Fifty years of thoracic aortic surgery: lessons learned and future directions.

During the past 50 years, the genetic basis and natural history of aortic disease has been defined. Surgical methods evolved to reduce mortality and morbidity from bleeding, renal impairment, cerebral injury, and paraplegia. Aortic surgery is now a specialty in itself. Experienced groups achieve a mortality rate of less than 2% for root operations and less than 15% for arch surgery and aortic dissection. The introduction of stent grafts has changed the approach to vascular pathology. These are less intimidating for the patient but have unsolved risks with uncertain long-term outcome. In the fu

Epicardial electrode insertion by means of video-assisted thoracic surgery technique.

BACKGROUND: The number of patients who depend on pacemaker (PM) insertion is steadily increasing. Slovenia has two centers for PM insertion, … f the superior vena cava compelled us to insert an epicardial permanent pacing lead. Video-assisted thoracic surgery (VATS) technique was selected. RESULTS: Surgery was safe for the patient, of 40 minutes' duration, and with minimum blood loss. The postoperative course was devoid of complications, the patient's circulation was stable, and he was able to leave the …

Thoracic electrical bioimpedance versus thermodilution in patients post open-heart surgery.

Thoracic electrical bioimpedance cardiography is a non-invasive, continuous and low-cost method of estimation of cardiac output and other haemodynamic parameters. Though subject to continuous technological refinement controversial opinions exist on its validity in subsets of critically ill patients, patients with heart disease or after cardiac surgery. A comparison study between thermodilution (TD) and bioimpedance (TEB) was performed in 28 patients undergoing elective cardiac surgery (CABG, aortic or mitral valve replacement or combined procedures). 128 pairs of cardiac index estimates at spe

Surgery for suspected neurogenic thoracic outlet syndromes: a follow up study.

OBJECTIVES: To assess the outcome of surgical treatment for thoracic outlet syndrome (TOS), and to compare the outcome in patients with and without an underlying cervical rib. METHODS: a heterogeneous group of 40 patients (33 women, seven men; aged 22-62 years) were evaluated 3 months to 20 years after surgery for suspected neurogenic TOS. Forty nine operations had been performed: cervical ribs were removed in 23 patients, together with fibrous band excision in nine. In the 17 without a cervical rib the thoracic outlet was decompressed by resection of the first thoracic rib in nine, and by ot

Incidence of arrhythmias after thoracic surgery: thoracotomy versus video-assisted thoracoscopy.

PURPOSE: Atrial arrhythmias, especially supraventricular tachycardia (SVT) and atrial fibrillation, are common after thoracotomy and lung surgery. There are few existing data on the incidence of postoperative arrhythmias after video-assisted thoracoscopy (VAT). The purpose of the present investigation was to retrospectively determine the incidence of postoperative arrhythmias in patients who underwent VAT compared with those who underwent thoracotomy, and which factors are associated with an increased risk for arrhythmias in both groups. DESIGN: A retrospective investigation. SETTING: A metr

Simultaneously stapled lobectomy: a safe technique for video-assisted thoracic surgery.

Currently, techniques for video-assisted thoracic surgery are being borrowed from the open conventional thoracotomy. However, these same techniques have made video-assisted lobectomy difficult, burdensome, and even dangerous. Simultaneously stapled lobectomy (simultaneous stapling of all hilar structures in their natural anatomic configuration) has been performed successfully in 16 patients. Every attempted simultaneously stapled lobectomy is included. The lesions included 14 malignant tumors, one giant benign pulmonary cyst, and one large necrotizing granuloma. Three right upper lobes, one ri…

Functional outcome after recurrent laryngeal nerve injury following thoracic aortic surgery.

OBJECTIVES: Recurrent laryngeal nerve injury is a recognized complication of aortic surgery, yet data on long-term functional outcomes remain limited. This study aimed to evaluate the incidence of recurrent laryngeal nerve injury and to assess voice recovery after thoracic aortic surgery. METHODS: Single-centre retrospective analysis included adult patients undergoing aortic arch or thoracoabdominal aortic surgery between 2001 and 2024. Recurrent laryngeal nerve injury was diagnosed by postoperative vocal cord paralysis. Aortic arch surgery included partial and total arch replacement, while t

Multiport Versus Single-Port Video-Assisted Thoracic Surgery for Pulmonary Resection.

OBJECTIVES: There have been many attempts to compare Uniportal Video-Assisted Thoracic Surgery (UVATS) with Multiportal VATS (MVATS) for anatomic lung resection. A review is warranted to appreciate: what has been learnt about the relative benefits of the 2 approaches; what lessons have been learned by the process of comparing approaches; and whether such comparisons should influence surgical practice. METHODS: A narrative review and qualitative analysis of the literature pertaining to UVATS and especially to comparisons between UVATS and MVATS was conducted. RESULTS: The balance of currently…

Postoperative neurological deficits following thoracic decompression surgery: incidence, trajectory, and resolution.

PURPOSE: To characterize neurological outcomes after thoracic decompression and to identify patient- and procedure-level factors associated with persistent postoperative neurological deficits across a multi-etiology cohort, with explicit pathology-stratified analysis. METHODS: We retrospectively reviewed 255 patients who underwent thoracic decompression from 2016 to 2023 for degenerative disease (n = 102), trauma (n = 53), infection (n = 37), tumor (n = 28), or deformity (n = 35), classified under a mutually exclusive primary-indication scheme. Neurological deficits were assessed pre- and pos

Non-Woven Haemostatic Agent Mimicking Perigraft Abscess Following Thoracic Aortic Surgery.

Background and Clinical Significance: Absorbable haemostatic agents such as Surgicel® Fibrillar are useful adjuncts to control post-surgical bleeding in cardiac surgery. The material is purposefully left in situ and it slowly degrades over time. Previous publications, mainly in general and gynaecological surgery, have demonstrated that these materials can mimic gangrenous infection, abscesses, anastomotic leak, and early tumour recurrence in imaging studies. These findings can often lead to unnecessary re-interventions or re-operations. The number of reports in the cardiothoracic surgical fiel

Thoracic Packing Failure due to Occult Diaphragmatic Injury after Empyema Surgery.

INTRODUCTION: Intrathoracic gauze packing is an established damage control technique for life-threatening thoracic hemorrhage in trauma settings. However, its role in nontraumatic coagulopathic bleeding remains unclear, and the mechanisms underlying packing failure have not been well characterized. We report a case in which intrathoracic packing failed due to an unrecognized diaphragmatic injury, highlighting an important limitation of this strategy. CASE PRESENTATION: An 82-year-old man with chronic kidney disease, diabetes mellitus, and hypoalbuminemia underwent emergency video-assisted tho

[Establishing Bronchial and Vascular Reconstruction in Robot-assisted Pulmonary Thoracic Surgery].

Robot-assisted thoracic surgery has enabled safe and precise surgical procedures by providing a high-definition three-dimensional visual field and highly dexterous instrument manipulation. At our institution, we have actively performed robot-assisted thoracic surgery, including complex segmentectomy, with a continuous emphasis on establishing safe and reliable surgical techniques. In recent years, with the widespread adoption of neoadjuvant immunotherapy, the number of surgical resections for advanced lung cancer has increased, and reports of robot-assisted high-complexity reconstructions, suc…

Extracorporeal Membrane Oxygenation-Assisted Thoracic Surgery: A Series of 10 Cases.

INTRODUCTION: Extracorporeal membrane oxygenation (ECMO) is a type of extracorporeal circulation used to divert blood from and deliver blood to peripheral blood vessels. Recently, the use of ECMO has been reported in various non-transplant surgeries. Particularly in tracheal surgeries, ECMO provides an unobstructed surgical field and enables safe induction of general anesthesia in difficult intubation cases. Here, we report on 10 cases of thoracic surgery in which ECMO was employed at our institution. CASE PRESENTATION: These 10 cases comprise 4 tracheal cancer surgeries, 2 lung cancer surger…

Devastating Neurological Outcome Following a Subarachnoid-Pleural Fistula During Thoracic Surgery.

Pneumocephalus due to a subarachnoid-pleural fistula (SPF) has previously been described in the literature and is a rare complication following thoracic surgery. In this report, we discuss a patient who developed profound neurologic sequelae following right-sided pneumonectomy which was complicated by T2 nerve root avulsion and SPF development. The patient returned to the OR on postoperative day 21 in the setting of significant neurologic deterioration secondary to intracranial hypotension and pneumocephalus for SPF closure via thoracic laminectomy in the prone position. We present a rare caus…

Factors related to post-thoracotomy pain following robotic-assisted thoracic surgery.

BACKGROUND: Robotic-assisted thoracic surgery (RATS) is a minimally invasive procedure; however, some patients experience persistent postoperative pain. This study aimed to investigate factors related to postoperative pain following RATS. METHODS: The data of 145 patients with lung cancer, who underwent RATS with a four-port (one in the sixth intercostal space [ICS] and three in the eighth ICS) lobectomy or segmentectomy between May 2019 and December 2022, were retrospectively analyzed. Factors associated with analgesic use for at least 2 months following postoperative pain (PTP group) were a…

Risk factors for discontinuing intravenous patient-controlled analgesia after thoracic surgery.

PURPOSE: This study examined the risk factors of experiencing side effects from using intravenous patient-controlled analgesia (IV PCA) following lung and esophageal surgery. METHODS: Our study included adult patients who underwent lung or esophageal surgery and received IV PCA for postoperative acute pain control between 2020 and 2022. We collected information on side effects from IV PCA use, the decision to discontinue PCA, and the PCA regimen from the daily reports of the acute pain management team and verified the accuracy using electronic records from ward nurses. The primary outcome was
