Persistent Postpartum Urinary Retention: A Case Report and Review of Literature.

This case report describes persistent urinary retention lasting over 30 days postpartum in a 23-year-old primiparous female after an otherwise uncomplicated vaginal delivery at 37 weeks gestation. Notable risk factors present included epidural anesthesia, episiotomy, third-degree perineal laceration, and inability to void spontaneously before leaving the delivery room. Despite initial catheterization draining a large volume, the patient experienced recurrent failed voiding trials requiring ongoing intermittent catheterization during her admission. One month after delivery, voiding trials were

Clean intermittent catheterization in spinal cord injury patients: a followup study.

A followup study on nonhospitalized spinal cord injury patients using clean intermittent catheterization was conducted to evaluate long-term clean intermittent catheterization for any genitourinary complications, and to institute and evaluate prompt management. A total of 50 patients (36 paraplegics and 14 quadriplegics) was followed for 3 months to 6.5 years (average followup 22 months). All patients had a baseline urodynamic study and renal scan before they were discharged from the hospital. Patients with a reflex bladder and sustained, high intravesical pressures (greater than 40 cm. water)

Secondary Amyloidosis of the Urinary Bladder Presenting As Urinary Retention.

Bladder amyloidosis is a rare condition of insoluble extracellular protein deposition in the urinary bladder arising as a complication of underlying inflammation and immune dyscrasia. Diagnosis of bladder amyloidosis is done through cystoscopy and evidence of amyloid deposition on pathologic evaluation. Here, we present a case of secondary bladder amyloidosis presenting with acute urinary retention. After ruling out bladder malignancy, the patient was managed with suprapubic catheterization after failing beta-3 agonist therapy and clean intermittent catheterization.

The rigid ureteroscope-guided Seldinger technique: a novel and effective method of Foley catheter deployment in patients with challenging transurethral access.

Transurethral Foley catheterization is the preferred method of bladder drainage in acute urinary retention. Various anatomical factors can make urethral access challenging, making perurethral catheterization impossible on occasions. In these instances, suprapubic catheterization or cystoscopic catheter deployment is often required. We describe an exceptionally difficult case in which transurethral catheterization remained unsuccessful despite cystoscopic visualization and establishing guidewire access. A Seldinger-like approach was used to safely deploy a Foley catheter over a semirigid ureter

Eosinophilic cystitis with recurrent urinary retention: case report.

Eosinophilic cystitis is a rare inflammatory disease of the bladder whose origin, pathogenesis, and treatment are unknown. Frequency, dysuria, and hematuria are frequent symptoms. Here, we report a rare occurrence of recurrent urinary retention and repetitive catheterization. A 67-year-old male presented with acute urinary retention and intermittent gross hematuria of 2 weeks duration. Urethral catheterization followed by a trial without catheter, was successful. Complete blood count showed presence of eosinophils (eosinophilia) and computed tomography of kidneys, ureter and bladder with contr

[Urinary infections with hospital germs in general surgery].

Nosocomial urinary tract infection is the most common type of sepsis in the post-surgical patient. This paper presents our experience with 218 nosocomial urinary infections (34.93% of our postoperative infections) which complicated the postoperative course of 1002 (21.75%) urethral catheterized patients out of a total of 5950 (3.6%) operated on and under study individuals. Thus we found that urethral catheterization is the most important risk factor for post-surgery urinary infections. Moreover, in our series the postoperative urinary tract sepsis bacteriology is dominated (> 95% of cases) by

Retrospective observational study of the incidence of short-term indwelling urinary catheters in elderly patients with neck of femur fractures.

BACKGROUND: 15-25% of general hospital admissions tend to involve patients that have had a short-term indwelling urinary catheter (IDC) inserted some time during their stay. There is little data on the specific incidence and complications of short-term urinary catheterization in elderly patients with neck of femur fractures. METHODS: Data was collected from the notes of 50 patients at Hemel Hempstead General Hospital with neck of femur fractures retrospectively from 31 August 2007. Specific information on patient demographics, premorbid status, record and reason for urethral catheterization, …

Yang-Monti's Catheterizable Stoma in Children.

BACKGROUND: In 1981, Mitrofanoff described a procedure to create a continent urinary stoma for clean intermittent catheterization. Since then several procedures have been described including Yang-Monti ileovesicostomy for effective catheterization. OBJECTIVES: We report on our experience from the use of Monti's procedure in children at our center. PATIENTS AND METHODS: Children < 18 years of age undergoing urinary diversion/reconstruction with Yang-Monti's procedure for congenital conditions or neuropathic bladder formed the study group. All these children, post-operatively were taught clean

Short-term versus long-term catheterization after vaginal prolapse surgery.

Utero-vaginal prolapse is one of the commonest cause for vaginal operations in our country. The expense of this operation can be reduced by reducing duration of catheterization and thereby reducing hospital stay. A prospective, comparative hospital based study was used to assess whether removal of an in-dwelling catheter after 24 hours of vaginal operations with anterior colporrhaphy affects the rate of recatheterization, asymptomatic urinary tract infections, and hospital stay in comparison to 72 hours catheterization. We assigned 100 women who underwent vaginal operations. In-dwelling cathet

Prolonged postpartum urinary retention.

Urinary retention after vaginal delivery is a relatively common problem, usually resolved by the temporary use of an indwelling catheter. Presented are two patients with persistent bladder atony after vaginal delivery, despite the use of an indwelling catheter for 10 days. Both patients had low forceps deliveries without severe perineal trauma. Both were treated by self-intermittent catheterization, with resolution of urinary retention at 24 and 37 days postpartum, respectively. These patients were able to learn this technique with minimal instruction. Advantages of intermittent catheterizatio

Bladder management methods and urological complications in spinal cord injury patients.

BACKGROUND: The optimal bladder management method should preserve renal function and minimize the risk of urinary tract complications. The present study is conducted to assess the overall incidence of urinary tract infections (UTI) and other urological complications in spinal cord injury patients (SCI), and to compare the incidence of these complications with different bladder management subgroups. MATERIALS AND METHODS: 545 patients (386 males and 159 females) of traumatic spinal cord injury with the mean age of 35.4±16.2 years (range, 18 - 73 years) were included in the study. The data rega

Managing recurrent urinary catheter blockage: problems, promises, and practicalities.

Long-term urinary catheterization is rarely completely free of complications, and encrustation by mineral salts leading to catheter blockage is common in around 40% to 50% of long-term catheterized patients. Recurrent blockage is a problem, which is both distressing to patients and caregivers and costly to health services in terms of time and resources. This article addresses the causes of recurrent urinary catheter blockage, proactive approaches to care, and the evidence for use of catheter maintenance solutions to reduce buildup of mineral deposits.

Biofilms and catheter-associated urinary tract infections.

Urinary catheter-related infections are commonly seen in several different patient populations and lead to substantial morbidity. The overall health care costs caused by these infections are sizable given how often urinary catheters are used in acute care settings, extended care facilities, and in persons with injured spinal cords. Recent attention has appropriately focused on biofilm development on the catheter surface because biofilm has important implications for the pathogenesis, treatment, and prevention of catheter-related infection. Because the most important risk factor for infection i

Diagnosis of Postoperative Urinary Retention Using a Simplified Ultrasound Bladder Measurement.

BACKGROUND: In this study, we sought to determine whether a simplified ultrasound measurement of the largest transverse diameter, using a standard ultrasound machine, could be used to diagnose postoperative urinary retention (POUR). This method may replace expensive bladder volume measuring devices or a more complex ultrasound procedure (involving the measurement of 3 bladder diameters). METHODS: Patients at risk of POUR if unable to void after orthopedic surgery were evaluated in the postanesthesia care unit before discharge. Bladder diameter was first measured using a portable ultrasound de

The diagnosis and management of lower urinary tract symptoms in multiple sclerosis patients.

Sixty-five percent of multiple sclerosis patients have moderate to severe urinary symptoms and up to 14% initially present with urinary symptomatology. Urinary retention, neurogenic detrusor overactivity, and detrusor sphincter dyssynergia, all increase the risk for urinary tract infections in patients with multiple sclerosis, and these infections may exacerbate their immune response, leading to symptom progression. Fewer than half of the patients with urinary symptoms have seen a specialist and only half have been treated for their neurogenic detrusor overactivity. Several treatments includin
