Sudden unexpected death in infants under 3 months of age and vaccination status- -a case-control study.

AIMS: To determine whether DTPP+Hib vaccination (diphtheria, tetanus, pertussis, poliomyelitis +/- haemophilus) increased the risk of sudden unexpected death (SUD) in children under 3 months of age. METHODS: We conducted a multicentre case-control study in the 28 French 'SIDS Centers'. Case selection was based on death labelled sudden infant death syndrome (SIDS) of an infant aged between 30 and 90 days. Three living controls were selected, matched for sex, gestational age and born immediately after the victim in the same maternity unit. RESULTS: We identified 114 cases of SUD aged between 3

Impact of osmolality on burning sensations during and immediately after intramuscular injection of 0.5 ml of vaccine suspensions in healthy adults.

A randomised placebo controlled double-blind cross-over trial was performed on twenty healthy adults to assess the effect of osmolality (300,600,850 and 1100 mOsm) on local tolerance of an intramuscular injection (0.5 ml) of five suspensions containing the same components as the excipients of a combined Diphtheria-Tetanus-acellular Pertussis-inactivated Poliomyelitis-Haemophilus influenzae type b paediatric vaccine (DtacP-IPV-Hib, PENTAVAC). The results did not show any dose-effect relationship between burning or pain sensations and the different osmolalities tested. Although mild and not clin

A new enzyme immunoassay for the detection of enteroviruses in faecal specimens.

A new enzyme immunoassay (EIA) for direct detection of enteroviruses based on a group-specific monoclonal antibody was evaluated using stool samples from patients with suspected enteroviral infection. The EIA was compared with polymerase chain reaction (PCR) and virus isolation in cell culture. Of 204 samples tested, 20 were positive by EIA, 34 by PCR, and 18 by cell culture. Compared with PCR, the most sensitive method, the sensitivity of EIA was 58% (20/34); the sensitivity of cell culture isolation was 52% (18/34). The results of both assays correlated in only 60% of cases. The combination

Delays in the primary vaccination of children.

The results of a population-based survey of 170 children's vaccination records were used to calculate the cumulative distributions of the ages (in months) at which each dose of vaccine had been received. Considerable delays in the administration of measles-mumps-rubella (MMR) vaccine and of the fourth dose of diphtheria-pertussis-tetanus vaccine were observed, particularly in children vaccinated by private physicians rather than at public health clinics. The delay before MMR vaccination causes concern because of the frequency of measles in children aged 1 to 2 years, particularly those attendi

Infant vaccination timing: Beyond traditional coverage metrics for maximizing impact of vaccine programs, an example from southern Nepal.

BACKGROUND: Immunization programs currently measure coverage by assessing the proportion of children 12-24 months who have been immunized but this does not address the important question of when the scheduled vaccines were administered. Data capturing the timing of vaccination in first 6 months, when severe disease is most likely to occur, are limited. OBJECTIVE: To estimate the time to Bacillus Calmette-Guérin (BCG) (recommended at birth), diphtheria-tetanus-pertussis-H, influenza b-hepatitis B (DTP-Hib-HepB), and oral polio vaccine (OPV) (recommended at 6, 10, and 14 weeks) vaccinations and

Impact of supplemental immunisation activity (SIA) campaigns on health systems: findings from South Africa.

BACKGROUND: Supplemental immunisation activity (SIA) campaigns provide children with an additional dose of measles vaccine and deliver other child health interventions including vitamin A supplements, deworming medications and oral polio vaccines. They also require the mobilisation of a large health workforce. We assess the impact of the implementation of SIA campaigns on selected routine child and maternal health services in South Africa (SA). METHODS: We use district-level monthly headcount data for 52 South African districts for the period 2001-2010, sourced from the District Health Inform

A randomized, controlled trial to assess the immunogenicity and safety of a heptavalent diphtheria, tetanus, pertussis, hepatitis B, poliomyelitis, hib and meningococcal serogroup C combination vaccine administered at 2, 3, 4 and 12-18 months of age.

BACKGROUND: Combination vaccines offer protection against multiple diseases with fewer injections. This study evaluated the immunogenicity and safety of an investigational diphtheria, tetanus, acellular pertussis, hepatitis B, poliomyelitis, Haemophilus influenzae type b (Hib) and meningococcal serogroup C (MenC) heptavalent combination vaccine (heptavalent vaccine) given as 4 doses at 2, 3, 4 and 12-18 months of age. METHODS: In this randomized, open, phase II study (NCT00970307/NCT01171989) conducted in Poland, 421 infants were enrolled to receive the heptavalent vaccine or licensed compara

[Cardiorespiratory events after first immunization in premature infants: a prospective cohort study].

OBJECTIVE: To investigate whether all preterm infants born before 33 weeks of gestation need cardiorespiratory monitoring due to the risk of cardiorespiratory disturbances following their first vaccination at 2 months of age. DESIGN: A prospective observational cohort study. METHOD: During a period lasting a little over a year, all preterm infants who were being cared for at the neonatal ward of Medical Centre Alkmaar because they had been born before 33 weeks of gestation received their first immunization at the age of 2 months and were subsequently monitored. Infants who had already been d

Effect of infant immunisation on childhood mortality in rural Bangladesh: analysis of health and demographic surveillance data.

BACKGROUND: In developing countries, immunisation programmes must compete with other strategies to improve public health and quality of life. Studies of long-term effects of immunisation programmes are rare. We assessed associations between vaccinations and mortality over 15 years after the introduction of routine infant immunisation programmes in Matlab, Bangladesh. METHODS: We analysed data recorded in a comprehensive health and demographic surveillance system from 1986 to 2001. We did univariate analyses and assessed vaccinations as independent factors with other variables in Cox models wi

Serum immunoglobulin G subclasses in healthy infants and children in Greece.

Serum concentrations of immunoglobulin IgG1, IgG2, IgG3 and IgG4 were determined by radial immunodiffusion in a sample of 414 healthy Greek children, who were admitted to the major Teaching Hospital for Children in Athens for minor surgical operations. Statistical analysis was performed by multiple regression after logarithmic transformation of the immunoglobulin values. There was a statistically significant increase of IgG3 with age, whereas IgG1, IgG2 and IgG4 levels reached a turning point at the age of five years. Older than 5 years male children were found to have marginally higher IgG4 l
