Recent advances and unmet needs in the treatment of drug-resistant tuberculosis.

INTRODUCTION: Drug-resistant tuberculosis (DR-TB) still represents a critical global health threat despite recent therapeutic advances. The shift from long, injectable-based regimens to shorter, all-oral regimens has improved outcomes, yet substantial challenges remain in ensuring efficacy, safety, and equitable access. AREAS COVERED: We reviewed recent literature (2020-2025) searching PubMed, Medline, Google Scholar, clinicaltrials.gov, WHO guidelines, conference abstract books focusing on recent policy-changing, or knowledge-advancing Phase II/III clinical trials assessing novel DR-TB drugs

Prevalence and indicators of HIV and AIDS among adults admitted to medical and surgical wards in Blantyre, Malawi.

Despite high seroprevalence there are few recent studies of the effect of HIV on hospitals in sub-Saharan Africa. We examined 1226 consecutive patients admitted during two 2-week periods in October 1999 and January 2000. 70% medical patients were HIV positive, and 45% had AIDS. 36% surgical patients were HIV positive and 8% had AIDS. Seroprevalence rose to a peak among 30-40 year olds; 91% medical, 56% surgical and 80% all patients in this age group were HIV positive. Seropositive women were younger than seropositive men (median age 29 v 35, p<0.0001). Symptoms strongly indicative of HIV were

A clinical review of 128 cases of head and neck tuberculosis presenting over a 10-year period in Bradford, UK.

AIMS: To analyse the epidemiology, presentation and diagnosis of head and neck tuberculosis (TB). METHODS: We conducted a 10-year retrospective study of all cases of tuberculosis of the head and neck region occurring in Bradford, UK. RESULTS: Of a total of 1315 cases of TB, 128 presented with head and neck TB (12 per cent of which (15/128) were in children). Cervical lymph nodes were most commonly involved (87 per cent, 111/128), other sites being: salivary glands (five cases); larynx, oral cavity, eyes and ears (two cases each); and skin, thyroid, nasopharynx and retropharyngeal space (one

Multidrug resistant lymph node fistula tracheobronchial tuberculosis: A case report.

RATIONALE: The patient in this case report has been diagnosed with multidrug resistant lymph node fistula tracheobronchial tuberculosis (TBTB). The PubMed was searched using the keywords "Tuberculosis, Multidrug-Resistant", "Tuberculosis", and "Bronchial Fistula", and the results yielded no similar case reports. Therefore, this report helps us to explore more on the causes of multidrug resistance and formation of lymph node fistula, as well as associated treatment strategies. PATIENT CONCERNS: A 15-year old Tibetan girl who was previously treated in the local Hospital for Infectious Diseases

[DEYCKE PASHA AND THE LÜBECK DISASTER].

Dr. Georg Deycke, known as Deycke Pasha in Turkey, came to Istanbul in 1898 with Prof. Robert Rieder in order to reorganize the Imperial Medical School. Prof Rieder es- tablished a new training hospital named as The Giilhane Clinics and Dr. Deycke worked there until 1904 as the vice director. After Rieder left Turkey, he became the director to the hospital and served until the end of his contract in 1907. So after that time we know very less of his life and works. As well as its almost unknown that he was responsible for the BCG inoculation misfortune in Lübeck in 1930. Oral tuberculosis vacci

Disseminated, fatal reactivation of bovine tuberculosis in a patient treated with adalimumab: a case report and review of the literature.

PURPOSE: Tumor necrosis factor inhibitors (TNFi) are known to increase the risk of tuberculosis (TB) reactivation, though cases involving Mycobacterium bovis are rarely reported. CASE PRESENTATION/RESULTS: We describe a case of disseminated TB with M. bovis in a 78-year-old woman with a negative Interferon-Gamma-Release Assay (IGRA), taking adalimumab due to rheumatoid polyarthritis, which resulted in a fatal outcome. The atypical clinical and histopathological features were initially interpreted as sarcoidosis. The case occurred in Switzerland, an officially bovine tuberculosis-free country.

[CLINICAL INVESTIGATION OF 6 CASES OF TUBERCULOUS SPONDYLITIS].

[Objective] A delay in the diagnosis of tubercu- lous spondylitis can result in worsening of the condition. We investigated previously reported cases of tuberculous spondylitis, as well as cases experienced in our hospital, to identify factors that are useful in the diagnosis. [Materials and Methods] We retrospectively evaluated six cases of tuberculous spondylitis diagnosed in our hospital between October 2007 and September 2012, and an additional 23 cases that had been reported in Japan between 1994 and 2014. [Results] The median age of our six patients was 78.5 years and five were women. In

Progress in the development of tuberculosis vaccines for cattle and wildlife.

Vaccination against bovine tuberculosis is likely to become an important disease control strategy in developing countries, which cannot afford a test and slaughter control programme, or in countries which have a wildlife reservoir of Mycobacterium bovis infection. In the past decade, considerable progress has been made in the development and evaluation of tuberculosis vaccines for cattle and for a range of wildlife maintenance hosts including possums, badgers, deer and African buffaloes. Experimental challenge systems have been established for the different target species and the resulting dis

Laboratory evaluation of a new long-acting 3-azinomethylrifamycin FCE 22250.

FCE 22250 (3-(N-piperidinomethylazino)methylrifamycin SV) is a member of the new class of 3-azinomethylrifamycins characterized by a long persistance in animals, a good oral absorption and a broad antibacterial spectrum including mycobacteria. In the experimental mice infection sustained by Mycobacterium tuberculosis H37Rv, FCE 22250 shows an efficacy 14 times higher than rifampicin and is still therapeutic when administered once every three weeks.

Erythema nodosum.

Erythema nodosum (EN) is now more commonly associated with streptococcal disease than with tuberculosis. Sarcoidosis is the third most frequently associated systemic disease. Oral contraceptives now lead the list of drugs associated with EN. The appearance and disappearance of crops of nodules is extremely variable. Salicylates and rest are useful. Steroids are not recommended because they may compromise defenses against an underlying disease process.

Intravenous use of rifampicin.

Nineteen seriously ill patients with suspected or verified tuberculosis were treated with a new formulation of rifampicin for intravenous use. The normal dosage was 300 mg twice a day. Local reactions at the injection site were noted in four patients but no serious adverse reactions were observed. The serum concentrations were similar to those after the same dose given orally.

Rare presentation of Mycobacterium avium-intracellulare infection.

Mycobacterium avium-intracellulare causes clinical disease mainly in immunocompromised patients with long-standing pulmonary disease, the symptoms of which are identical to those of pulmonary tuberculosis. In patients with AIDS the infection is typically disseminated. Extrapulmonary infection by M. avium-intracellulare is seen in children aged 1-5 years as cervicofacial lymphadenitis. Other extrapulmonary sites are less common and include musculoskeletal, maxillary sinus, mastoid, breast, small bowel, genitourinary tract and cornea. Such infection of the hard palate is rare. We present an unus

Aspirin and ibuprofen enhance pyrazinamide treatment of murine tuberculosis.

OBJECTIVES: Aspirin (acetylsalicylic acid) or ibuprofen [2-(4-isobutyl-phenyl)-propionic acid] was administered to mice undergoing treatment of tuberculosis infection with pyrazinamide to determine if these non-steroidal anti-inflammatory drugs (NSAIDs) enhance pyrazinamide activity in vivo. METHODS: BALB/c mice were infected with Mycobacterium tuberculosis H37Rv through aerosol exposure. Mice were treated orally with aspirin, ibuprofen or pyrazinamide, alone and in combination. The impact of daily administration of these drugs for 1 month was determined by enumerating viable bacteria in the

Malabsorption of antimycobacterial drugs as a cause of treatment failure in tuberculosis.

Malabsorption of oral antimycobacterial drugs is a rare cause of treatment failure in tuberculosis (TB). Several predisposing comorbidities have been recognised. HIV infection is the most important risk factor referred in the literature. There are few reports about antimycobacterial drugs malabsorption, particularly in the absence of predisposing comorbidities. The authors present a clinical case of oral treatment failure in TB due to malabsorption; however, what caused the failure remained unclear. Possible causes of malabsorption are discussed under various sections. Purpose of this case rep

Lyophilised reservoirs in combination with hydrogel-forming microarray patches for transdermal delivery of isoniazid and pyridoxine hydrochloride.

Tuberculosis remains a major global health concern, presenting as either active disease or latent infection, the latter carrying a risk of activation, particularly in immunocompromised individuals. Prolonged isoniazid monotherapy is the standard preventive treatment, often supplemented with pyridoxine to mitigate isoniazid-induced pyridoxine depletion, as recommended by the US Centers for Disease Control and Prevention. This present study investigates an alternative transdermal approach using hydrogel-forming microarray patches (MAPs) incorporating lyophilised isoniazid and pyridoxine wafers.
