Secondary lymphoid tissue and costimulation-blockade resistant rejection: A nonhuman primate renal transplant study.

Naïve T cell activation requires antigen presentation combined with costimulation through CD28, both of which optimally occur in secondary lymphoid tissues such as lymph nodes and the spleen. Belatacept impairs CD28 costimulation by binding its ligands, CD80 and CD86, and in doing so, impairs de novo alloimmune responses. However, in most patients belatacept is ineffective in preventing allograft rejection when used as a monotherapy, and adjuvant therapy is required for control of costimulation-blockade resistant rejection (CoBRR). In rodent models, impaired access to secondary lymphoid tissue

Host Lymphotoxin-β Receptor Signaling Is Crucial for Angiogenesis of Metanephric Tissue Transplanted into Lymphoid Sites.

The mouse lymph node (LN) can provide a niche to grow metanephric kidney to maturity. Here, we show that signaling through the lymphotoxin-β receptor (LTβR) is critical for kidney organogenesis both in the LN and the omentum. By transplanting kidney rudiments either in the LNs of mice undergoing LTβR antagonist treatment or in the omenta of Ltbr knockout (Ltbr-/-) mice, the host LTβR signals were found to be crucial for obtaining a well-vascularized kidney graft. Indeed, defective LTβR signaling correlated with decreased expression of endothelial and angiogenic markers in kidney grafts as well

West Nile virus RNA in tissues from donor associated with transmission to organ transplant recipients.

We identified West Nile virus (WNV) RNA in skin, fat, muscle, tendon, and bone marrow from a deceased donor associated with WNV transmission through solid organ transplantation. WNV could not be cultured from the RNA-positive tissues. Further studies are needed to determine if WNV can be transmitted from postmortem tissues.

Mesenchymal stem cells and progenitor cells in connective tissue engineering and regenerative medicine: is there a future for transplantation?

PURPOSE: Transplantation surgery suffers from a shortage of donor organs worldwide. Cell injection and tissue engineering (TE), thus emerge as alternative therapy options. The purpose of this article is to review the progress of TE technology, focusing on mesenchymal stem cells (MSC) as a cell source for artificial functional tissue. RESULTS: MSC from many different sources can be minimally invasively harvested: peripheral blood, fat tissue, bone marrow, amniotic fluid, cord blood. In comparison to embryonic stem cells (ESC), there are no ethical concerns; MSC can be extracted from autologous

Treatment of chronic anal fissures and associated stenosis by autologous adipose tissue transplant: a pilot study.

PURPOSE: A substantial body of recent evidence suggests that autologous adipose tissue transplant promotes healing in different lesions associated with local ischemia. We report the outcome of lipoaspirate transplant in hard-to-treat chronic anal fissures. METHODS: Eight patients were included in this pilot study, 5 with single and 3 with multiple fissures. All had intense anal pain and had previously undergone proctologic surgery, with internal sphincterotomy performed in 6 patients. Severe stenosis was present in 3 patients and moderate stenosis in 2. Preoperative assessment included anosco

European homograft bank: twenty years of cardiovascular tissue banking and collaboration with transplant coordination in Europe.

Established in 1989 in Brussels as an international nonprofit association, the European Homograft Bank (EHB) has been collaborating closely with the transplant coordination of the different centers in Belgium and other European countries. Donor selection is made after discussion of exclusion criteria with the transplant coordinator of the procurement center. EHB collaborates with 15 Belgian, 11 German, 10 French, 10 Swiss, 3 Italian, 3 Dutch, and some other procurement and/or implantation centers. Donor ages range from newborn to 65 years. Tissue preparation, morphologic evaluation, and functi

National Association of Medical Examiners position paper on the medical examiner release of organs and tissues for transplantation.

The medical examiner community plays a key role in the organ and tissue procurement process for transplantation. Since many, if not most, potential organ or tissue donors fall under medicolegal jurisdiction, the medical examiner bears responsibility to authorize or deny the procurement of organs or tissues on a case-by-case basis. This responsibility engenders a basic dichotomy for the medical examiner's decision-making process. In cases falling under his/her jurisdiction, the medical examiner must balance the medicolegal responsibility centered on the decedent with the societal responsibility

Widespread nonhematopoietic tissue distribution by transplanted human progenitor cells with high aldehyde dehydrogenase activity.

Transplanted adult progenitor cells distribute to peripheral organs and can promote endogenous cellular repair in damaged tissues. However, development of cell-based regenerative therapies has been hindered by the lack of preclinical models to efficiently assess multiple organ distribution and difficulty defining human cells with regenerative function. After transplantation into beta-glucuronidase (GUSB)-deficient NOD/SCID/mucopolysaccharidosis type VII mice, we characterized the distribution of lineage-depleted human umbilical cord blood-derived cells purified by selection using high aldehyde

Artificial neural network and tissue genotyping of hepatocellular carcinoma in liver-transplant recipients: prediction of recurrence.

BACKGROUND: Liver transplantation (LT) is the treatment of choice for early stage hepatocellular carcinoma (HCC) with excellent 5-year survival, with a recurrence rate after LT of 3.4%. An artificial neural network (ANN), combined with genotyping for microsatellite mutations/deletions (TM-GTP), was designed at the University of Pittsburgh to predict tumor recurrence with a discriminatory power of 85%. This study aims to validate the ANN/TM-GTP model on patients receiving transplants in a single center. METHODS: Nineteen patients with HCC underwent LT at our center between 1999 and 2002 (mean

Molecular imaging using labeled donor tissues reveals patterns of engraftment, rejection, and survival in transplantation.

Tissue regeneration and transplantation of solid organs involve complex processes that can only be studied in the context of the living organism, and methods of analyzing these processes in vivo are essential for development of effective transplantation and regeneration procedures. We utilized in vivo bioluminescence imaging (BLI) to noninvasively visualize engraftment, survival, and rejection of transplanted tissues from a transgenic donor mouse that constitutively expresses luciferase. Dynamic early events of hematopoietic reconstitution were accessible and engraftment from as few as 200 tra

Autologous fat transplants influence compensatory white adipose tissue mass increases after lipectomy.

Direct tests of the hypothesized total body fat regulatory system have been accomplished by partial surgical lipectomy. This usually results in the restoration of the lipid deficit through compensatory increases in nonexcised white adipose tissue (WAT) masses of ground squirrels, laboratory rats, and mice, as well as Siberian and Syrian hamsters. We challenged this hypothesized total body fat regulatory system by testing the response of Siberian hamsters to 1) lipid deficits [lipectomy; primarily bilateral epididymal WAT (EWAT) removal], 2) lipid surfeits (addition of donor EWAT with no lipect

Treatment of chronic tibial osteomyelitis, segmental bone loss, and soft tissue defect by bone transplant.

New methods of treatment have greatly improved the success of limb salvage in severe lower extremity trauma. We report the use of a microvascular free tissue transfer and distraction osteogenesis in the reconstruction of soft tissue and bone defects complicated by infection. Soft tissue coverage and bony stabilization by external fixation resulted in eradication of the infection. The external fixator also allowed bone transport and regeneration to overcome a bone defect of approximately 6 cm. Bone transportation can be accomplished in a distal to proximal direction if the situation necessitate

Long-term survival of human fetal pancreatic tissue transplanted into an insulin-dependent diabetic patient.

The explants of two human fetal pancreases of 15 weeks gestational age were cultured for 6-7 days before being implanted in a 29-year-old insulin-dependent diabetic woman who had received a renal graft two months previously. One pancreas was placed in the flexor muscles of the forearm whilst the other was implanted in an omental pouch. To reduce the chances of rejection the tissue was cultured in vitro, the donor of the tissue placed in the forearm was DR antigen matched with the recipient and the patient remained on cyclosporin and prednisone therapy. At 3 months a mass developed in the forea

[Combined cell transplant based upon autologic multipotent stromal cells of adipose tissue use in patients with distinct deficit of bone tissue in maxillae region].

Use of synthetic osteoplastic materials not always leads to formation of the necessary bone tissue volume. The alternative high technological implantation material can become tissue engineering constructions containing osteogenic precursor cells. Clinical observation of sinus lifting with the use of tissue engineering construction demonstrates the efficacy, shortening the terms of operation wound healing, young bone tissue formation after transplantation and possibility to install implants already after 3 months if the amount of bone is sufficient for primary fixation.

Transplants of fresh and cryopreserved autologous adipose tissue improve reintegrative phenomena in newly formed muscle tissue in the reconstruction of muscle volume loss: A histological evaluation.

Repair of volumetric muscle loss (VML) lesions tends to be poor, leading to fibrosis and functional loss. Here we investigated the effectiveness of fresh versus cryopreserved autologous adipose tissue transplantation to facilitate repair. Wistar rats were distributed into four experimental groups: normal control, VML lesions, VML lesions treated with a fresh autologous adipose tissue graft, and VML treated with a cryopreserved autologous adipose tissue graft. At 60 days post-intervention, a histological, histochemical, and immunohistochemical analysis was performed to assess changes related to…

Microplastic contamination in kidney transplant donor tissues and its association with early post-transplant blood pressure changes in recipients: A multi-modal analytical study.

Microplastic (MP) contamination in human tissues is an emerging environmental and clinical concern, yet its relevance to organ transplantation remains poorly understood. This study characterized MPs in kidney transplantation-associated donor tissues and examined their relationship with early postoperative blood pressure (BP) in recipients. Tissue samples, including kidney, adrenal gland, renal artery, renal vein, and ureter, were collected from 28 kidney transplant donors. MPs were extracted from each tissue and analyzed using a multimodal strategy: pyrolysis-gas chromatography-mass spectromet

Subcutaneous Adipose Tissue Attenuation Change on Chest CT Is Associated with Overall Survival After Allogeneic Hematopoietic Stem Cell Transplantation.

OBJECTIVE: Early chest computed tomography (CT) examinations are frequently performed after allogeneic hematopoietic stem cell transplantation (allo-HSCT) due to respiratory symptoms or clinical deterioration. Subcutaneous adipose tissue attenuation (SATA) has recently been proposed as a quantitative imaging marker of systemic tissue alterations related to fluid redistribution and inflammatory processes. This study aimed to evaluate interval changes in SATA after allo-HSCT and to investigate their association with overall survival. MATERIAL AND METHODS: In this retrospective single-center stu
