The records of a total of 427 eyes with various types of glaucoma treated by trabeculectomy were compiled from 10 institutions. The follow-up periods of these cases were from 3 months to 5 years. Simple statistics gave an overall success rate of 75% in controlling the intraocular pressure (IOP) below 21 mmHg. In 344 eyes followed more than 1 year, the IOP distribution showed that the IOP was between 14 and 19 mmHg in 47% of th 10 institutions. The follow-up periods of these cases were from 3 months to 5 years. Simple statistics gave an overall success rate of 75% in controlling the intraocular…

BACKGROUND: Trabeculectomy is the standard surgical treatment for advanced glaucoma, yet the relationship between postoperative intraocular pressure (IOP) and long-term structural preservation remains unclear. We investigated longitudinal changes in peripapillary retinal nerve fiber layer (RNFL) thickness following trabeculectomy in primary open-angle glaucoma and evaluated whether lower postoperative IOP levels are associated with improved structural stability.
METHODS: This retrospective consecutive case series included 106 eyes undergoing trabeculectomy between 2010 and 2020 with follow-up…

BACKGROUND: To propose that possible alteration or damage to the ciliary zonules during uncomplicated endoscopic cyclophotocoagulation (ECP) can cause complications in subsequent filtration surgery.
CASE PRESENTATION: We present two cases with uncomplicated primary combined phacoemulsification and ECP that underwent subsequent trabeculectomy. Both cases were complicated with vitreous prolapse during the trabeculectomy procedure. We review the anatomy of the ciliary zonules and their spatial relationship with the ciliary processes targeted during ECP and propose an association between ECP, zon…

The characteristics of the conjunctival bleb are some of the most important factors for the surgical success of glaucoma filtering surgery. To improve surgical outcome, we investigated bleb histology after 3 different glaucoma surgeries. Surgery was performed in 21 white rabbits. Rabbits were randomized to trabeculectomy or implantation with EX-PRESS or a silicone tube (each n = 7). Bleb survival, intraocular pressure (IOP), and vascularity were evaluated. At 6 weeks, eyes were enucleated for histological analysis. Postoperative IOP at 2 weeks was significantly lower in the trabeculectomy and …

IMPORTANCE: The Tube vs Trabeculectomy Trial (TVT) found that the 350-mm2 Baerveldt implant (tube) and trabeculectomy with mitomycin may be similarly effective in lowering intraocular pressure in primary open-angle glaucoma. However, to date, there are no published long-term clinical data on the cost-effectiveness of trabeculectomy with mitomycin vs tube insertion.
OBJECTIVE: To assess the cost-effectiveness of these procedures compared with maximal medical treatment.
DESIGN, SETTING, AND PARTICIPANTS: We used the Markov cohort model with a 5-year time horizon to study a hypothetical cohort …
