The Reptilian Renal Wrench: Acute Kidney Injury in the Wake of Snakebite Envenoming.

Snakebite-induced acute kidney injury (AKI) is a severe, potentially fatal complication driven by venom's direct toxicity, inflammation, and complex mechanisms such as intravascular hemolysis, rhabdomyolysis, and coagulation disorders. While early antivenom administration and supportive care are crucial, many survivors may face long-term renal dysfunction, emphasizing the need for advanced understanding to prevent severe outcomes and improve patient management. We present a case of an 81-year-old farmer who sustained a snakebite injury and developed local and systemic features of envenomation

Time to wake up to the potential benefits of targeting sleep in osteoarthritis management.

Sleep disturbance is highly prevalent in people with osteoarthritis (OA) yet it is rarely considered in routine care. Emerging evidence suggests that sleep disturbance contributes to heightened musculoskeletal pain and poorer physical and emotional outcomes in those with OA. This narrative review synthesises current knowledge on the epidemiology and impact of sleep disturbances in OA, including their associations with and impact on fatigue, depressive symptoms, pain catastrophising, and negative mood. It examines the effects of core OA treatments such as exercise, diet and weight loss on sleep

Tryptamine from wake-active monoaminergic neurons regulates sleep homeostasis.

Wakefulness produces sleep-promoting substances and the cerebrospinal fluid contains substances that reflect homeostatic sleep pressure. However, identities of such molecules, and the neural mechanisms for producing and sensing them, remain mysterious. Here we show that cerebrospinal fluid levels of tryptamine (TrpA) track homeostatic sleep pressure in nocturnal mice and diurnal pigs, reflecting physical activity history independently of light-dark cycles. We developed a ratiometric fluorescent sensor for TrpA and showed that TrpA is produced by wake-active monoaminergic nuclei in the dienceph…

Optimal wake-up time for short-term peak performance in the morning.

BACKGROUND: Sports performance is low in the morning compared with those later in the day, and taking steps to improve performance in the morning is crucial. The awakening timing is important; however, few studies have focused on this issue. The aim of this study was to clarify the optimal wake-up time for short-term peak performance in the morning. METHODS: Fourteen male college baseball players completed three performance tests changed with the elapsed time from awakening (ET); 1, 2.5, and 4 h (1H, 2.5H, and 4H, wake-up times were 09:00, 07:30, and 06:00, respectively). Performance tests st

Effect of lemborexant on sleep-wake state discrepancy in participants with insomnia disorder.

RATIONALE: The efficacy of hypnotic drugs for sleep-wake state discrepancy remains unclear. OBJECTIVE: This study evaluated the efficacy of lemborexant in improving sleep-wake state discrepancy in insomnia disorder and its associations with sleep quality and daytime functioning. METHODS: Twenty-nine participants diagnosed with insomnia disorder were treated with lemborexant for 12 weeks. Objective sleep parameters were measured using a home electroencephalography device, and subjective parameters were obtained from electronic sleep diaries. Sleep-wake state discrepancy indices, including dis

COVID-19 Pandemic: Wake-up Call and Accelerator for Cardiac Rehabilitation.

Cardiac rehabilitation (CR) is a cornerstone of secondary prevention in cardiovascular care, improving survival, reducing rehospitalization, and enhancing quality of life. Despite robust evidence and strong guideline support, CR remains markedly underutilized in Canada and globally, with significant disparities by sex, race, geography, and socioeconomic status. The COVID-19 pandemic disrupted more than three-quarters of CR programs worldwide, exposing deep-rooted limitations in access, infrastructure, and delivery models. At the same time, the pandemic served as a catalyst for innovation. Rapi

The Impact of Quantifying Human Locomotor Activity on Examining Sleep-Wake Cycles.

Actigraphy quantifies human locomotor activity by measuring wrist acceleration via wearable devices at relatively high rates and converting it into lower-temporal-resolution activity values; however, the computational implementations of this data compression differ substantially across manufacturers. Building on our previous work comparing activity determination methods, we have investigated how they (e.g., digital filtering and data compression) influence nonparametric circadian rhythm analysis and sleep-wake scoring. In addition to our generalized actigraphic framework, we have also emulated

CROSsing the Rubicon: Brainstem peroxide levels drive sleep-wake dynamics.

A crucial goal of sleep science is to uncover the molecular mechanisms that translate sleep need into neural pathways that discharge sleep/wake transitions. In a new study, Tian et al.1 provide support in living animals that peroxide in the substantia nigra is a critical driver of these states.

mCLAS adaptively rescues disease-specific sleep and wake phenotypes in neurodegeneration.

Sleep alterations are hallmarks of prodromal Alzheimer's (AD) and Parkinson's disease (PD), with fundamental neuropathological processes of both diseases showing susceptibility of change upon deep sleep modulation. However, promising pharmacological deep sleep enhancement results are hindered by specificity and scalability issues, thus advocating for noninvasive slow-wave activity (SWA) boosting methods to investigate the links between deep sleep and neurodegeneration. Accordingly, we have recently introduced mouse closed-loop auditory stimulation (mCLAS), which is able to successfully boost S

Posttraumatic Stress Symptoms and Posttraumatic Cognitions in the Wake of the Death of Michael Brown.

On August 9, 2014, Michael Brown was shot and killed by police officer Darren Wilson in Ferguson Missouri, sparking protests and civil unrest. Three studies have yielded inconsistent findings regarding the presence of posttraumatic stress symptoms (PTSSs) in the aftermath of the unrest in Ferguson. Additional work is needed to understand how exposure to community-level stressors may correspond with trauma-related outcomes, as well as accounting for knowledge of, and engagement in the events. Posttraumatic cognitions have also not been researched. The present study examined the associations bet

Electrophysiological Properties of the Medial Mammillary Bodies across the Sleep-Wake Cycle.

The medial mammillary bodies (MBs) play an important role in the formation of spatial memories; their dense inputs from hippocampal and brainstem regions makes them well placed to integrate movement-related and spatial information, which is then extended to the anterior thalamic nuclei and beyond to the cortex. While the anatomical connectivity of the medial MBs has been well studied, much less is known about their physiological properties, particularly in freely moving animals. We therefore carried out a comprehensive characterization of medial MB electrophysiology across arousal states by co

Wake-Up Thrombolysis With Negative Diffusion-Weighted Magnetic Resonance Imaging.

This case underscores the importance of treating neurological deficits of an acute stroke presentation despite initial negative diffusion-weighted imaging (DWI), especially in the acute phase when there is high clinical suspicion of stroke. Additionally, it highlights the appropriate use of a WAKE-UP protocol for patients that present with stroke symptoms without a well-defined inception time. A 71-year-old female presented to the emergency department with symptoms of dizziness and double vision upon wake-up. While clinical exam findings revealed left intranuclear ophthalmoplegia (INO) and ina

Examining the association between depersonalisation traits and the bodily self in waking and dreaming.

Depersonalisation (DP) is characterized by fundamental alterations to the sense of self that include feelings of detachment and estrangement from one's body. We conducted an online study in healthy participants (n = 514) with DP traits to investigate and quantify the subjective experience of body and self during waking and dreaming, as the vast majority of previous studies focussed on waking experience only. Investigating dreams in people experiencing DP symptoms may help us understand whether the dream state is a 'spared space' where people can temporarily 'retrieve' their sense of self and s

The shadow pandemic: rising syphilis rates in the wake of coronavirus (COVID-19).

The coronavirus disease 2019 (COVID-19) aftermath left an alarming surge in syphilis cases, contradicting the previously stable trajectory of the infection. US Centers for Disease Control and Prevention also reported a 38% increase in primary and secondary syphilis in 2021 compared to 2019 in the United States, prompting a retrospective analysis at our tertiary care centre in New Delhi, India. There was a persistent linear rise, surpassing pre-COVID levels. Male clinic attendees, exhibit a pronounced increase, likely due to the influence of MSM. Intravenous Thrombolysis in Patients With White Matter Hyperintensities in the WAKE-UP Trial.

BACKGROUND: White matter hyperintensities of presumed vascular origin (WMH) are the most prominent imaging feature of cerebral small vessel disease (cSVD). Previous studies suggest a link between cSVD burden and intracerebral hemorrhage and worse functional outcome after thrombolysis in acute ischemic stroke. We aimed to determine the impact of WMH burden on efficacy and safety of thrombolysis in the MRI-based randomized controlled WAKE-UP trial of intravenous alteplase in unknown onset stroke. METHODS: The design of this post hoc study was an observational cohort design of a secondary analys

Problems in the Development of the Sleep-Wake Rhythm Influence Neurodevelopmental Disorders in Children.

Development of the sleep-wake rhythm has a significant effect on the physical and mental development of children. The sleep-wake rhythm is controlled by aminergic neurons in the brainstem's ascending reticular activating system, which is associated with synaptogenesis and the promotion of brain development. The sleep-wake rhythm develops rapidly within the first year after birth. At 3-4 months of age, the framework of the circadian rhythm is established. The objective of the present review is to assess a hypothesis concerning problems in the development of the sleep-wake rhythm and their effec

Is There a Relationship Between Waking Up from Sleep and the Onset of Testicular Torsion?

Testicular torsion is a serious urologic emergency that can present with unusual or atypical history and examination. Classical pain from testicular torsion is of sudden onset, significantly severe, and is accompanied by nausea and vomiting. However, in some patients, the initial scrotal pain appears to considerably subside within the next few hours. In others, testicular torsion tends to occur while sleeping, and many patients recount a history of being woken up from sleep by intense pain. Furthermore, some patients in this subset can resume normal activities and even sleep through the night

Sleep/Wake Disorders After Sports Concussion: Risks, Revelations, and Interventions.

Sleep-wake disturbances (SWDs) are among the most prevalent, persistent, and often disregarded sequelae of traumatic brain injury. Identification and treatment of SWDs in patients with traumatic brain injury is important and can complement other efforts to promote maximum functional recovery. SWDs can accentuate other consequences of traumatic brain injury, negatively affect mood, exacerbate pain, heighten irritability, and diminish cognitive abilities and the potential for recovery. The risk for sports injuries increases when athletes are sleep deprived. Sleep deprivation increases risk-takin
