Re-Endothelialization of Decellularized Liver Scaffolds: A Step for Bioengineered Liver Transplantation.

Bioengineered livers (BELs) are an attractive therapeutic alternative to address the donor organ shortage for liver transplantation. The goal of BELs technology aims at replacement or regeneration of the native human liver. A variety of approaches have been proposed for tissue engineering of transplantable livers; the current review will highlight the decellularization-recellularization approach to BELs. For example, vascular patency and appropriate cell distribution and expansion are critical components in the production of successful BELs. Proper solutions to these components of BELs have ch…

A simple and novel technique for training in microvascular suturing in a rat model.

BACKGROUND: Though microvascular clamps are widely used for anastomosis training, there still have several shortcomings, including the bulging, expensiveness and unavailability due to sterilization. The aim of this study is to introduce a simple and novel microvascular training model without use of microvascular clamps. METHODS: Femoral vessels of Sprague Dawley rats training model were used to evaluate the usefulness of 4-0 silk as a slipknot for performing arterio-arterial and veno-venous microvascular anastomoses. A total of 12 Sprague Dawley rats were randomly assigned to either slipknot

Pathophysiology and Mechanisms of Saphenous Vein Graft Failure.

INTRODUCTION: Coronary artery bypass grafting remains one of the best therapies for advanced coronary artery disease. The most used conduit remains the great saphenous vein, which is susceptible to short-term and long-term failure, the result of acute thrombosis, intimal hyperplasia, and late superimposed atheroma. In this review, we present the current findings related to the pathophysiology of vein graft failure. METHODS: A search of three databases - MEDLINE®, Web of Science™, and Cochrane Library - was undertaken for the terms "pathophysiology", "prevention", and "treatment" plus the term

[Reconstruction of aneurismal arteriovenous fistula after arrosive bleeding].

Aneurysmal transformation of arteriovenous fistula (AVF) is a common complication and associated with increased risk of arrosive bleeding. Ligation of fistulous vein is conventional surgery for bleeding. This is followed by AVF malfunction and need for implantation of central venous catheter. We report reconstructive surgery and maintaining the function of AVF in a patient with aneurysmal transformation of AVF after arrosive bleeding. Reconstructive vascular surgery can significantly extend the period of patent AVF for hemodialysis. This is extremely important in patients with reduced resource

Orbital atherectomy for calcified femoropopliteal lesions: a current review.

The orbital atherectomy system is a novel form of atherectomy that uses orbital sanding and pulsatile forces, an effective method of treatment for peripheral atherosclerotic lesions with varying levels of occlusion. Although the devices only has a general indication from the FDA to treat atherosclerotic lesions, they are effective in treating all kinds of lesions, and can therefore mitigate effects of all severities of peripheral artery disease. This approach to endovascular therapy involves the use of differential sanding to preferentially ablate fibrous, fibrofatty and calcified lesions, whi

Pulmonary artery sling. Results of surgical repair in infancy.

Pulmonary artery sling is a rare congenital vascular anomaly in which the left pulmonary artery originates from the right pulmonary artery and encircles the right main-stem bronchus and distal trachea before entering the hilum of the left lung. This causes compression of the trachea and right main-stem bronchus, and most infants with this anomaly have severe respiratory distress within the first year of life. Between 1953 and 1990 12 infants (nine male, three female) underwent surgical repair of pulmonary artery sling. Ages ranged from 8 days to 9 months (mean age 5 months). Bronchoscopic exam

Patency of the arterial pedal-plantar arch in patients with chronic kidney disease or diabetes mellitus.

BACKGROUND: Patency of the pedal-plantar arch limits risk of amputation in peripheral artery disease (PAD). We examined patients without chronic kidney disease (CKD)/diabetes mellits (DM) [PAD-control], those with DM without CKD, and those with CKD without DM. METHOD: Uni- and multivariate logistic regression was used to assess association of CKD with loss of patency of the pedal-plantar arch and presence of tibial or peroneal vessel occlusion. Multivariate models adjusted for age, sex, hypertension, hyperlipidemia and smoking. RESULTS: A total of 419 patients were included [age 75.2 ± 10.3

Long-term results of arteriovenous fistulas using transposed autologous basilic vein.

BACKGROUND: The transposed autologous basilic vein to brachial artery arteriovenous fistula provides vascular access in the absence of adequate superficial vein. The long-term patency and complication rates of this conduit are largely unreported. PATIENTS AND METHODS: Seventy-four brachiobasilic fistulas were performed in 65 patients. This was a secondary or tertiary access procedure in 84 per cent of patients. RESULTS: Of the 74 fistulas performed, 50 were successfully used for dialysis (68 per cent) and 24 (32 per cent) were never used, including 13 fistulas that failed without being used

Endothelium-dependent vasoactivity of the human internal mammary artery.

Coronary artery disease is recognized as a major health problem worldwide, particularly because of the associated morbidity and mortality. Coronary artery bypass grafting has been an established mainstay in the treatment of this disease for almost half a century and is arguably the most intensively studied surgical procedure ever undertaken. Because of its unique properties, the human internal mammary artery has long been considered the best graft to use in this type of surgery. Previous studies have shown several advantages of this graft compared with others, that is, lower incidence of ather

Particle hemodynamics analysis of Miller cuff arterial anastomosis.

OBJECTIVE: Studies of animal and human below-knee anastomoses with Miller cuffs indicate that improved graft patency results from redistribution of intimal hyperplasia away from areas critical to flow delivery, such as the arterial toe. We hypothesize that particle hemodynamic conditions are a biophysical mechanism potentially responsible for the clinically observed shift in intimal hyperplasia localization associated with better patency of the Miller configuration. METHODS: Computational fluid dynamics analysis of vortical flow patterns, wall shear stress fields, and potential for platelet i

Intimal staining for visibility enhancement during microanastomoses.

Absolute perfection in microvascular surgery continues to be thwarted by technical inadequacies in construction of the microanastomosis. Occasionally this is a consequence of limited visualization or misidentification of the intima proper predisposing to thrombosis. At least in rats, we have demonstrated that intimal staining with clinically inert vital dyes has enhanced gross appreciation of the exact intimal location. This maneuver has simplified suture placement without evidence of any associated increased risk of anastomotic failure.

Vein cuff interposition prevents juxta-anastomotic neointimal hyperplasia.

This study sought to minimize juxta-anastomotic neointimal hyperplasia (JNIH) following the use of polytetrafluorethylene (PTFE) conduits. PTFE anastomoses to canine carotid arteries (noncuff grafts) were compared with grafts with vein cuffs interposed proximally and distally between the graft and native artery. This technique has been suggested clinically for below-knee PTFE femoropopliteal reconstruction. Twelve dogs received aspirin for 1 week before operation, which was continued after each animal received bilateral cuff and noncuff 4-mm PTFE grafts. At sacrifice, after 3-12 weeks, graft p

Covered Stent Placement for a Splenic Artery Pseudoaneurysm in a Patient With Inflammatory Vascular Disease.

Splenic artery pseudoaneurysms (SAPs) are potentially life-threatening vascular lesions with a high risk of rupture that may be associated with inflammatory vascular pathologies. While transcatheter embolization is the most common endovascular treatment for SAPs, covered stent placement preserves arterial patency and splenic perfusion but is limited by anatomic constraints, such as inadequate landing zones and excessive vessel tortuosity. This case report discusses the clinical and technical considerations in successfully stenting a SAP in a patient with a history of systemic lupus erythematos

Patent ductus arteriosus in a lamb: A case report.

Patent ductus arteriosus (PDA) is a persistent patency of a vessel normally present in the fetus that connects the pulmonary arterial system to the aorta. The ductus arteriosus fails to close at birth when breathing commences and placental blood circulation is removed. Closure of the ductus arteriosus arises in response to decline pulmonary vascular resistance and increased systemic vascular resistance. This report describes a case of PDA in a two-month-old male lamb with clinical signs of machinery murmur, tachycardia, increase respiratory rate, weakness and ill thrift. Echocardiographic exam

The forearm basilic vein looped transposition fistula as a tertiary option for upper limb vascular access.

INTRODUCTION:: The forearm basilic vein can serve as an option for haemodialysis access but may not be possible in cases where the wrist arteries are unsuitable. In this setting, the forearm basilic vein can be used in a looped transposition with a brachial artery anastomosis. AIMS:: The aims of this study were to assess the outcome of forearm basilic vein looped transposition as an option for vascular access. MATERIAL AND METHODS:: Data from January 2007 to December 2010 were prospectively collected and analysed. Outcome measures were operative success, complications, maturation and primary

Aortic versus non-aortic cryopreserved allograft and donor-specific outcome: a retrospective observational study.

INTRODUCTION: Cryopreserved arterial allografts are an established conduit for vascular reconstruction in patients with vascular graft infection (VGI). Although outcomes after aortic and peripheral implantation have been reported separately, comparative data including donor- and graft-specific characteristics are limited. This study compared clinical, procedural, and donor-related factors between aortic and non-aortic allograft implantation and evaluated their association with postoperative outcomes. MATERIALS AND METHODS: This retrospective single-center study included all consecutive patien

Vascular restenosis. Basic science and clinical implications.

Cardiovascular interventions to correct arterial occlusive disease have excellent short-term results, but long-term patency is still seriously compromised by the development of restenosis. This review will provide a portrait of the mechanisms following vascular injury, define the clinical scope of the problem and outline developments of different therapeutical strategies to prevent restenosis.
