How Systemic Barriers Can Impact Health Inequities When Facing Climate Change Stressors: A Scoping Review of Global Differences.

The objective of this scoping review is to explore the systemic barriers that impact health inequities among vulnerable populations (e.g., racial/ethnic and gender groups, people with disabilities, refugees, immigrants, elders, young children, agricultural and fishery workers, and low-income individuals) when facing climate change stressors. We conducted an extensive review using nine search engines, which yielded 21 publications that focused on the health outcomes and barriers on the topic of climate change among vulnerable populations. Our findings indicated that poverty is the largest chall…

Heat vulnerability: health impacts of heat on older people in urban and rural areas in Europe.

Exposure to extreme heat is associated with both increased morbidity and mortality, especially in older people. Health burdens associated with heat include heat stroke, diabetes mellitus, hypertension, ischemic heart diseases, heart failure and arrhythmia, pulmonary diseases but also injuries, problems with activities of daily living, and mental disorders. In Europe, there are remarkable spatial differences in heat exposure between urban and less populated areas. In Austria, for example, there is a significant gradual association between population density and the number of heat days, where th

Critique and lessons learned from using multiple methods to estimate population size of men who have sex with men in Ghana.

Population size estimation of key populations at risk of HIV is essential to every national response. We implemented population size estimation of men who have sex with men (MSM) in Ghana using a three-stage approach within the 2011 Ghana Men's Study: during the study's formative assessment, the larger integrated bio-behavioral surveillance (IBBS) survey; and during the stakeholder meeting. We used six methods in combination within the three-stage approach (literature review, mapping with census, unique object multiplier, service multiplier, wisdom of the crowd, and modified Delphi) to generat

Climate change as a threat multiplier: conflict pathways, inequities, and mental health impacts.

Climate change is increasingly recognized as a major driver of global instability, understood here as the interconnected disruption of social, economic, political, and security systems, acting as a threat multiplier that intensifies existing vulnerabilities. Its mental health impacts emerge through direct exposures-such as extreme heat, wildfires, floods, and other disasters-and through indirect pathways, including livelihood disruption, resource insecurity, displacement, and social fragmentation. These pressures contribute to heightened risks of post-traumatic stress disorder, depression, anx

Failing those who most need support to quit smoking: Australia must do better.

Tobacco remains the leading cause of preventable death in Australia, and although overall smoking has declined, rates remain much higher among disadvantaged populations. Thus, those most likely to be nicotine dependent are least able to afford cessation treatment. With strict limitations on duration of treatment and the range of products currently subsidised through the Australian Pharmaceutical Benefits Scheme, Australian Federal Government spending on tobacco addiction treatment falls well short of levels recommended by the World Health Organization, and best-practice interventions are not a

Medical debt in America, Part II: Financial support for the most vulnerable.

In this second part of a series on medical debt and the costs of care, we examine the past and present financial protections for low-income patients at nonprofit hospitals. Born of almshouses and religious orders, nonprofit hospitals were devoted at their founding to the care of the poor. However, over the course of the twentieth century, they became more focused on high-priced care for paying patients. Federal regulations surrounding tax exemption and charity care have been loosened to allow hospitals to spend relatively little on financial assistance, contributing to inequitable and inadequa

Adapting to heat-health vulnerability in temperate climates: current adaptation and mitigation responses and future predictions in Aotearoa New Zealand.

INTRODUCTION: Climate change is raising global temperatures, leading to more extreme heat events, even in temperate climates like Aotearoa|New Zealand (A|NZ). The impact of rising temperatures and the adequacy of planning measures remain underexplored. This paper highlights A|NZ's anticipated heat-health challenges by analyzing vulnerable populations and assessing current response systems, thereby reinforcing the need for system-level redress, mitigation and adaptation. CONTENT: A scoping review examined the impact of heat and existing mitigation and adaptation responses for vulnerable popula…

A pilot cluster randomised controlled trial of a peer-delivered outreach community-based smoking cessation intervention for women living in disadvantaged communities in Ireland.

OBJECTIVES: Behçet's disease (BD) is a chronic, multisystemic disorder characterised by recurrent oral aphtous ulcers, genital ulcers and ocular inflammation. Vasculitis and thrombotic events are the most important causes of mortality. Vena cava thrombosis, pulmonary artery aneurysms, Budd-Chiari syndrome, peripheral artery aneurysms, dural sinus thrombosis and abdominal aorta aneurysms are the other less common vascular manifestations of BD. Cardiac involvement in BD is a rare and life-threatening complication. The aim of this study was to assess the clinical characteristics and outcome of pa

Motivational interview-based health mediator interventions increase intent to vaccinate among disadvantaged individuals.

Coverage for recommended COVID-19 and diphtheria-tetanus-poliomyelitis (DTP) booster shots is often inadequate, especially among disadvantaged populations. To help health mediators (HMs) involved in outreach programs deal with the problems of vaccine hesitancy (VH) in these groups, we trained them in motivational interviewing (MI). We evaluated the effectiveness of this training among HMs on their MI knowledge and skills (objective 1) and among the interviewees on their vaccination readiness (VR) and intention to get vaccinated or accept a booster against COVID-19 and/or DTP (objective 2). How has the COVID-19 pandemic affected young people?-Mapping knowledge structure and research framework by scientometric analysis.

Since the outbreak of COVID-19, there has been a large body of literature focusing on the relationship between the COVID-19 pandemic and young people. The purpose of this study is to explore the current research status and the specific mechanism of COVID-19's effects on young people based on related literature. This paper mainly used VOS viewer and CiteSpace software to conduct a scientometric analysis of 5,077 publications retrieved from the Web of Science database. The results show that the main contributors to the field were mainly from North America and Europe, and the trend of research fo

Identifying vulnerability in police custody: Making sense of information provided to custody officers.

Vulnerable individuals frequently come into contact with the criminal justice system with those with mental health disorders over-represented in custody. Therefore, it is crucial to identify vulnerability in order to ensure the appropriate safeguards can be put in place. Research has documented that this is problematic and can be influenced by a number of factors. The current study, therefore, aimed to understand how custody officers in England and Wales made sense of different types of information presented to them, the impact that it had on their initial disposal option and whether there was

Improving Care Outcomes for PLWH Experiencing Homelessness and Unstable Housing: a Synthetic Review of Clinic-Based Strategies.

PURPOSE OF REVIEW: Singular interventions targeting vulnerable populations of people living with HIV (PLWH) are necessary for reducing new infections and optimizing individual-level outcomes, but extant literature for PLWH who experience homelessness and unstable housing (HUH) has not been compiled. To inform implementation of clinic-based programs that improve care outcomes in this population, we present a synthetic review of key studies examining clinic-based interventions, specifically case management, patient navigation, financial incentives, and the use of mobile technology. Do transitions in and out of homelessness relate to mental health episodes? A longitudinal analysis in an extremely disadvantaged population.

This paper studies the association between homelessness and mental health episodes focusing on episodes of depression, anxiety and other conditions (bipolar affective disorder, schizophrenia, and post-traumatic stress disorder). Using panel data from the Australian Journeys Home survey, we investigate the extent to which this association is due to common determinants of homelessness and mental health episodes, or whether there is a causal link between them. The results indicate that episodes of depression increase the probability that a person becomes homeless. This is not the case for other m

Prevalence and Associated Factors of Depressive Symptoms Among Disadvantaged Adolescents: Results from a Population-Based Study in Bangladesh.

PROBLEM: Few studies have examined the adolescents' depression in low-income countries and no research has yet been carried out in Bangladesh. This study estimated the prevalence of depressive symptoms and explored the associated factors and help seeking behavior among adolescents in Bangladesh. METHODS: Data originated from a cross section of 2,440 randomly selected boys and girls aged 13-19 years in a rural district and urban slums of Dhaka city, Bangladesh, during October-November 2012. Beck Depression Inventory (BDI), a 21-item scale, measured the prevalence of depressive symptoms using a

Minding the gap: access to palliative care and the homeless.

BACKGROUND: With an ever increasing number of individuals living with chronic and terminal illnesses, palliative care as an emerging field is poised for unprecedented expansion. Today's rising recognition of its key role in patients' illnesses has led to increased interest in access to palliative care. It is known that homelessness as a social determinant of health has been associated with decreased access to health resources in spite of poorer health outcomes and some would argue, higher need. This article aims to discuss the current state of affairs with regards to accessing palliative care

Social vulnerability index for the older people-Hong Kong and New York City as examples.

Many world cities have suffered large-scale disasters, causing a significant loss of lives, property damage, and adverse social and economic impact. Those who are most vulnerable during and in the immediate aftermath of disaster crises are the elderly. Therefore, it is imperative to identify them and determine their specific needs in order to support them. Although several Social Vulnerability Indexes (SVIs) have been developed to assess different types of disaster vulnerability across geographic and population levels, few have been tailored to the older population. Building on the research of

Intrauterine determinants of diabetic kidney disease in disadvantaged populations.

Disadvantaged populations worldwide are experiencing an increasing incidence of kidney disease, much of which is attributable to diabetes. This report reviews the evidence that intrauterine exposure to growth retardation, diabetes, and vitamin A deficiency contribute disproportionately to the rising incidence of kidney disease in disadvantaged people, because they encounter these exposures more frequently than people from developed countries. These abnormal intrauterine exposures reduce nephron mass by impairing nephrogenesis, thereby increasing the susceptibility to kidney damage from disease

Accessing and Recruiting a Community-Based Sample of Men of Mexican Origin to Explore Intimate Partner Violence.

INTRODUCTION: Vulnerable populations are at greater risk for the burden of disease and negative health outcomes including alcohol abuse, high-risk sexual behavior, and intimate partner violence (IPV). Moreover, health disparities experienced by vulnerable populations are mediated by socioeconomic, ethnic, and racial factors. Risk factors for IPV perpetration are exacerbated by cultural and gender norms, particularly among men of Mexican origin. PURPOSE: To discuss research methods used in a community-based sample of men of Mexican origin and ways to overcome recruitment barriers through commu…
