Aims: The clinical significance of nonvisualized sentinel lymph nodes (non-vSLNs) is unknown. The authors sought to determine the incidence of non-vSLNs on lymphoscintigraphy, the identification rate during surgery, factors associated with non-vSLNs and related axillary management. Thyroid hormones affect the function of almost every body organ, and thyroid dysfunction produces a wide range of metabolic disturbances. Severe hypothyroidism is associated with significant effects on the kidney. The pathophysiology is thought to be multifactorial, while the exact mechanism remains unclear. Hypothyroidism as a cause of renal impairment is usually overlooked, leading to unnecessary diagnostic procedures. We describe two patients with acute renal failure due to severe hypothyroidism in whom thyroid hormone substitution therapy led to a significant improvement in renal function.

Inversion of the left atrial appendage is a rare complication after open-heart surgery. To our knowledge only 16 cases, besides that reported herein, have been described so far. Echocardiographically, the inverted left atrial appendage appears as a mass mimicking a thrombus, a vegetation or a tumor of the left atrium. Lack of awareness of this entity can result in a misdiagnosis and unnecessary procedures. The case here reported deals with an inverted left atrial appendage occurring in an infant after repair of an atrial septal defect.
