[Elbow pain].

Pain and disability in the elbow are not as common as in the neck, shoulder or wrist, for example. The elbow may, however, present disorders that may in a prolonged state be difficult and cause significant loss of working capacity. These include epicondylitis, osteoarthritis and entrapment of the ulnar nerve.

A modified surgical procedure for cubital tunnel syndrome: partial medial epicondylectomy.

Cubital tunnel release with partial medial epicondylectomy was performed in 32 patients with cubital tunnel syndrome unresponsive to conservative management. Twenty-seven patients were available for examination an average follow-up of 13 months. Ninety-three percent were subjectively improved, with results being excellent in 8, good in 10, fair in 8, and poor in 1. Regression analyses showed a statistically significant correlation between outcome and slow ulnar nerve conduction velocity across the elbow, abnormal preoperative 2-point discrimination, abnormal preoperative terminal sensory laten

Is ulnar nerve entrapment at wrist frequent among patients with carpal tunnel syndrome occupationally exposed to monotype wrist movements?

OBJECTIVES: Association between carpal tunnel syndrome (CTS) and ulnar nerve entrapment at wrist remains controversial. The aim of the study has been to investigate the prevalence of Guyon's canal syndrome amongst patients diagnosed with the CTS, occupationally exposed to repetitive wrist movements. MATERIAL AND METHODS: The retrospective analysis of 310 patients (268 females, 42 males) representing the mean age of 52±7 years old hospitalized for the suspected occupational CTS was performed. RESULTS: In the analyzed cohort, 4 patients had undergone decompression of the Guyon's canal in the r

Sensory innervation of the little finger by an anomalous branch of the median nerve associated with recurrent, atypical carpal tunnel syndrome.

Superficial sensory communication between the ulnar and median nerves is well recognized. In the vast majority of cases this communication is from the ulnar nerve to the median nerve. We report a case in which a communicating branch passed from the median nerve to the ulnar nerve immediately proximal to the wrist to supply sensation to the little finger. The presence of this branch correlated with the presence of symptoms which had persisted in spite of conventional open carpal release 7 years earlier. Surgical decompression of this branch led to complete resolution of those symptoms.

Anterior submuscular transposition of the ulnar nerves by the Learmonth technique.

Thirty-eight patients with progressive posttraumatic ulnar neuropathy at the elbow underwent anterior submuscular transposition of their nerves. Multiple mechanical causes of neuropathy were identified. Fourteen patients had undergone previous surgery for ulnar neuropathy, while 24 had not. Postsurgical follow-up averaged 23.1 months. The operative technique is described and illustrated in detail. Complications attributable to surgery were minimal. No absolute prognostic factors could be identified, and even those patients with significant muscular atrophy or time delay before operation were g

Anterior interosseous nerve neuropathy in a patient with spinal cord injury: case report and literature review.

INTRODUCTION: Entrapment neuropathies, typically carpal tunnel syndrome and ulnar neuropathy, frequently occur in patients with spinal cord injury (SCI). Upper limb impairments due to entrapment neuropathy can be particularly debilitating in this population. Anterior interosseous nerve (AIN) neuropathy has not been previously described in the SCI population. CASE PRESENTATION: A 27-year-old left-handed man with a history of C7 ASIA Impairment Scale B spinal cord injury five years prior presented to clinic with decreased left thumb function as well as thumb flexion. Workup including nerve cond

Transverse Ultrasound Assessment of the Displacement of the Median Nerve in the Carpal Tunnel during Wrist and Finger Motion in Healthy Volunteers.

PURPOSE: The purpose of this study was to investigate the displacement of the median nerve in the carpal tunnel during finger motion at varied wrist positions using transverse ultrasound in healthy volunteers, in order to clarify the appropriate position of a wrist splint in treating carpal tunnel syndrome. METHODS: Fifty wrists of 25 asymptomatic volunteers were evaluated by transverse ultrasound. The location of the median nerve in the carpal tunnel was examined at 5 wrist positions (neutral, 60° dorsiflexion, 60° palmar flexion, 40° ulnar flexion, 10° radial flexion) with all 5 fingers in

More Than Just Another Elbow Tendinopathy: A Case Series of Misdiagnosed Ulnar Nerve Compression and Review of Common Upper Extremity Misdiagnoses in Climbing.

Climbing is an increasingly popular sport. Yet, we have encountered numerous misdiagnosed climbing injuries, highlighting a gap in climbing-related literature and a need for heightened awareness within the sports medicine community. We present a case series that highlights the clinical manifestation of ulnar nerve entrapment and compression in three patients with initially misdiagnosed and mismanaged elbow tendinopathy. After a correct diagnosis of ulnar nerve pathology was made, each patient underwent a prescribed rehabilitation protocol that focused on nerve motility exercises (tensioning an

Compression neuropathy of the ulnar digital nerves in the thumbs of a massage therapist.

Compression neuropathies of digital nerves, caused by hypertrophied or anomalous muscles, are rare compared with such occurrences above the wrist. We reported a case of compression neuropathy of the ulnar digital nerves in bilateral thumbs of a massage therapist. Entrapment of the digital nerves by the hypertrophied first dorsal interosseous and adductor pollicis muscles over the first web space of the right hand was detected by magnetic resonance imaging. Surgical debulking of the muscles and neurolysis were performed on the dominant right hand. The left hand was successfully treated with bot

Neglected reverse Essex-Lopresti injury with ulnar nerve compression.

A 45 year old woman was diagnosed as having anteromedial radial head dislocation and distal radius fracture five months after her injury on right forearm. The radial head dislocation led to ulnar nerve compression. She had severe restriction of her elbow movements. She was treated with arthrolysis, decompression of the ulnar nerve and radial head resection. The reverse Essex Lopresti injury and radial head dislocation compressing the ulnar nerve has not been reported in English language literature to the best of our knowledge. A mechanism is proposed for the injury. In acute presentations, res

An ulnar intraneural ganglion arising from the pisotriquetral joint: case report.

Intraneural ganglions of the hand are rare and remain poorly understood. We report a case of an intraneural ganglion arising from the pisotriquetral joint that penetrated the ulnar nerve at Guyon's canal. Although rare, these ganglions should be considered in the differential diagnosis of any neoplasms causing compression neuropathy.

Correlation between the lengths of the upper limb and cubital tunnel: potential use in patients with proximal ulnar nerve entrapment.

INTRODUCTION: We hypothesized that a correlation may exist between the length of the upper limb and the length of the cubital tunnel, which transmits the ulnar nerve from the arm to the forearm. If true, this association might aid in predicting individuals at greater risk of developing ulnar nerve compression at this site. MATERIALS AND METHODS: A total of 46 cadaveric upper limbs were dissected. The lengths of the upper limb and cubital tunnel were measured and analysis made of any correlation between these two distances. RESULTS: The average length of the cubital tunnel was found to be 2.7

Electrodiagnosis in entrapment neuropathy by the arcade of Struthers.

Two cases of high ulnar nerve neuropathy are reported. Lesions were localized at the midarm level by electrophysiologic studies. In the first case, the lesion was found mainly to be a prolonged neurapraxia, and neurolysis was effective. The ulnar nerve was swollen 1 cm in length under the arcade of Struthers. After neurolysis, the palsy recovered rapidly. In the other case, the lesion seemed to be a mild injury to the myelin sheath. Delayed segmental conduction velocity and partial conduction block were found at the midarm level. The paresis improved slightly during the 11-month followup witho

Characteristics of surgically treated Guyon canal syndrome: A multicenter retrospective study.

Guyon canal (GC) syndrome is a rare peripheral neuropathy involving the distal part of the ulnar nerve. Several causes are associated with GC syndrome, including anatomic variations, space-occupying tumors, and trauma. Because of disease rarity, the only reported studies of GC syndrome are case series with small sample size. We conducted a multicenter study to identify the basic characteristics of patients with surgically treated GC syndrome and the risk factors for the disease. This retrospective multicenter study was conducted between January 2001 and December 2020. We screened 70 patients w

Cervical dermatomal zona misdiagnosed as ulnar nerve entrapment.

Herpes zoster is a secondary reactivation of primary contagious varicella-zoster virus in the dorsal root ganglia. While thoracic zona is common, cervical dermatomal zona is a rare segmental complication of herpes zoster and can be easily misdiagnosed as other diseases. This article describes a patient with initial neuralgia without dermatomal lesions that was treated as ulnar nerve entrapment syndrome until manifestation of herpetiform cutaneous lesions appeared. It is important that clinicians should be aware of the possibility of zoster infection when evaluating the onset of neuralgia in a

Anatomic considerations in evaluation of the proximal ulnar nerve.

Knowledge of general ulnar anatomy aids in the understanding of the possible site of ulnar nerve entrapment and the rationale for differential diagnosis possibilities. Understanding of internal nerve topography sheds light on reasons for ulnar nerve susceptibility at the elbow region and offers an explanation for the sparing of forearm muscles in ulnar neuropathy at the elbow. Dynamic anatomy or biomechanics of the elbow help elucidate the pathophysiology of ulnar nerve compression at the elbow. The anatomy interface with electrodiagnosis illustrates rationale for particular methods employed a
