Baseline synovitis-tenosynovitis is associated with remission in early rheumatoid arthritis, but discordance with disease activity is a changeable state.

OBJECTIVES: The objectives of this study were to investigate the association between baseline joint-complex inflammation [power Doppler-detected joint synovitis (PDUS) and/or tenosynovitis (PDTS)] and remission in treatment-naïve, new-onset RA patients and to evaluate concordance and discordance states between clinical disease activity and power Doppler US and transition between these states longitudinally. METHODS: At baseline, treatment-naïve early RA patients from a randomized controlled trial were categorized according to dominant hand PDUS and/or PDTS presence into four groups (PDUS+PDTS…

First report of Mycobacterium virginiense-induced synovitis and tenosynovitis of flexor digitorum superficialis and profundus muscles in Japan.

Mycobacterium virginiense, a species of the Mycobacterium terrae complex, was first identified in 2016. Although M. virginiense has only been reported to cause tenosynovitis, there have been only a few reports. Moreover, there is no established standard treatment, and no cases of M. virginiense infection have been reported in Japan. A 70-year-old Japanese man with a history of hand injury and wound contamination was diagnosed with synovitis and tenosynovitis of the left flexor digitorum superficialis and profundus muscles. M. virginiense was detected in perisynovial reservoirs and surgically r…

Anomalous course and stenosing tenosynovitis of the extensor pollicis longus tendon at the radial styloid process: A case report.

RATIONALE: Anomalous course and tenosynovitis of extensor pollicis longus (EPL) tendon is a rare condition that presents clinical symptoms very similar to de Quervain's disease. Herein we report a case of anomalous course and tenosynovitis of the extensor pollicis longus (EPL) tendon associated with symptoms of de Quervain's disease. PATIENT CONCERNS: A 44-year-old right-handed man visited the clinic because of radial pain associated with the left wrist, which was aggravated during the previous 10 days. The patient tested positive on the Finkelstein's test and displayed a limited range of mot…

Tuberculous Tenosynovitis of the Wrist and the Hand: The 3 Anatomo- Clinical Forms Described by Kanavel (About 4 Cases).

INTRODUCTION: Tuberculous tenosynovitis localization is rare. This unusual presentation, often misunderstood, results in frequent misdiagnosis. We report 4 cases of patients which represent the 3 anatomo-clinical forms described by Kanavel. The knowledge of its different manifestations will make it possible to recognize and to suspect the tuberculous origin. CASE REPORT: This was a retrospective study from January 2006 to May 2017 which involved all patients received and treated with tuberculous tenosynovitis. Of the 4 case studies presented here, there were 3 men and 1 woman, aged, respectiv…

The presence of limited joint mobility is significantly associated with multiple digit involvement by stenosing flexor tenosynovitis in diabetics.

OBJECTIVE: Diabetes is associated with several disorders of the hand, including stenosing flexor tenosynovitis (SFTS). The feature of SFTS in diabetics is a higher prevalence of multiple digit involvement. We examined the magnitude of the tendency for involvement of more digits by SFTS in diabetic patients than in nondiabetic patients, and attempted to clarify the factors influencing multiple digit involvement by SFTS in diabetics. METHODS: The study comprised 302 diabetic patients with SFTS and 235 nondiabetic patients with SFTS. The total number of digits exhibiting SFTS within the 1-year p…

Arthritis, tenosynovitis, fasciitis, and bursitis due to sea urchin spines. A series of 12 cases in Réunion Island.

UNLABELLED: Arthritis, tenosynovitis, and bursitis due to sea urchin spine injuries have unique pathological features and run a chronic course until the spines are removed. Of the 40 cases of sea urchin spine-related clinical symptoms published to date, only 12 had osteoarticular symptoms. PATIENTS AND METHODS: We studied 12 cases with osteoarticular symptoms seen in Réunion Island from 1994 to 1998. There were nine cases of arthritis and one case each of tenosynovitis, fasciitis, and bursitis. The nine males and three females had an age range of 9 to 50 years. RESULTS: The injury was at the…

Severe tenosynovitis with rapidly fluctuating power Doppler activity, a clue for the diagnosis of Blau syndrome: a case report.

BACKGROUND: Blau Syndrome is a rare monogenic disorder characterized by granulomatous polyarthritis, dermatitis, and uveitis. The diagnosis can be challenging as symptoms may not always align with the classic triad. CASE DESCRIPTION: An 8-year-old girl presented with fluctuant swellings in the wrists and ankles, strength reduction and stiffness. Blood tests showed lymphopenia, elevated inflammation markers and positivity of anti-nuclear antibodies. Ultrasound revealed severe tenosynovitis with no power Doppler signal. After fifteen days, she developed fever and widespread joint pain. Laborato…

Bilateral gonococcal septic tenosynovitis and septic arthritis as a first presentation of HIV in a 53-year-old male.

Infection with Neisseria gonorrhoeae is common, primarily affecting the urogenital system. Disseminated gonococcal infection (DGI) through the bloodstream is much less common but has important ramifications on the musculoskeletal system, as it can infect tendon sheaths and joints. We describe a case of bilateral gonococcal septic tenosynovitis and septic arthritis with contiguous osteomyelitis in the ankles/feet of a 53-year-old male whose presentation triggered an immunodeficiency workup revealing HIV. This case highlights challenges associated with the diagnosis of DGI, the need for heighten…

De Quervain's Tenosynovitis Release With Excision of the First Dorsal Compartment: Novel Surgical Technique and a Case Series.

Incision of the dorsal side of the tendon sheath in release of De Quervain's tenosynovitis has traditionally been advocated to prevent the risk of volar tendon subluxation. We describe a novel technique of complete excision, rather than simple incision, of the first dorsal compartment tendon sheath. Over a 10-year period, 147 patients (154 wrists) underwent first dorsal compartment release using this technique of complete excision of the sheath. No postoperative immobilization is used. Patients were followed for a mean of 7.0 months. Records were assessed for any complications including reoper…

A Comparison of the Results of Platelet-Rich Plasma Injection Versus the Results of Corticosteroid Injections in De-Quervain Tenosynovitis.

BACKGROUND: De Quervain tenosynovitis (DQT) is a condition that affects the first extensor compartment of the wrist, resulting in stenosing tenosynovitis. This work aimed to evaluate the effects of platelet-rich plasma (PRP) injection in the treatment of DQT in comparison with corticosteroid (CS) injections. METHODS: This study was carried out on 40 DQT patients aged above 18 years old of both sexes, based on a combination of clinical symptoms and signs including persistent tenderness on the radial styloid, swelling on the radial styloid, positive provocative tests such as the Finkelstein tes…

Results of surgical treatment of De Quervain's tenosynovitis: 80 cases with a mean follow-up of 9.5 years.

INTRODUCTION: Surgery is indicated in De Quervain's tenosynovitis only after failure of medical treatment, often due to individual anatomical variants. We use Le Viet's technique, to avoid tendon instability. The aim of the present study was to evaluate long-term results, with the hypothesis that this surgical technique is reliable, providing lasting results. PATIENTS AND METHODS: All patients operated on between 1993 and 2015 were included, and results were assessed by telephone questionnaire at a minimum 1-year's follow-up. Surgical technique was systematically as described by Le Viet, with…

Gouty Tenosynovitis of the Distal Biceps Tendon Insertion Complicated by Partial Rupture: First Case and Review of the Literature.

Background: Given the appreciable prevalence of gout, gout-induced tendon ruptures in the upper extremity are extremely rare. Although these events have been reported only 5 times in the literature, all in patients with a risk factor for or history of gout, they have conspicuously never been diagnosed in the shoulder or elbow. Methods: A 45-year-old, right-hand-dominant man with a history of gout presented with pain in his right anterior elbow and weakness in his forearm after a trivial injury. Results: Here, we report the first case of gouty tenosynovitis of the distal biceps tendon insertion…
