Somnophilia and Sexual Abuse Using Vaginal Administration of Triazolam.

Somnophilia is a rare paraphilia, a form of sexual fetishism which is characterized by the desire to have sex with an unconscious human object who is unable to respond. To the author's knowledge, this is the first case study concerning somnophilic sexual abuse associated with vaginal administration of triazolam. The perpetrator video-recorded his sexual acts with two unconscious female victims with whom he also had normal sexual intercourse and who were unaware of his paraphilic activities. His Internet conversations with other persons whom he thought to be interested in somnophilic sex and hi

Criteria-based diagnosis and antibiotic overuse for upper respiratory infections.

OBJECTIVE: Studies of antibiotic overuse often rely on physicians' reports of diagnoses, which can overestimate bacterial illness. To assess potential overdiagnosis, we determine bacterial upper respiratory infection diagnoses by direct observation of visit videotapes. DESIGN: From an observational study of videotaped visits for upper respiratory symptoms (N = 66), coders assessed diagnostic criteria (symptoms, physician description of physical examination findings, and diagnostic tests), physician diagnosis, and prescribing. Survey data included patient demographics and health care utilizati

Longitudinal monitoring of fetal behavior in twins when one is anencephalic.

BACKGROUND: To our knowledge, there has been no report on longitudinal monitoring of fetal behavior in the anencephalic fetus. CASE: A 30-year-old woman was diagnosed with a twin pregnancy at 8 weeks' gestation; at week 20, one of the twins was identified as anencephalic. From 25-36 weeks' gestation, the behavioral patterns of each fetus were recorded weekly on videotape for 60 minutes, then compared with morphologic findings after birth. CONCLUSION: Our data comparing normal and anencephalic fetuses indicate that the development of the central nervous system above the medulla oblongata play

A retrospective video analysis of canonical babbling and volubility in infants later diagnosed with childhood apraxia of speech.

Canonical babbling and volubility are common indicators of infant vocal development. If either indicator falls below values found in typical infants, it may be an early signal of a developmental disorder. The aim of this retrospective investigation was to examine home videos for the frequency of canonical babbles, age of canonical babbling onset, and overall volubility of eight infants later diagnosed with childhood apraxia of speech and compare these data to that of six typically developing infants as well as four infants later diagnosed with a speech sound disorder. Results revealed the infa

Cluster analysis to identify foot motion patterns in children with flexible flatfeet using gait analysis-A statistical approach to detect decompensated pathology?

INTRODUCTION: The paediatric flexible flatfoot constitutes the major cause of clinic visits for orthopaedic foot problems. It shows variations of deformities in different planes and locations of the foot and its indication for treatment have been extensively discussed. Despite its high prevalence there exists no classification of flatfeet during walking as a prerequisite for treatment decision. Therefore, the aim of this study is to classify flexible flatfeet based on 3D foot kinematics during walking. METHODS: Patients age 7-17 years with flexible flatfeet (N = 129, 255 feet) of non-neurogen

Student health professionals' attitudes and experience after watching "Ida's Diary", a first-person account of living with borderline personality disorder: Mixed methods study.

BACKGROUND: There is increasing interest in the use of commercial movies in nursing education, or "cinenurducation". There is a need for educational interventions which target mental health nurses' attitudes towards people with borderline personality disorder. OBJECTIVES: To investigate and evaluate the experience and effects of attendance at a screening of the movie Ida's Diary, a first-person account of living with borderline personality disorder. DESIGN: Mixed methods design comprising a within-subjects AB longitudinal survey, and a qualitative analysis of participant-generated data and r

Sleep in frontotemporal dementia is equally or possibly more disrupted, and at an earlier stage, when compared to sleep in Alzheimer's disease.

BACKGROUND: Conversely to other neurodegenerative diseases (i.e., Alzheimer's disease, AD), sleep in frontotemporal dementia (FTD) has not been studied adequately. Although some evidence exists that sleep-wake disturbances occur in FTD, very little is known regarding sleep macrostructure and/or primary sleep disorders. OBJECTIVE: To investigate these issues in this population and compare them to similar issues in AD and in healthy elderly (HE). METHODS: Twelve drug-naïve behavioral-variant FTD (bvFTD) patients (7 men/5 women) of mean age 62.5 ± 8.6 years were compared to seventeen drug-naïve

The spatiotemporal dynamics of rheotactic behavior depends on flow speed and available sensory information.

Rheotaxis is a robust, multisensory behavior with many potential benefits for fish and other aquatic organisms. Visual (optic flow) cues appear to be sufficient for rheotaxis, but other sensory cues can clearly compensate for the loss of vision. Nevertheless, the nature of multisensory interactions and the relative contributions of different senses under varying conditions are poorly understood - largely because there is so little description of the actual behavior. Here, we examined the effects of different flow speeds and different sensory conditions on the spatiotemporal dynamics of rheotax

Paroxysmal non-epileptic motor events in childhood: a clinical and video-EEG-polymyographic study.

AIM: The aim of this article was to describe the phenomenology and polymyographic features of paroxysmal non-epileptic motor events (PNMEs) observed in a series of typically developing and children with neurological impairment. METHOD: We conducted a retrospective evaluation of 63 individuals (29 females; 34 males) affected by PNMEs at the National Neurological Institute 'C. Besta' between 2006 and 2008. Individuals were included in the study if they had PNMEs documented by a video-electroencephalography-polymyographic study and were aged between 1 month and 18 years (mean age at the time of

Echoes from childhood--imitation in Gilles de la Tourette Syndrome.

BACKGROUND: Tourette syndrome patients are reported to show automatic imitation (echopraxia), but this has not yet been proven experimentally. METHODS: Video clips showing either tics of other Tourette patients or spontaneous movements of healthy subjects were presented to Tourette patients and healthy subjects. Participants' responses were assessed using blinded review of video recordings by 2 independent raters and related to stimuli presented. RESULTS: Both raters detected more echoes in patients. In a permutation analysis, no healthy subject had echoes above chance level. In contrast, 6

Nonpharmacological treatment of tics in Tourette syndrome adding videotape training to self-hypnosis.

OBJECTIVE: This case series examines the practicality of using a standardized method of training children in self-hypnosis (SH) methods to explore its efficiency and short-term efficacy in treating tics in patients with Tourette syndrome. METHODS: The files of 37 children and adolescents with Tourette syndrome referred for SH training were reviewed, yielding 33 patients for analysis. As part of a protocol for SH training, all viewed a videotape series of a boy undergoing SH training for tic control. Improvement in tic control was abstracted from subjective patient report. RESULTS: Seventy-ni

Detection of walking periods and number of steps in older adults and patients with Parkinson's disease: accuracy of a pedometer and an accelerometry-based method.

The aim of this study was to examine if walking periods and number of steps can accurately be detected by a single small body-fixed device in older adults and patients with Parkinson's disease (PD). Results of an accelerometry-based method (DynaPort MicroMod) and a pedometer (Yamax Digi-Walker SW-200) worn on each hip were evaluated against video observation. Twenty older adults and 32 PD patients walked straight-line trajectories at different speeds, of different lengths and while doing secondary tasks in an indoor hallway. Accuracy of the instruments was expressed as absolute percentage erro

The impact of aggressive priming, rumination, and frustration on prison sentencing.

We tested the hypothesis that ruminating about a previous aggressive prime interacts with a subsequent minor frustration to augment aggression. Sixty participants watched a video showing a murder during a bank robbery (the aggressive prime). Those in the rumination condition were asked to write about the video for 20 min. In the no rumination condition, participants were given 20 min to complete an irrelevant task. Participants were then either frustrated or not frustrated. Our results supported the main hypothesis. Relative to the control condition, neither rumination nor frustration alone im

Vocal fold polyp in a professional brass/wind instrumentalist and singer.

Wind instrumentalists, especially brass players, and singers share common factors, including vocal tract shape, function and pressure, vocal fold opening and closure, breath vector of force and air flow rates. To understand the mechanism and function of the vocal folds with a pathological lesion, it is necessary to visualize the differing interactions of the vocal tract during wind and brass instrument playing and in singing. A school band director, singer, wind and brass instrumentalist, was referred by musician colleagues with intermittent dysphonia, aphonia, and inability to sing high notes

Characterizing magnetoencephalographic spike sources in children with tuberous sclerosis complex.

PURPOSE: Tuberous sclerosis complex (TSC) often causes medically intractable seizures. Magnetoencephalography (MEG) localizes epileptiform discharges. To evaluate the use of MEG spike sources (MEGSSs) for localizing epileptic zones in TSC patients, we characterized MEGSSs and correlated them to EEG and magnetic resonance imaging (MRI) results. METHODS: We analyzed data from seven children who underwent prolonged video-EEG, MEG, and MRI. We classified MEGSSs as clusters (six or more spike sources, <or=1 cm between sources) and scatters (fewer than six spike sources regardless of distance betwe

Dystonia as a presenting sign of spinocerebellar ataxia type 1.

We report on a 39-year-old man who presented initially with marked blepharospasm, oromandibular dystonia and retrocollis and one year later developed mild ataxia. Our findings suggest that dystonia can be a disabling presenting sign of SCA1 and support the clinical heterogeneity of SCA1, highlighting the importance of considering this entity in patients combining dystonia and cerebellar ataxia.
