Imaging case study of the month: traumatic pneumolabyrinth.

Pneumolabyrinth is a condition in which air is present in the vestibule and/or in the cochlea. Air inside the inner ear structures is uncommon, and is not detected even in otic capsule-violating fractures or in transverse fractures of the temporal bone. It is rarely described in the literature. We present a longitudinal computed tomography (CT) study of a significant pneumolabyrinth due to temporal bone trauma in a 31-year-old man. Routine CT of the cranium performed 3 hours after the accident showed air inside the vestibule (pneumolabyrinth). Two days later, a new CT study was performed. The

Ultrastructure of the vestibular sensory organs in delayed endolymphatic hydrops.

Vestibular sensory organs were examined ultrastructurally in two cases of delayed endolymphatic hydrops. The patients, two women, 27 and 16 years of age, suffered profound sensory hearing loss in childhood and experienced severe, recurrent vertigo. Specimens were obtained by a translabyrinthial vestibular nerve transection. We studied the utricular macula and the lateral and posterior cristae in one case, and the utricular macula, the anterior and posterior cristae, and the vestibular ganglion in the other. The otoconia and the otoconial membrane, the sensory epithelia, and the vestibular gang

Vascular dizziness and transcranial Doppler ultrasonography.

Dizziness as defined herein will include an illusion of motion caused by various degrees of ischemia to the vestibular pathway or its interconnecting pathways. "Syndrome," such as the lateral medullary syndrome, denotes a macroinfarct, while a microinfarct or an area of incomplete infarct (where there may develop an incomplete degeneration of the neural tissue secondary to the arteriolar microatheromatous stenosis) may cause only one neurologic deficit, such as dizziness per se as the only symptom. However, the latter may presage a larger and more debilitating neurologic deficit. The transcran

Medical fitness examination of commercial pilots: new criteria for evaluation of vestibular tests.

In medical fitness examinations of commercial pilots, extensive vestibular investigations should be conducted. In particular, the pendular test, as a weak rotational stimulation method, can disclose central vestibular disorders. Thus, the central nystagmus tracing can be considered as a sign of irritation of vestibular centers, most frequently as a consequence of head trauma, a finding that may influence the decision in the enlistment of the pilot. In contrast, a small nystagmus amplitude tracing is observed in cases of insufficient blood supply, occurring mostly in elderly persons. Therefore,

[Evaluation of cochlear and vestibular functions of patients treated by dibekacin in an E.N.T. unit].

Cochlear and vestibular ototoxicity of dibekacin was evaluated in 27 patients treated with aminoglycoside after surgery on head and neck cancer. The treatment was administered intramuscularly or intravenously during 13 days at the schedule of 3 mg/kg per day. Cochlear and vestibular functions were tested by audiometry and electronystagmography before the administration of the antibiotic and on the 14th, 30th day, and for certain patients on the 4th month after the start of the treatment. No cochlear ototoxicity was found after the dibekacin treatment. On the other hand, the dibekacin treatment

Heredo-ataxia and cochleo-vestibular disorders.

Cochleo-vestibular dysfunction of patients suffering from a heredo-ataxia, have been frequently described. A family and a single case are presented, which show the association of a heredo-ataxia, more specifically the Roussy-Lévy syndrome, progressive sensorineural loss, nystagmus and vestibular a- or hyporeflexia. A review of the literature is included and the standpoints concerning the nosological entities are discussed.

Inner ear degeneration in Reye's syndrome.

A 9-year-old girl developed Reye's syndrome. On admission to the hospital on the fourth day of illness, responses for verbal stimuli had been absent, but were present for painful stimuli. Coma developed on the fourth day and was present until death ten days later. The fundus of both internal meatus showed vascular congestion of nerves with evidence of repeated hemorrhages. All structures within the endolymphatic system showed advanced degeneration, with exception of the tectorial membrane and cupulae. One ear showed vascular congestion, formation of thrombi, and free hemorrhage within the modi

Effect of fistulae on endolymphatic hydrops.

An experiment was performed on 31 guinea pigs to study the effect of fistulae of the horizontal canal, superior canal, common crus, and utricle on the course of experimentally induced endolymphatic hydrops. The effect of fistulae on corresponding parts of normal ears was also studied in an additional 17 animals. The results indicated a remarkable consistency of healing of the fistulae in both groups of animals. Fistulae had no significant effect on the course of endolymphatic hydrops, irrespective of whether the fistulae were made immediately before or several months after obliteration of the

Labyrinthine fistulas: Surgical outcomes and an additional diagnostic strategy.

PURPOSE: To evaluate perioperative findings and audiological and vestibular outcomes in patients operated for cholesteatoma with labyrinthine fistulas. Also to assess radiological fistula size. MATERIALS AND METHODS: Patients who underwent surgery for a labyrinthine fistula caused by a cholesteatoma between 2015 and 2020 in a tertiary referral center were retrospectively investigated. Fistula size was determined on preoperative CT scan. Bone and air conduction pure tone average thresholds were obtained pre- and postoperatively. Clinical outcomes, such as vertigo and otorrea were also evaluate
