BACKGROUND: To investigate patient-ventilator interaction during different levels of noninvasive proportional assist ventilation (PAV) compared with noninvasive pressure support ventilation (PSV).
METHODS: Fifteen subjects with severe COPD and hypercapnia were consecutively recruited. After the baseline assessment of unassisted spontaneous breathing, 3 levels of ventilatory support were applied. The proportional assist (PA) and pressure support (PS) levels were set by subject comfort. PA-, PS- or PA+, PS+ were set at 25% more or less of PA or PS (PA- = 75% PA, PA+ = 125% PA, PS- = 75% PS, PS+

Heated, humidified, high-flow nasal cannula (HHHFNC) is increasingly being used, but there is a paucity of evidence as to the optimum flow rates in prematurely born infants. We have determined the impact of three flow rates on the work of breathing (WOB) assessed by transcutaneous diaphragm electromyography (EMG) amplitude in infants with respiratory distress or bronchopulmonary dysplasia (BPD). Flow rates of 4, 6 and 8 L/min were delivered in random order. The mean amplitude of the EMG trace and mean area under the EMG curve (AEMGC) were calculated and the occurrence of bradycardias and desat…

We reviewed the literature on the effects of high flow nasal cannula (HFNC) and heated, humidified, high-flow, nasal cannula (HHHFNC) treatment in preterm infants. We found nine studies, but only two were randomized controlled trials. These studies show that: HFNC application is associated to the delivery of continuous distending pressure (CDP) in patients with closed mouth, whose value is proportional to the delivered flow only in smaller infants; the CDP delivered by HFNC is unpredictable and present large inter-patient and intra-patient variability; the use of recently available HHHFNC devi

INTRODUCTION: Oxygen toxicity is a problem in diving and can have fatal consequences in the water. Various aspects of oxygen diving have been studied in dry hyperbaric chambers, but there is a lack of information on in-water diving using closed-circuit oxygen apparatus.
METHOD: We collected 2527 dive reports from 473 closed-circuit oxygen divers (a mean of 5.2 reports per diver), and analyzed the relationships between various symptoms and their dependence on depth and diving time.
RESULTS: No CNS oxygen toxicity-related symptoms were reported at a depth of 2 m seawater (msw), but their propo

Oesophageal pressure (Pes) and oronasal flow are necessary to describe upper airway obstruction in patients with obstructive sleep apnoea syndrome (OSAS), but Pes interferes with sleep. We developed a device applying an oscillating flow (20 Hz) through a nasal mask. An additional flow (2.6 l/min) is needed to reduce dead space and humidity. 24 patients (age 55.8 +/- 8.3 years, BMI 28.6 +/- 3.9, RDI 38.6 +/- 19.4, Raw 0.27 +/- 0.07 kPa/s/l) underwent polygraphy (oronasal flow, thoracic and abdominal effort, oxygen saturation, microphone, heart rate). Pes and oscillatory impedance (OI) were meas

Oesophageal pressure swings were analysed during complete occlusion of the upper airways in 8 patients with obstructive sleep apnoea. We hypothesed that in dependence of the apnoea type, there is a significant decrease in oesophageal inspiratory effort in REM vs. NREM that is caused by a decrease of the respiratory drive. We analysed in each patient 40 apnoeas in which the respiratory timing (Ti) and the inspiratory effort (Pin) were calculated and compared with the type of apnoea and the degree of oxygen desaturation. A significant decrease in inspiratory effort in REM vs NREM occurred only i

While exposure to air contaminants from metal arc welding at workplaces has been intensively investigated over the last five decades, other hot work processes, such as flame and plasma cutting, air carbon arc gouging, and surface grinding have not received as much attention. Exposures to particulate matter (PM) during selected hot work processes, such as metal active gas (MAG) and manual metal arc (MMA) welding, flame and plasma cutting, air carbon arc gouging, and surface grinding were measured. Respirable, inhalable, and "total" fractions of the PM were collected with different air samplers

BACKGROUND: Patients with heart failure with preserved ejection fraction (HFpEF) have pulmonary system abnormalities resulting in impaired ventilatory reserve and heightened respiratory muscle work during submaximal exercise.
RESEARCH QUESTION: Do patients with HFpEF have higher respiratory muscle oxygen uptake (V˙o2) during submaximal exercise than control participants?
STUDY DESIGN AND METHODS: Patients with HFpEF (n = 13) and control participants (n = 16) completed 2 visits. The first visit included pulmonary function tests and an incremental exercise test. On the second visit, participan

INTRODUCTION: This paper describes the research work done toward the development of a breath analyzing electronic nose (e-nose), and the results obtained from testing patients with lung cancer, patients with chronic obstructive pulmonary disease (COPD), and healthy controls. Pulmonary diseases like COPD and lung cancer are detected with MOS sensor array-based e-noses. The e-nose device with the sensor array, data acquisition system, and pattern recognition can detect the variations of volatile organic compounds (VOC) present in the expelled breath of patients and healthy controls.

BACKGROUND: Portable breath ketone sensors may help people with Type 1 Diabetes Mellitus (T1DM) avoid episodes of diabetic ketoacidosis; however, the design features preferred by users have not been studied. We aimed to elucidate breath sensor design preferences of young people with T1DM (age 12 to 16) and their parents to inform the development of a breath ketone sensor prototype that would best suit their diabetes management needs.
RESEARCH DESIGNS AND METHODS: To elicit foundational experiences from which design preference ideas could be generated, two commercially available breath ketone

A case is described of a 37-year-old man who was breath tested by the U.K. police following a traffic accident and was found to have a breath-alcohol concentration of 70 micrograms/100 mL--twice the U.K. legal limit. The defendant protested vigorously that he had not consumed sufficient alcohol to account for this excessive reading, and Widmark calculations, assuming his account of the alcohol consumed was accurate, suggested his contention was possibly correct. No evidence was obtained suggesting that the Lion Intoximeter used for the breath analysis was anything other than accurate. Alcohol

Impaired pulmonary gas exchange can result from lung parenchymal failure inducing oxygenation deficiency and fatigue of the respiratory muscles, which is characterized by hypercapnia or a combination of both mechanisms. Contractility of and coordination between the diaphragm and the thoracoabdominal respiratory muscles predominantly determine the efficiency of spontaneous breathing. Sepsis, cardiac failure, malnutrition or acute changes of the load conditions may induce fatigue of the respiratory muscles. Augmentation of spontaneous breathing is not only achieved by the application of differen

Cocaine (COC) abuse during pregnancy may be a factor in the development of neonatal breathing abnormalities. To examine this possibility, pr … ent functional teratogenesis and either results in a greater ventilatory effort which increases the work of breathing and the consumption of oxygen. An increase in oxygen demand due to an increase in metabolism or an increase in ventilatory effort might compromise some neonates and contribute to an increased incidence of sudden-infant-death.

RATIONALE: Computational fluid dynamics (CFD) simulations of respiratory airflow can quantify clinically useful information that cannot be obtained directly, such as the work of breathing (WOB), resistance to airflow, and pressure loss. However, patient-specific CFD simulations are often based on medical imaging that does not capture airway motion and thus may not represent true physiology, directly affecting those measurements.
OBJECTIVES: To quantify the variation of respiratory airflow metrics obtained from static models of airway anatomy at several respiratory phases, temporally averaged …

BACKGROUND: New mass spectrometry (MS) techniques analysing exhaled breath have the potential to better define airway diseases. Here, we present our work to profile the volatile organic compounds (VOCs) in exhaled breath from patients with chronic obstructive pulmonary disease (COPD), using real-time MS, and relate this disease-specific breath profile to functional disease markers.
METHODS: In a matched cohort study, patients with COPD, according to GOLD criteria, were recruited. Exhaled breath analysis by untargeted MS was performed using secondary electrospray ionization - high-resolution M

BACKGROUND: Neural circuits produce reliable activity patterns despite disturbances in the environment. For this to occur, neurons elicit synaptic plasticity during perturbations. However, recent work suggests that plasticity not only regulates circuit activity during disturbances, but these modifications may also linger to stabilize circuits during future perturbations. The implementation of such a regulation scheme for real-life environmental challenges of animals remains unclear. Amphibians provide insight into this problem in a rather extreme way, as circuits that generate breathing are in
