Online Mode of Tobacco Hazards Awareness for Capacity Building and Awareness Generation: A Promising Initiative in Addition to the Existing Tobacco Control Measures.

Our tobacco hazards awareness work in Mumbai since 2014 had to be stopped with the onset of the coronavirus disease 2019 (COVID-19) pandemic. There was a need to prioritize and strengthen tobacco cessation efforts and disseminate information in society and to medical professionals and healthcare workers to highlight such facilities. Hence, this online tobacco hazards awareness initiative was undertaken on various platforms for a diverse audience. This included: 1. Creating awareness in society about the hazards of tobacco and the facilities available for cessation through articles for online n

Impact of a pilot leadership development programme for rural and remote primary care physicians in Aceh, Indonesia: a Kirkpatrick level 3 evaluation.

BACKGROUND: While physicians are crucial to rural or remote healthcare systems, they are rarely trained as effective leaders, particularly in low-income and middle-income countries context. We conducted a 2-day workshop using the LEADS framework to assess whether rural or remote primary care physicians in Aceh, Indonesia, benefited from the leadership development programme and how their professional behaviour changed 6 months post-workshop. METHODS: We applied a mixed-methods study. 10 rural and remote primary care physicians who attended the 2-day workshop were interviewed using a self-repor

Barriers and Facilitators for Participation in Global Health Research Training Programs Among Underrepresented Minority Groups.

The optimal global health (GH) workforce should be racially and ethnically diverse, yet few persons from historically underrepresented minority (URM) groups in the United States participate in GH training programs. We conducted a study to explore barriers and facilitators for URM individuals to participate in the NIH Fogarty International Center's GH Program for Fellows and Scholars (FGHFS), which offers yearlong international research training opportunities. We used an exploratory sequential mixed methods study design that used qualitative in-depth interviews (n = 18) to inform a subsequent q

Palliative Care Integration in Oncology: A Review and Update.

Palliative care (PC) is increasingly recognized as an essential component of high-quality cancer care, with evidence from randomized trials and meta-analyses demonstrating that it improves quality of life, mood, and goal-concordant end-of-life care. Despite these benefits, PC integration has been inconsistent, with many patients still receiving PC later in their disease course. Current models of integration include outpatient co-located PC clinics, inpatient consultation services, community- and home-based programs, and more recent innovations, such as telehealth and stepped approaches. These

Opportunities and challenges for paediatricians requesting funded genomic tests for children.

Genomic diagnosis for children with childhood syndromes can inform treatment, management and reproductive planning. Shortages in the clinical genetics workforce mean that practices are changing, with paediatricians likely requesting initial genomic investigations and clinical genetics services reserved for particularly complex cases or post-test genetic counselling. In Australia, paediatricians can request funded genomic testing for patients, yet ordering rates are less than a quarter predicted. Using a theory-informed approach, we aimed to understand barriers and enablers to general paediatri

Systematic review of the effectiveness of health promotion interventions targeting obesity prevention in school-based staff.

School-based employee interventions can benefit the health of staff and have the potential to influence the health of school students through role-modelling. However, interventions within schools typically focus on students, with very few studies addressing obesity and related health behaviours among school staff. A systematic review of the peer-reviewed literature published between January 2000 and May 2020 was undertaken to synthesize the evidence on the impact that school-based obesity prevention programmes have on the staff they employ. Search terms were derived from four major topics: (i)

Cardiac surgeons' concerns, perceptions, and responses during the COVID-19 pandemic.

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic has had an unprecedented impact on health care and cardiac surgery. We report cardiac surgeons' concerns, perceptions, and responses during the COVID-19 pandemic. METHODS: A detailed survey was sent to recruit participating adult cardiac surgery centers in North America. Data regarding cardiac surgeons' perceptions and changes in practice were analyzed. RESULTS: Our study comprises 67 institutions with diverse geographic distribution across North America. Nurses were most likely to be redeployed (88%), followed by advanced care pra

A mixed-method comparison of physician-reported beliefs about and barriers to treatment with medications for opioid use disorder.

BACKGROUND: Evidence demonstrates that medications for treating opioid use disorder (MOUD) -namely buprenorphine, methadone, and extended-release naltrexone-are effective at treating opioid use disorder (OUD) and reducing associated harms. However, MOUDs are heavily underutilized, largely due to the under-supply of providers trained and willing to prescribe the medications. METHODS: To understand comparative beliefs about MOUD and barriers to MOUD, we conducted a mixed-methods study that involved focus group interviews and an online survey disseminated to a random group of licensed U.S. physi

Choosing wisely: the American College of Rheumatology's Top 5 list of things physicians and patients should question.

OBJECTIVE: We sought to develop a list of 5 tests, treatments, or services commonly used in rheumatology practice whose necessity or value should be questioned and discussed by physicians and patients. METHODS: We used a multistage process combining consensus methodology and literature reviews to arrive at the American College of Rheumatology's (ACR) Top 5 list. Rheumatologists from diverse practice settings generated items using the Delphi method. Items with high content agreement and perceived high prevalence advanced to a survey of ACR members, who comprise >90% of the US rheumatology work

Child and adolescent psychiatry: challenges, solutions, opportunities, and future directions.

It is estimated that, globally, the mean point prevalence of diagnosable mental disorders in children and adolescents is higher than 11%, and around half of cases of major mental disorders have their onset before the age of 18. Mental disorders with onset in childhood or adolescence have an enormous impact on the developing brain, body and personal identity, as well as on the short- and long-term social, educational and functional capacity of individuals. Child and adolescent psychiatry - as a discipline, profession, academic field, and network of clinical services - is still relatively young,

Beyond biomedical control: governance, social protection, and system gaps in tuberculosis control in Kazakhstan - a qualitative study of decision-makers.

BACKGROUND: Tuberculosis (TB) remains a major public health challenge globally and in Kazakhstan, despite progress in reducing incidence. Increasing evidence highlights the role of social and structural determinants in shaping TB risk, disease progression, and treatment outcomes. However, limited research has explored how decision-makers conceptualize these factors and identify system-level gaps in TB control. This study aimed to examine regional decision-makers' perspectives on TB burden, social protection, health system capacity, and governance in Kazakhstan. METHODS: A qualitative study wa

Associated factors, barriers, and interventions to promote physical activity and reduce sedentary time in academics: a systematic review.

BACKGROUND: Low physical activity and prolonged sedentary time harm health and psychological well-being. Being insufficiently active is associated with a risk of numerous non-communicable diseases, including cardiovascular disease, hypertension, and diabetes. Academics comprise a notable portion of the workforce and serve as important role models, yet their positions may involve many sedentary behaviours. The factors associated with their physical activity and sedentary behaviours are unclear. This review aimed to identify the associated factors, barriers, and the effectiveness of intervention

Understanding the Medical Education Experiences of Low-Income Students Through a Maslow's Hierarchy of Needs Lens: An Exploratory Qualitative Study.

BACKGROUND: Diversity in the physician workforce is critical for quality patient care. Students from low-income backgrounds represent an increasing proportion of medical school matriculants, yet little research has addressed their medical school experiences. OBJECTIVE: To explore the medical school experiences of students from low-income backgrounds using a modified version of Maslow's Hierarchy of Needs (physiologic, safety, love/belonging, esteem, and self-actualization) as a theoretical framework. DESIGN: We conducted an exploratory qualitative study through in-depth, semi-structured inte

SHEA position statement on pandemic preparedness for policymakers: introduction and overview.

Throughout history, pandemics and their aftereffects have spurred society to make substantial improvements in healthcare. After the Black Death in 14th century Europe, changes were made to elevate standards of care and nutrition that resulted in improved life expectancy.1 The 1918 influenza pandemic spurred a movement that emphasized public health surveillance and detection of future outbreaks and eventually led to the creation of the World Health Organization Global Influenza Surveillance Network.2 In the present, the COVID-19 pandemic exposed many of the pre-existing problems within the US h

Race-based experiences of ethnic minority health professionals: Arab physicians and nurses in Israeli public healthcare organizations.

UNLABELLED: Increasing workforce diversity was found to contribute to the narrowing of disparities in health. However, racism toward ethnic minority health professionals has not been adequately researched. In Israel, public healthcare organizations that serve a mixed Jewish-Arab population employ Arab minority healthcare professionals. Instances of prejudice and manifestations of racism toward them, which frequently surface in public discussion and the media, have unfortunately gained little scholarly attention. We used the intergroup contact approach and the theory of the social process of ev…

A Shadow of Doubt: Is There Implicit Bias Among Orthopaedic Surgery Faculty and Residents Regarding Race and Gender?

BACKGROUND: Orthopaedic surgery continues to be one of the least diverse medical specialties. Recently, increasing emphasis has been placed on improving diversity in the medical field, which includes the need to better understand existing biases. Despite this, only about 6% of orthopaedic surgeons are women and 0.3% are Black. Addressing diversity, in part, requires a better understanding of existing biases. Most universities and residency programs have statements and policies against discrimination that seek to eliminate explicit biases. However, unconscious biases might negatively impact the

Making continuity of midwife care implementable across health systems: A global qualitative study.

BACKGROUND: Continuity of midwife care is associated with improved maternal and neonatal outcomes, reduced intervention, and better care experiences, yet implementation across health systems remains uneven. Existing research has largely identified barriers and facilitators, with less attention to how continuity is made implementable across different health system contexts. OBJECTIVE: To identify the recurring forms of contextual and system-level work required to make continuity of midwife care implementable across diverse global settings, from the perspectives of midwives and implementation s

COVID-19 status and utilisation of essential maternal and child healthcare services during the pandemic in Ahmedabad, India.

BACKGROUND: Access to essential healthcare services is pertinent to the achievement of universal health coverage in any nation. The COVID-19 lockdown was used to mitigate the spread of the pandemic. Consequently, there was a reduction in the Utilisation of Basic Healthcare Services (UBHS) in diverse dimensions. However, variation existed in the UBHS by COVID-19 status, but the extent of this disparity has not been extensively addressed in Ahmedabad, India. Therefore, this study explores the relationship between COVID-19 status and utilisation of essential maternal and child healthcare services
