Efficacy of a multimodal analgesia protocol in total knee arthroplasty: a randomized, controlled trial.

A total of 100 osteoarthritis patients undergoing unilateral total knee arthroplasty were randomly assigned to receive either a multimodal analgesia protocol, comprising oral celecoxib and tramadol before and after surgery and intra-articular injection of large doses of morphine, ropivacaine, adrenaline and betamethasone during surgery (trial group), or oral and intra-articular placebo (control group). All patients received patient-controlled analgesia for 48 h after surgery. Morphine consumption up to 48 h after surgery was significantly lower in the trial than in the control group. Compared

Spontaneous Retroperitoneal Hemorrhage Treated With Transcatheter Arterial Embolization in a Patient With Acquired Cystic Kidney Disease: A Case Report.

Spontaneous retroperitoneal hemorrhage is a rare condition with a poor prognosis. It is usually treated conservatively unless vital signs are unstable. We report a case of a 60-year-old male patient with acquired cystic kidney disease who presented to ED due to continuous left lower abdominal pain. His vital signs were normal, and a physical examination revealed tenderness in the lower left abdomen. CT revealed a hematoma with extravasation around the left kidney. He was treated conservatively due to stable vital signs and laboratory data. However, a decrease in the hemoglobin level and an inc…

Development and Comparative Performance of Physiologic Monitoring Strategies in the Emergency Department.

IMPORTANCE: Accurate and timely documentation of vital signs affects all aspects of triage, diagnosis, and management. The adequacy of current patient monitoring practices and the potential to improve on them are poorly understood. OBJECTIVE: To develop measures of fit between documented and actual patient vital signs throughout the visit, as determined from continuous physiologic monitoring, and to compare the performance of actual practice with alternative patient monitoring strategies. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study evaluated 25 751 adult visits to continuou…

Identifying acute illness phenotypes via deep temporal interpolation and clustering network on physiologic signatures.

Using clustering analysis for early vital signs, unique patient phenotypes with distinct pathophysiological signatures and clinical outcomes may be revealed and support early clinical decision-making. Phenotyping using early vital signs has proven challenging, as vital signs are typically sampled sporadically. We proposed a novel, deep temporal interpolation and clustering network to simultaneously extract latent representations from irregularly sampled vital signs and derive phenotypes. Four distinct clusters were identified. Phenotype A (18%) had the greatest prevalence of comorbid disease w…

Cardiovascular effects of polymethylmethacrylate use in percutaneous vertebroplasty.

BACKGROUND AND PURPOSE: Previous investigators have described an association between polymethylmethacrylate (PMMA) use in hip arthroplasty and cardiovascular derangement. Our purpose was to evaluate the effects of PMMA injection on patient vital signs during percutaneous vertebroplasty. METHODS: We retrospectively reviewed patient charts at our institution to gather blood pressure, heart rate, and arterial oxygen saturation data for the following time points: before, during, 5 minutes after, and 10 minutes after PMMA injection during percutaneous vertebroplasty. These data were obtained for 1…

[Continuous intracarotid infusion of mannitol in severe head injury].

The clinical significance of continuous intracarotid infusion of a small dose of mannitol (ICI of mannitol) was discussed. Eighteen patients suffering from severe head injury with Glasgow coma scale (GCS) less than 6 were treated by ICI of mannitol for the improvement of raised intracranial pressure (ICP). In all of these 18 cases, conventional venous administration of mannitol could not be carried out, because of the unstable vital signs due to hypovolemic shock such as multiple trauma or disturbance of serum sodium and potassium levels. This method requires that a 20% mannitol solution be di…

Resistant alcohol withdrawal: does an unexpectedly large sedative requirement identify these patients early?

INTRODUCTION: While most patients with alcohol withdrawal (AW) respond to standard treatment that includes doses of benzodiazepines, nutrition and good supportive care (non resistant alcohol withdrawal-NRAW), a subgroup may resist therapy (resistant alcohol withdrawal-RAW). This study describes a distinct group of AW patients, their sedative requirements, and hospital courses. METHODS: Over a period of 6 months, AW patients requiring 50 mg diazepam IV in the first hour were followed. We recorded admission indices and diazepam doses with vital signs at 1, 2, 3, 6, 12, and 24 hours. Patients we…

Clinical presentation of tension pneumothorax among patients undergoing prehospital thoracostomy: A retrospective cohort study.

BACKGROUND: Tension pneumothorax (TPTX) is a life-threatening condition requiring rapid diagnosis and management. However, accurate diagnosis is challenging in the prehospital environment. We sought to investigate the clinical presentation of TPTX in the prehospital environment. Secondary objectives were to investigate if any vital signs are independently associated with TPTX, and if any differences in vital signs of TPTX exist between patients based on ventilatory status. METHODS: We conducted a retrospective cohort study of adult patients > 18 years of age, not in cardiac arrest, who receiv…

Continuous-Wave Radar and Motion-Derived Biomarkers for Non-Contact Vital Status Classification in End-of-Life Care: A Clinically Validated Machine Learning Approach.

In palliative care, effective communication about anticipated death is critical for aligning therapeutic goals, managing family expectations, and ensuring dignified care. However, prognostic uncertainty - particularly regarding the time of death - remains a challenge due to the limited reliability of current methods. This study explores the potential of radar-derived motion biomarkers as a novel approach to distinguish between living and deceased patients, addressing the need for objective decision-support tools in palliative care. Using continuous-wave radar, we recorded the torso displacemen

Feasibility of Using Resting Heart Rate and Step Counts From Patient-Held Sensors During Clinical Assessment of Medical Emergencies (FUSE): Protocol for Prospective Observational Study in European Hospitals.

BACKGROUND: Abnormalities of vital signs are quantified by comparison with normal ranges, which are those observed in resting healthy populations. It might be more appropriate to compare the vital sign values of an individual in distress with their own usual values recorded when they were stable and well. Sensors from smartwatches or smartphones might make this possible at scale, but the proportion of patients using them is not known. OBJECTIVE: This study aimed to assess the feasibility of using heart rate and mobility data from patients' own wearable sensors as part of clinical assessments …

A laser-engraved wearable sensor for sensitive detection of uric acid and tyrosine in sweat.

Wearable sweat sensors have the potential to provide continuous measurements of useful biomarkers. However, current sensors cannot accurately detect low analyte concentrations, lack multimodal sensing or are difficult to fabricate at large scale. We report an entirely laser-engraved sensor for simultaneous sweat sampling, chemical sensing and vital-sign monitoring. We demonstrate continuous detection of temperature, respiration rate and low concentrations of uric acid and tyrosine, analytes associated with diseases such as gout and metabolic disorders. We test the performance of the device in

A systematic review of near real-time and point-of-care clinical decision support in anesthesia information management systems.

Anesthesia information management systems (AIMS) are sophisticated hardware and software technology solutions that can provide electronic feedback to anesthesia providers. This feedback can be tailored to provide clinical decision support (CDS) to aid clinicians with patient care processes, documentation compliance, and resource utilization. We conducted a systematic review of peer-reviewed articles on near real-time and point-of-care CDS within AIMS using the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols. Studies were identified by searches of the electronic data

A mixed-methods study to investigate feasibility and acceptability of an early warning score for preterm infants in neonatal units in Kenya: results of the NEWS-K study : Neonatal early warning scores in Kenya.

Preterm birth (< 37 weeks gestation) complications are the leading cause of neonatal mortality. Early-warning scores (EWS) are charts where vital signs (e.g., temperature, heart rate, respiratory rate) are recorded, triggering action. To evaluate whether a neonatal EWS improves clinical outcomes in low-middle income countries, a randomised trial is needed. Determining whether the use of a neonatal EWS is feasible and acceptable in newborn units, is a prerequisite to conducting a trial. We implemented a neonatal EWS in three newborn units in Kenya. Staff were asked to record infants' vital sign…
