Predictors of PTSD symptoms in adults admitted to a Level I trauma center: a prospective analysis.

Trauma centers are an ideal point of intervention in efforts to prevent posttraumatic stress disorder (PTSD). In order to assist in the development of prevention efforts, this study sought to identify early predictors of PTSD symptoms among adults admitted to a Level I trauma center using a novel analytic strategy (Fournier et al., 2009). Upon admission, participants (N=327) were screened for PTSD symptoms and provided information on potential predictor variables. Their PTSD symptoms were assessed again 3 months later (N=227). Participants were classified as symptomatic (positive PTSD screen) …

Morbidity of pediatric dog bites: a case series at a level one pediatric trauma center.

BACKGROUND: Pediatric dog bite injuries are common and vary in severity. We sought to characterize predisposing factors, required interventions, and morbidity. METHODS: A prospective clinical database at a level one pediatric trauma center was reviewed for dog bite injuries over 74 consecutive months ending December 2013. This included all patients brought in by ambulance and/or seen by the trauma team. RESULTS: Of 650 dog bite incidents, 282 met the criteria for inclusion in the trauma database. Median age was 5 years (range, 2 months to 17 years) and 55% (154/282) of patients were male. Pi

Endovascular repair of acute traumatic aortic injury: experience of a level-1 trauma center.

BACKGROUND: Endovascular repair of the thoracic aorta after traumatic rupture is an alternative to open repair and its use is becoming increasingly widespread. We aimed to determine the concurrent injuries sustained in patients who underwent thoracic endovascular aortic repair (TEVAR) as well as their Glasgow Coma Score (GCS) and Injury Severity Score (ISS). We also aimed to identify the intraoperative and early complications of TEVAR grafting up to 6 months after procedure and in addition identify the late postoperative complications occurring after 6 months after stent. METHODS: Data were c

Pediatric noncontiguous spinal injuries: the 15-year experience at a level 1 trauma center.

STUDY DESIGN: Retrospective review. OBJECTIVE: To determine the incidence and clinical characteristics of noncontiguous spinal injuries (NCSI) in a pediatric population. The secondary objective is to identify high-risk patients requiring further imaging to rule out NCSI. SUMMARY OF BACKGROUND DATA: NCSI can add significant complexity to the diagnosis, management, and outcome of children. There is very little in the pediatric literature examining the nature, associated risk factors, management, and outcomes of NCSI. METHODS: All children up to 18 years of age with a spinal injury, as defined

Major thoracic vessels and cardiac trauma: case series from a center in a developing country.

BACKGROUND: Seventy-five percent of all trauma-related deaths are related to thoracic trauma. Very few penetrating cardiac trauma patients arrive to the hospital alive. Due to its high prevalence, an understanding of the pathogenesis, manifestations and management of cardiac trauma by the medical personnel is becoming increasingly important. METHODS: We retrospectively reviewed the files of 169 patients with a preoperative diagnosis of vascular injury who underwent management at the Aga Khan University Hospital from 2001 to 2006. Of these patients, 13 had cardiovascular and cardiac injuries.

Epidemiological trends of traumatic optic nerve injuries in the largest Canadian adult trauma center.

BACKGROUND: There has been a paucity of information on the epidemiology of traumatic optic neuropathy (TON). This study documents epidemiology of TON over 2 decades in the largest level I adult trauma center in Canada. METHODS: Data on all the trauma patients admitted to Sunnybrook Health Sciences Centre from 1986 to 2007 were collected in a prospective database. The aggregate data on optic nerve injuries including demographic data, etiology, Injury Severity Score (ISS), and associated head and facial injuries were recorded. These were analyzed using univariate and multivariate techniques to

Child abuse and orthopaedic injury patterns: analysis at a level I pediatric trauma center.

BACKGROUND: Child abuse is a serious threat to the physical and psychosocial well-being of the pediatric population. Musculoskeletal injuries are common manifestations of child abuse. There have been multiple studies that have attempted to identify the factors associated with, and the specific injury patterns seen with musculoskeletal trauma from child abuse, yet there have been no large studies that have used prospectively collected data and controlled comparisons. The purpose of our study was to describe the patterns of orthopaedic injury for child abuse cases detected in the large urban are

Brachial artery injuries in a rural catchment trauma center: are the upper and lower extremity the same?

BACKGROUND: The predictors of amputation for patients with lower extremity vascular trauma are well described in the literature, but the predictors of amputation in the upper extremity are not so well defined. We hypothesize that the predictors of amputation in the lower extremity are much different when compared with the upper extremity. METHODS: Retrospective chart review of all brachial artery traumatic injuries presenting to a rural-state university trauma center. RESULTS: In a 6-year period, 41 patients presented with brachial artery injuries. Operative management was performed in 38 (9

Pericardial tamponade type injury: a 17-year study in an urban trauma center in Japan.

We defined injuries to the heart and the pericardium together with hemopericardium as pericardial tamponade type injury regardless of symptoms or signs due to pericardial tamponade. The aim of the study is to examine the important factors related to the diagnosis and treatment of this type of injury. A retrospective chart review was conducted of traumatized patients admitted with hemopericardium to our institution between 1978 and 1995. Ten out of the 19 consecutive patients with pericardial tamponade type injury demonstrated shock and showed a higher Injury Severity Score and mortality (7/11)

A Cross-Sectional Analysis of Inappropriate Sharps Container Utilization at a Level 1 Trauma Center.

BACKGROUND: U.S. hospitals generate ∼6 billion tons of medical waste annually. Disposal costs exceed $4 billion. High-temperature incineration, required for sharp wastes, is the most costly and carbon-intensive disposal. Inappropriate waste contributes significantly to health care's financial and environmental burdens. METHODS: This single-blinded cross-sectional study involved a one-time audit of 176 sharps containers from operating rooms, procedure rooms, and clinics across a Level 1 trauma center, conducted on a single day in 2023. Containers were photographed before disposal. Metadata, su

Predictors of organ injury in pediatric bicycle handlebar trauma: a single-center retrospective study.

PURPOSE: Bicycle handlebar injuries in children can cause severe internal trauma despite a seemingly low-energy mechanism. External signs are unreliable indicators of underlying organ damage, and independent clinical predictors of organ injury have not been systematically evaluated. METHODS: A retrospective review was performed of patients under 18 years presenting with bicycle handlebar injuries to a tertiary pediatric surgery center (June 2023-December 2025). Firth penalized-likelihood logistic regression was used to identify independent predictors of abdominal organ injury, and diagnostic
