Obstruction of the inferior vena cava with resultant Budd-Chiari syndrome after giant omphalocele repair has not previously been reported. Such a case is presented. We hypothesize the mechanism of this blockage to be posterior displacement of the inferior vena cava during reduction of the viscera and closure of the anterior abdominal wall, thereby creating angulation and mechanical blockage of the inferior vena cava at the diaphragmatic hiatus.

Blunt or penetrating trauma to the chest can cause several life-threatening conditions: open or closed pneumothorax, each with or without hemothorax; flail chest; pericardial tamponade, and injury to other structures in the chest--the esophagus, trachea or great vessels. Any trauma sufficient to compromise function of thoracic organs must also be suspect for extrathoracic injuries, especially to the spleen and other abdominal viscera.

A 30-year-old man of African origin presented to our emergency department (ED) with subjective fever and abdominal pain which started on the day of attendance. METHOD: Gavage with high, medium, small dose of curculigoside once daily for 30 d consecutive days. Record the related behavior index. Inguinal hernias are a common presentation to surgical admission units throughout the world. The majority of presentations are due to hernias containing either fat or small bowel. However, a wide range of intra-abdominal viscera have been demonstrated in inguinal hernias. We report a case of an 87-year-old man who presented with gastric outlet obstruction secondary to an incarcerated inguinal hernia containing the gastric pylorus.

A middle-aged woman with common variable immunodeficiency noted a papular skin eruption that simulated Gottron's sign of dermatomyositis on the dorsal hands. Examination of a skin biopsy specimen demonstrated noninfectious granulomatous inflammation. The patient was subsequently found to have visceral granulomas when examined using laparotomy. Noninfectious granulomas of the viscera and integument have been previously reported in patients with several immunodeficiency syndromes, including common variable immunodeficiency.
