A case of a 12-year-old boy with sacroiliitis documented by positive Tc-99m MDP and Ga-67 scans is described. Isolation of brucella melitensis from the blood and bone marrow established the diagnosis. He responded promptly to docycycline therapy. Throughout the course of his disease this boy had neither fever nor other signs of brucellosis, and x-ray was normal.

Six cases are presented which exhibit a peculiar type of follicular degeneration leading to alopecia and characterized by edema and the presence of mucin in the outer root sheath and sebaceous gland. This is accompanied by varying degrees of inflammation. The clinical lesions are sharply defined plaques which enlarge slowly and seem to be curable by relatively small doses of x-ray therapy.

Limited indications for radiation treatment of benign dermatoses are presented in order to emphasize modern restrictions and techniques in the use of dermatologic radiation therapy and to promote measures of protection against stray radiation. The use of softer, less penetrating X rays, least effective doses and meticulous shielding are recommended. So done, radiation therapy offers an effective therapeutic alternative for conditions that do not respond to other forms of treatment.

A 37-year-old woman was admitted with fever and cough. The chest roentgenogram revealed a solitary large round mass in the right lower lung field. Diagnosis was made by transbronchial lung biopsy. There was no evidence of abnormal cell-mediated immunity. The patient made an uneventful recovery and the chest X-ray showed progressive clearing without specific therapy. After 6 months, the mass lesion disappeared. This case suggests that specific therapy may not be necessary for all cases of primary pulmonary cryptococcosis.

Deep pelvic venous thrombosis is a severe complication during pregnancy and requires an early diagnosis and an adequate well-timed therapy. 12 pregnant women with thrombosis of deep pelvic veins have been registered ++ over a period of 10 years. Diagnosis was secured with phlebography in 5 of these cases. It is reported about the necessity and the x-ray exposure of the examination. Treatment of choice is the heparin-longtime-therapy 90 per cent of our cases have been cured successfully.

A study of post-orchiectomy surveillance without radiation therapy was done in patients with histologically pure seminoma apparently confined to the testicle. We report a preterm newborn diagnosed as having congenital cytomegalovirus infection who developed lung cysts during her clinical follow-up. The lung cysts were shown in chest X-ray, and confirmed by thoracic computed tomography. A few weeks after starting ganciclovir therapy, we observed that the cystic lung changes of the patient were completely improved. No adverse effects of antiviral therapy were observed. Based on this presented patient, it should be kept in mind that congenital cytomegalovirus infection may cause lung cysts.

Immune reconstitution inflammatory syndrome (IRIS) in the setting of antiretroviral therapy is well described, but it is not as common in non-HIV patients; here, we present a case of immune reconstitution inflammatory syndrome presenting as acute respiratory distress syndrome in a leukemia patient who had neutropenic fever and septic shock after high-dose cytarabine. During neutropenia recovery, his chest X-ray showed progressive worsening despite being on adequate therapy, we started him on steroids which resulted in significant clinical improvement.

Tuberculosis of the esophagus is a rare disease, occurring in most cases secondary to tuberculous infection of other organs. There are neither characteristic symptoms or diagnostic signs, nor typical X-ray or laboratory findings. Histology, too, often fails to establish a reliable diagnosis, so that esophageal tuberculosis may be mistaken for esophageal carcinoma. Most cases can be successfully treated with antituberculous therapy, although patients with esophageal tuberculosis in the presence of AIDS do not respond as well to anti-tuberculous therapy.

In two patients with mycosis fungoides, a squamous cell carcinoma developed during therapy with psoralens plus long-wave ultraviolet radiation (PUVA). In both patients the carcinoma developed on areas not usually exposed to sunlight. Before receiving PUVA therapy one patient had been treated with topically applied thiotepa and x-ray irradiation, while in the other patient PUVA was the only treatment received except for topical corticosteroids. The carcinomas developed after a total UVA dose of 2,042 and 1,296 joules/sq cm, respectively.

A 3-year followup on ten of 12 patients with mycosis fungoides (MF) treated with photochemotherapy (PUVA; psoralens + ultraviolet light) reveals eight patients whose diseases have remained clear while receiving maintenance PUVA. An additional 35 patients with MF have been successfully treated with PUVA. Eight cases of parapsoriasis en plaque have cleared with PUVA. PUVA is indicated in early eczematous and plaque stages of MF. It may be used as adjunctive therapy in tumor-stage MF treated with X-ray therapy or chemotherapy.

Incomplete atypical femur fractures (iAFFs) are associated with the long-term use of anti-resorptive therapies. Although X-rays are typically used to screen for iAFFs, images from dual-energy X-ray absorptiometry (DXA) offer an alternate method for detecting iAFFs. Although a previous 2019 ISCD Official Position on this subject exists, our task force aimed to update the literature review and to propose recommendations on reporting findings related to iAFFs that may be observed on DXA images. AIM OF THE STUDY: Extracorporeal shock wave therapy (ESWT) is a new therapeutic procedure for chronically painful calcifying tendinitis of the rotator cuff. The therapy may vary with the number of applied impulses or with impulse energy. Shock waves with an energy of 0.04 to 0.12 mJ/mm2 define low-dose ESWT, in contrast to high-dose ESWT (> 0.12 mJ/mm2).
