Refractory chylothorax caused by massive thymic hyperplasia in an infant: thoracoscopic thymectomy.

INTRODUCTION: Spontaneous chylothorax in infancy is most commonly associated with congenital lymphatic malformations or iatrogenic injury. Massive thymic hyperplasia (MTH) causing refractory chylothorax through extrinsic compression is rare. We report an infant with severe chylothorax secondary to MTH, successfully managed with thymectomy. CASE PRESENTATION: A 2-month-old female infant presented with respiratory distress and recurrent pleural effusions. Thoracentesis yielded milky chylous fluid with a triglyceride level of 19.8 mmol/L. Chest MRI demonstrated a homogeneous anterior mediastinal…

Progressive encephalomyelitis with rigidity and myoclonus after thymectomy in a woman with myasthenia gravis.

Progressive encephalomyelitis with rigidity and myoclonus (PERM), part of the spectrum of stiff-person syndrome (SPS), is a rare neurological disorder characterized by axial and limb rigidity, painful muscle spasms, hyperekplexia, brainstem signs, and autonomic dysfunction. Here, we present the case of a 44-year-old woman with a history of myasthenia gravis (MG) who had previously undergone a thymectomy. She presented with a 20-day history of startle-induced episodes of generalized rigidity and painful spasms affecting her face, trunk, and limbs. Her symptoms began gradually, initially with nu…

Summary of the best evidence for the prevention and management of myasthenic crisis after thymectomy.

BACKGROUND: Myasthenic crisis (MC) may occur after thymectomy in patients with myasthenia gravis (MG), but effective preventive interventions can reduce the occurrence of this complication. Previous research on MC focused on risk factors, emergency treatment, etc., which was relatively scattered and did not form a comprehensive management framework. This study sought to retrieve and summarize the relevant evidence on the prevention and management of postoperative MC to provide a theoretical reference for clinical medical staff. METHODS: According to the evidence pyramid model, relevant articl…

Anesthetic Management of Robotic Thymectomy in a Patient With Morvan Syndrome: A Case Report.

Morvan syndrome (MvS) is a rare acquired paraneoplastic autoimmune neuromyotonia with central and autonomic nervous system involvement that has been incompletely described in the literature. We describe the successful administration of general anesthesia for robotic thymectomy to an MvS patient with severe encephalopathy, cardiac dysautonomia, and peripheral nerve hyperexcitation. Importantly, thymus removal provided effective source control with eventual resolution of MvS symptoms. MvS is briefly reviewed and novel observations are described of related interactions between nondepolarizing neu…

Post-thymectomy myasthenia gravis: a case report and systematic review of literature.

Myasthenia gravis (MG) is an autoimmune condition affecting the neuromuscular junction characterised by weakness and fatiguability, carrying a high morbidity if treatment is delayed. A clear association with thymoma has led to management with thymectomy as a common practice, but MG presenting post-thymectomy has rarely been reported. We present a case of an 82- year-old woman developing fatigue, ptosis and dysarthria 3 months after thymectomy. After a clinical diagnosis of MG was made, she responded well to prompt treatment with prednisolone and pyridostigmine. Her anti-acetylcholine receptor …

Transcervical Thymectomy Is the Most Cost-Effective Surgical Approach in Myasthenia Gravis.

BACKGROUND: Extended thymectomy has been proven to improve the course of myasthenia gravis. Retrospective studies demonstrate that several techniques for thymectomy achieve overlapping remission rates. We therefore compared perioperative outcomes and costs among 3 approaches to thymectomy: sternotomy, video and/or robot assisted, and transcervical. METHODS: To ensure similar study groups, we excluded patients with >4 cm or invasive tumors and those who underwent less than an extended thymectomy or concurrent procedures. Hospital costs were collected and analyzed by blinded finance personnel. …

[Perioperative challenges and neuromuscular blockade concerns in robotic thymectomy for myasthenia gravis].

Myasthenia Gravis (MG) is an autoimmune disease characterized by weakness and fatigability of skeletal muscles, with improvement following rest. It is a disease of great significance to the anesthesiologist because it affects the neuromuscular junction. Robotic thymectomy has come up in recent times due to the minimally invasive nature and its advantages. This presents a new set of challenges for the anesthesia team, and here we present the various anesthesia considerations and perioperative management in a series of 20 patients who underwent robotic thymectomy. As it is a recent upcoming proc…

A Neurologist's Perspective on Thymectomy for Myasthenia Gravis: Current Perspective and Future Trials.

The first randomized blinded study of thymectomy in nonthymomatous myasthenia gravis was designed to answer 3 questions: does the combination of prednisone and removal of the thymus gland via extended transsternal thymectomy after 3 years compared with an identical dosing protocol of prednisone alone (1) lead to better disease status for generalized MG patients with antiacetylcholine receptor antibodies, (2) reduce their prednisone requirements, and/or (3) reduce the side-effect burden from medications used to treat the disease? The study demonstrated that thymectomy confers these benefits for…
