization and derealization is presented. The attacks increased in frequency and were associated with internal derogatory voices, vivid frightening imagery, and suicidal ideation, leading to 3 emergency psychiatric hospitalizations in a period of less than 3 months. She had been treated unsuccessfully for a presumptive diagnosis of panic disorder without agoraphobia, prompting a reconsideration of this diagnosis. Although electroencephalography and magnetic resonance imagi

[Arachnoid cyst, hypoplasic temporal lobe and facial anomalies: a neurocristopathy].

The authors report on a patient which presented at birth facial anomalies similar to those of facial alcoholic syndrome (i.e. high forehead, wide nasal bridge, upturned nose, flat philtrum), low set ears, short neck. Successive-ly, also an arachnoid right temporo-polar cyst with hypoplasic right temporal lobe was diagnosed. They think that cerebral cyst, hypoplastic temporal lobe and facial anomalies are congenital. By the origin of facial and leptomeningeal structures from the neural crests, the authors conclude that facial, arachnoid and cerebral anomalies depend from the derangement of the

Focus laterality and interictal psychiatric disorder in temporal lobe epilepsy.

The current study was carried out in order to find the possible associations between foci laterality and kind of prevailed psychopathological disorder in patients with temporal lobe epilepsy (TLE). One hundred and ten patients with TLE (40 men and 70 women) were included into the study. Among all studied patients the left-focus activity was detected in 67 patients, right-sided foci-in 43 patients. No relationships between chronology variables of epilepsy (age, age at epilepsy onset, epilepsy duration) and different subtypes of psychopathology in studied patients were revealed. Temporal lobe epilepsy and GEFS+ phenotypes associated with SCN1B mutations.

SCN1B, the gene encoding the sodium channel beta 1 subunit, was the first gene identified for generalized epilepsy with febrile seizures plus (GEFS+). Only three families have been published with SCN1B mutations. Here, we present four new families with SCN1B mutations and characterize the associated phenotypes. Analysis of SCN1B was performed on 402 individuals with various epilepsy syndromes. Four probands with missense mutations were identified. Detailed electroclinical phenotyping was performed on all available affected family members including quantitative MR imaging in those with temporal

Temporal lobe epilepsy in children: electroclinical study of 77 cases.

PURPOSE: Temporal lobe epilepsy (TLE) is probably more difficult to recognize in children than in adults. In fact, ictal symptoms in children are less stereotyped and less obvious, and the neuropathological substrate is more heterogeneous than in adults. The aim of this study is to examine the relationships between etiology, age at onset and electroclinical findings in 77 children with TLE, 32 of whom were surgically treated. METHODS: Electroclinical study including video-EEG recording of seizures in 77 children with TLE. The investigation focused on the first five initial ictal symptoms. RE

The fate of old memories after medial temporal lobe damage.

Damage to the hippocampal region and related medial temporal lobe structures (perirhinal, entorhinal, and parahippocampal cortices) impairs new learning (anterograde amnesia) as well as memory for information that was acquired before the damage occurred (retrograde amnesia). We assessed retrograde amnesia with the Autobiographical Memory Interview (AMI) and with a news events test in six patients with damage limited primarily to the hippocampal region (H group) and two patients with large medial temporal lobe lesions (MTL group). On the news event test, the H group exhibited temporally limited

Temporal lobe involvement in Japanese encephalitis: problems in differential diagnosis.

BACKGROUND AND PURPOSE: On MR imaging and CT, Japanese encephalitis (JE) shows lesions in the thalami, substantia nigra, basal ganglia, cerebral cortex, cerebellum, brain stem, and white matter, whereas temporal lobe involvement is characteristically seen in Herpes simplex encephalitis (HSE). Temporal lobe involvement in JE may cause problems in differentiating it from HSE. We undertook this study to show the temporal lobe involvement pattern in JE and highlight differentiating features from temporal lobe involvement in HSE. METHODS: Sixty-two patients with JE underwent CT or MR imaging or bo

Ictal dystonic posturing in mesial versus neocortical temporal lobe seizures.

PURPOSE: Ictal contralateral dystonic posturing is a frequently observed clinical feature in temporal lobe seizures. It is generally interpreted as the result of spread of the ictal discharge into basal ganglia structure. In previous reports, analysing ictal behavior, a precise definition and description of the upper limb ictal dystonia is often lacking or contradictory. In our study we aimed to determine whether different subtypes of dystonia and their latency from the clinical onset of seizure might be of value for the differentiation between mesial temporal lobe epilepsy (MTLE) and neocorti

MR tractography predicts visual field defects following temporal lobe resection.

A superior homonymous quadrantanopia is a well recognized complication of anterior temporal lobe resection and occurs because of disruption of the Meyer loop, the anterior part of the optic radiation. The authors used diffusion tensor imaging tractography to visualize the optic radiation before and after surgery, demonstrating the disruption of Meyer loop in a patient who developed a quadrantanopia. Preoperative imaging of the optic radiation will be useful in predicting visual field defects following temporal lobe resection.

Temporal lobe intraparenchymal retained foreign body from remote orbital trauma.

We present the case of a 35-year-old man with a new onset of seizure disorder who was found to harbor an intraparenchymal retained foreign body related to remote orbital trauma. Imaging revealed a rim-enhancing anterior medial temporal lobe mass. Histologic evaluation of the resected mass showed evidence of acute and chronic inflammation with associated foreign material. The patient described a "bar fight" 16 years previously in which he received a blow to the orbit with a pool cue stick. The diagnosis of a foreign body reaction should be considered when an intraparenchymal mass is identified

[Magnetic resonance imaging (MRI) findings of temporal lobe radiation encephalopathy].

BACKGROUND & OBJECTIVE: The imaging appearance of radiation encephalopathy (REP) was widely reported. However, there were few researches on imaging multiplicity of REP. The current study was designed to observe the morphological features of REP on magnetic resonance imaging(MRI) and to investigate its diagnostic value. METHODS: The MRI of 160 lesions with the diagnosis of temporal lobe REP on MRI was retrospectively analyzed in 104 patients with nasopharyngeal carcinoma (NPC). The MRI was performed after radiation therapy of NPC with an interval ranged from 8 months to 13 years. The imaging s

Hypertensive encephalopathy: antecedent to hippocampal sclerosis and temporal lobe epilepsy?

The authors describe three patients with refractory temporal lobe epilepsy (TLE) following an episode of hypertensive encephalopathy as their only identified antecedent event. All patients had typical MR features of hippocampal sclerosis (HS), and the two operated cases had typical HS histology and became seizure-free postoperatively. These cases suggest that hypertensive encephalopathy may be a rare form of initial precipitating injury, leading to TLE and HS.

Functional changes in temporal lobe activity during transient global amnesia.

The integrity of temporal lobe activity during and after recovery from transient global amnesia (TGA) was assessed in a case study using functional MRI. TGA was associated with scene-encoding deficits in a temporolimbic circuit that recovered over time. Frontoparietal areas recruited during the amnesic state may signify a compensatory reliance on visuospatial or working memory strategies. Reduction of extrastriate cortex responses over repeated testing sessions possibly indicates intact visual priming in TGA.

Decrease of sympathetic cardiovascular modulation after temporal lobe epilepsy surgery.

In temporal lobe epilepsy (TLE), there is evidence of ictal and interictal autonomic dysregulation, predominantly with sympathetic overactivity. The effects of TLE surgery on autonomic cardiovascular control and on baroreflex sensitivity (BRS) have not been studied. To evaluate such effects, we monitored heart rate (HR), systolic blood pressure (BP(sys)) and respiration in 18 TLE patients 3-4 months before and after TLE surgery. We used Blackman-Tukey spectral analysis to assess sympathetic and parasympathetic modulation as powers of HR and BP(sys) oscillations in the low frequency (LF, 0.04-0

Twins with different temporal lobe malformations: schizencephaly and arachnoid cyst.

The etiology and relationships between different forms of malformations of cortical development are poorly understood. Schizencephaly is generally regarded as unrelated to arachnoid cysts. As part of a systematic study of epilepsy in twins we observed a monozygotic twin pair discordant for temporal lobe epilepsy where the twin with epilepsy had unilateral temporal schizencephaly and periventricular heterotopia. The twin without epilepsy had an arachnoid cyst in the same temporal lobe. Although an incidental association is possible, this observation, together with occasional reports of schizenc

[A case of deep sylvian meningioma presenting temporal lobe epilepsy].

A rare case of deep sylvian meningioma is presented. A 62-year-old woman was admitted to our hospital because of one year history of temporal lobe epilepsy. She had no neurological deficit except for EEG abnormality. CT scans showed a small calcified mass in the left temporal lobe adjacent to the sylvian fissure with no enhancement by contrast medium. The mass was low-intense in both T1- and T2-weighted MR images. The T1-weighted image after the infusion of gadolinium revealed enhancement of the middle cerebral artery adjacent to the mass, similar to dural tail sign. Left external carotid angi
