BACKGROUND: Laparoscopic adjustable gastric banding (LAGB) causes weight loss primarily through a mechanical restrictive mechanism. The vagus nerve provides connections between the brain and the gut through afferent and hormonal signals that regulate fullness and satiety. Published studies demonstrate clinically significant weight loss by subjects undergoing open surgical truncal vagotomy for ulcer disease and morbid obesity. We report the case of a 40-year-old man with a perforated duodenal ulcer who underwent antrectomy at laparotomy, and in whom standard truncal vagotomy was not technically possible due to an intraabdominal abscess. One hundred twenty-one patients with prepyloric ulcer disease entered a randomized clinical trial comparing gastroduodenostomy with Roux-Y gastrojejunostomy after antrectomy and selective gastric vagotomy. The influence of transgastric selective proximal vagotomy (TSPV) on the cardia functional state was studied in 120 patients with ulcer disease of the duodenum. It was established that seromyotomy in the zone of cardia and lesser curvature of the stomach when performing TSPV resulted in disturbance of the constrictor-valvar mechanism of the esophago-gastric passage. Fundoesophagography does not allow the insufficient closure of the cardia to be adequately corrected. The significance of fundoplication is stressed in order to restore the mechanism of cardia closure after TSPV.

Total duodenal diversion by gastric resection, Roux-en-Y reconstruction and vagotomy has been suggested as treatment of recurrent reflux oesophagitis, to avoid the increased risk of gastric and oesophageal fistulas associated with repeated antireflux surgery. A report is presented of this procedure in six patients with previous failed antireflux surgery. At follow-up (mean 17 months) the result was classified as good in three cases, moderately good in two and poor in one case.

A clinical series of 208 patients who had elective operations for gastroduodenal ulcer in Surgical Department II, Oslo City Hospital, has been reviewed. The study concentrates on postoperative gastric retention, comparing the frequency of this complication following antrectomy and gastroduodenal anastomosis with and without vagotomy.
