BACKGROUND: Endometriosis of the canal of Nuck is an extremely rare manifestation of extrapelvic endometriosis that may mimic common groin pathologies such as inguinal hernia or hydrocele, often leading to diagnostic difficulty and delayed recognition.

Vulval and vaginal symptoms are common after the menopause and are frequently assumed to be due to the normal physiological changes that occur at this time. However, there are several important dermatoses that can occur in this patient group which need accurate diagnosis and appropriate management. This review discusses the clinical features and basic management of some of the common vulval problems occurring after the menopause.

BACKGROUND: Angiomyofibroblastoma (AMF) is a rare benign mesenchymal neoplasm that arises in the pelviperial region.
CASE: A patient presented with a painless mass in the right vulva. Under the preoperative diagnosis of Bartholin cyst, she underwent a simple tumor excision. Pathological examination revealed an AMF. Immunohistochemical examination showed that tumor cells were positive for estrogen receptor, progesterone receptor, vimentin, and CD34. She has been with no evidence of local recurrence for ten months after surgery.

Treatment results in 360 patients with vulvar carcinoma during the time period from 1975 to 1994 were evaluated. Out of the whole group radical vulvectomy with bilateral inguinal lymphadenectomy was performed in 215 patients. After excluding 35 patients having the Stage IV of the disease, 11 with malignant melanoma and 9 missing, in the group of remaining 160 cases with squamous cell carcinoma the survival rates of 3, 5 and 10 years were evaluated. The gynaecological problems of neurofibromatosis (NF) are not well documented. The present survey of the literature defines 3 main problems, namely: clitoral, vulvar and pelvic NF. The clinical suspicion of NF would in most cases improve patient care and prevent undue procedures. There remain some important unsettled questions to be answered.

Radiation therapy plays a critical role in the management of locally advanced vulvar cancers but can lead to a unique spectrum of side effects, with >25% of patients experiencing high-grade toxicities. The treatment phase requires meticulous perineal skincare and may require pharmacologic management of dysuria and cystitis, diarrhea, nausea, and dermatitis/mucositis. The addition of chemotherapy warrants close laboratory monitoring for hematologic and metabolic derangements.

Vulvar leiomyoma is rare and is often misdiagnosed as a cyst or abscess in the Bartholin's glands. Other causes of benign tumours of the vulva are Gartner's duct cysts, fibromas, fibroadenomas, lipomas and hamartomas. Adenoma was the tentative diagnosis is this case history, but the histology showed a benign leiomyoma.
