Distal locking screw migration in the depuy-synthes retrograde femoral nail-advanced-an unexpected consequence of an implant upgrade.

AIMS: The Retrograde Femoral Nailing Advanced (RFNA) system is a new generation of retrograde femoral nail (RFN) introduced to extend the range of distal fractures treated by intramedullary (IM) nailing. There is concern that the rate of distal interlock screw back-out may be higher for the RFNA. This study aims to highlight a higher-than-expected incidence of distal locking screw migration. METHOD: Patient and implant data were prospectively collected from theatre and electronic patient records. The study focused on monitoring the migration of distal locking screws, treating each screw as an

Incidence of Rotational Malalignment after Intertrochanteric Fracture Intramedullary Nailing: A CT-Based Prospective Study.

Background and Objectives: Rotational malformation after intramedullary nailing of intertrochanteric fractures is a relatively common, possibly severe, and difficult-to-detect complication, since intraoperative radiographic imaging allows for the assessment of the quality of reduction in the frontal and sagittal planes, but not in the transverse plane. The purpose of this study is to evaluate the rotational malalignment after intramedullary nailing of intertrochanteric fractures and to investigate a possible connection with specific patients' or fractures' characteristics. Materials and Method

The association between intracompartmental pressure and skeletal muscle recovery after tibial diaphyseal fractures: an ambispective cohort study.

BACKGROUND: Due to the special anatomy of the lower leg, tibial diaphyseal fracture causes increased intracompartmental pressure (ICP). Not only is this increased ICP the manifestation of skeletal muscle injury, but it induces further deterioration of the injury. The aim of this study was to assess the association between short-term ICP elevation and long-term skeletal muscle recovery after severe limb trauma. METHODS: In this single-center ambispective cohort study, we retrospectively screened and recruited a cohort of tibial diaphyseal fracture patients with integrated ICP data during the e

The impact of intramedullary nailing on the characteristics of the pulmonary neutrophil pool in rodents.

PURPOSE: Dysregulation of polymorphonuclear neutrophil (PMN) biology is associated with the development of inflammatory complications after trauma, such as acute respiratory distress syndrome (ARDS). It has been demonstrated that intramedullary nailing is both associated with altered pulmonary neutrophil deposition and the occurrence of ARDS. This standardized study aimed to characterize the long-term remote neutrophil response in the lungs in case of a femur fracture and intramedullary nailing. METHODS: A standardized rat model including intramedullary nailing and a femur fracture was utiliz

Modified Transosseous Wiring Technique for Neglected Fracture-Dislocation of the Proximal Interphalangeal Joint.

BACKGROUND: Fracture-dislocation of the proximal interphalangeal (PIP) joint of the finger is challenging due to the high risk of stiffness. The purpose of this study is to evaluate the clinical and radiological results of a modified transosseous wiring technique for the management of chronic fracture-dislocations of the PIP joint. METHODS: Ten patients (nine men and one woman; mean age, 38.3 years; range, 21 to 69 years) with neglected fracture-dislocation of the PIP joint were included. The mean duration from injury to operation was 14.7 weeks (range, 3 to 66 weeks). The dorsolateral approa

The Incidence of Pin Tract Infections and Septic Arthritis in Percutaneous Distal Femur Pinning.

BACKGROUND: Retrograde percutaneous pinning often involves intra-articular pin placement. Classic teaching has cautioned about the risk of septic arthritis with intra-articular pins, although an incidence has not been reported for this complication. The purpose of this study was to determine the incidence of pin tract infections and septic arthritis following retrograde percutaneous pinning of the distal femur. METHODS: A retrospective review identified patients who underwent retrograde percutaneous pinning of the distal femur for osteotomy or physeal fracture fixation at a tertiary pediatric

Preliminary results: use of multi-hole injection nails for intramedullary nailing with simultaneous bone cement injection in long-bone metastasis.

OBJECTIVE: For symptomatic metastasis of the long bones, intramedullary nailing has been the most accepted fixation method. Intramedullary nailing has effective control of pain, perioperative bleeding, and local tumor progression by augmentation with bone cement around the nail. Here, we report the preliminary results of a new surgical implant that allows for simultaneous injection of bone cement while inserting a percutaneous, flexible intramedullary nail. MATERIALS AND METHODS: We performed palliative surgeries for long-bone metastasis using a multi-hole injection nail (MIN) with multiple s

Femoral fracture repair and sciatic and femoral nerve blocks in a guinea pig.

A four-month-old, entire male guinea pig was presented for surgical repair of a closed oblique femoral fracture. Analgesia was provided with 30 µg/kg buprenorphine intramuscularly (im) four times a day and 0 · 3 mg/kg meloxicam subcutaneously once a day. The following day, anaesthesia was induced and maintained with 100 µg/kg medetomidine im, 20 mg/kg ketamine im, 30 µg/kg buprenorphine im and isoflurane in oxygen. Femoral and sciatic nerve blocks were performed with bupivacaine. The fracture was reduced and aligned using an intramedullary 1 · 4 mm K-wire and a 3 metric polydioxanone cerclage.

Short, locked humeral nailing via Neviaser portal: an anatomic study.

OBJECTIVE: Supraspinatus tendon trauma may contribute to residual shoulder pain after nail fixation for proximal humeral fractures. Some have proposed a more medial starting point for humeral nail insertion to avoid cuff tendon footprint damage. We hypothesized that percutaneous nail insertion via Neviaser portal would not only be possible, but would avoid tendon trauma, while sacrificing articular cartilage. MATERIALS AND METHODS: Under c-arm guidance and in percutaneous fashion, we nailed 16 consecutive complete (head and neck, etc, intact) specimen right proximal humeri with locked short h

Distal targeting device for long Gamma nail(®). Monocentric observational study.

INTRODUCTION: Intramedullary nail distal locking screws make it possible to control length and rotation but include an increased risk of radiation exposure. A distal targeting device was recently developed for long Gamma(®) nails (Stryker(®)). The aim of this practical observational study was to evaluate the reliability of this system. Our hypothesis was that the targeting device would be systematically used without conversion or complications. MATERIALS AND METHODS: All of the long Gamma(®) nails implanted between November 2011 and October 2012 were recorded: 91 nails (59W/32M, mean age 73.5

[Complications after intramedullary nailing of the tibia].

As demonstrated by the experience, treatment of fractures of the tibial intramedullary nail proved to be the most advantageous method of operating as: put the rod runs in the biomechanical axis of the limb, is inside of the bone, is a minimally invasive procedure (carried out with small cuts). Initially, the method of intramedullary nailing was used for the treatment of fractures of the shaft center of the tibia. However, the development of surgical techniques has meant that the method can also be used to treat fractures of the proximal and distal tibia. The most common complications of surgic

Surgical techniques: how I do it? The Reamer/Irrigator/Aspirator (RIA) system.

Although reamed intramedullary nailing has long been considered a safe procedure, pulmonary complications have been reported in some groups of patients. Concerns over fat embolisation, adult respiratory distress syndrome (ARDS), and sudden intraoperative death have prompted some authors to question whether the benefits of reaming are outweighed by its potential adverse effects to the patient. In response to these complications, recent evaluations have focused on alternative systems that reduce intramedullary pressure during reaming. [Ender nails in the stabilization of trochanteric fractures of the femur in the elderly].

Results of treatment for trochanteric fracture of the femur in 50 patients aged 25-91 (mean 71 years) are presented. In all cases reviewed the fracture has been stabilized with 3 or 4 Ender nails. In majority of cases bony union has been achieved. Short operating time, small incision remote from fracture site, early weight bearing constitute unquestionable advantages of the method. Most frequent complications as shortening of the extremity involved, excessive external rotation, pain related to migration of the nails, varus deformity at the fracture site were usually found in patients with mark

[Unreamed intramedullary nailing as minimal invasive palliative intervention in osteolysis and pathologic fractures of long tubular bones].

Pathological fractures and osteolyses with impending fractures of long bones impose a severe problem in patients with advanced cancer. For palliative treatment between June 1993 and October 1995 intramedullary nailing was performed in 19 patients with 13 femoral, 7 humeral and 1 tibial lesions; AO unreamed nails were used for this purpose (unreamed femur nail UFN with or with spiral blade for femoral lesions and unreamed tibia nail UTN for humeri and tibia). Stabilization was achieved without severe complications; full weight bearing of both femora and humeri (while using crutches) was allowed

Zickel supracondylar nails for fractures of the distal end of the femur.

Between 1974 and 1984 the Zickel supracondylar nail system was used to fix 82 femora. Sixty-nine femora had supracondylar fractures and 13 supracondylar osteotomies. A high rate of union (98%) was achieved in the fracture group, which included a variety of fracture types, but five supracondylar osteotomies failed to unite initially and required additional surgery. Knee motion exceeded 90 degrees in 70% of the femora that healed but was poorest in the T or Y intracondylar fracture group, in which only 46% exceeded 90 degrees. The operation may be performed as either open or closed nailing. Clos

[The posttraumatic osteomyelitis in guinea-pigs (author's transl)].

A model of posttraumatic osteomyelitis was developed in 120 Duncan-Hartley guinea-pigs. For this purpose the right femura of all animals were fractured surgically. In 60 guinea-pigs immobilisation of the fractured extremity was omitted, in remaining 60 the medullary space was splinted with intramedullary pin. Each of the two groups was further subdivided in three subgroups, i.s. non infected controls, staphylococcus aureus-infections and escherichia coli-infections. Comparative evaluations were based on ten follow-up studies, nine of which were scheduled for the first three postoperative month
