High frequency of brachiocephalic trunk stent fractures does not impair clinical outcome.

OBJECTIVE: Stenting is the preferred, minimally invasive treatment for innominate artery (IA) stenosis or occlusion. Stent fractures in the IA have not been assessed in larger cohorts. In this retrospective study, we examined the frequency and risk factors of IA stent fractures. METHODS: The final analysis included 32 patients (15 women; mean age, 59.4 ± 12.0 years) with 32 balloon-expandable stents (2000 to 2009). In 2010, the patients were asked to come back for a fluoroscopic examination of the implanted stents. Stent fractures and their relationship to atherosclerotic risk factors, lesion

Evaluation of postmortem MDCT and MDCT-angiography for the investigation of sudden cardiac death related to atherosclerotic coronary artery disease.

The goal of this study was to evaluate the diagnostic value of postmortem multi-computed tomography (MDCT) and MDCT-angiography for sudden cardiac deaths related to ischemic heart disease. Twenty three cases were selected based on clinical history and the results of native MDCT, multiphase post-mortem CT-angiography and conventional autopsy were compared. Radiological examination showed calcification of coronary arteries in 78% of the cases, most of which were not detailed at autopsy. MDCT-angiography allowed better visualization of the coronary arteries than MDCT and permitted the evaluation

May dual-source computed tomography angiography replace invasive coronary angiography in the evaluation of patients referred for valvular disease surgery?

BACKGROUND: Coronary computed tomography (CT) angiography is currently the only alternative to invasive angiography in the evaluation of coronary anatomy. In patients referred for valvular or thoracic aortic disease surgery, invasive coronary angiography remains the gold standard required by cardiac surgeons during the preoperative evaluation. According to the current European Society of Cardiology guidelines, evaluation of coronary anatomy is recommended in all patients above 40 years of age, with a history of coronary artery disease (CAD), in postmenopausal women, patients with left ventricu

Cellular and Molecular Profiling of Native Heart Valves in Infective Endocarditis: A Comparative Study with Calcific Aortic Valve Disease.

Background: Infective endocarditis (IE) affects both native and prosthetic heart valves, the endocardial surface, as well as cardiac implantable electronic devices. Identifying specific IE biomarkers for its early risk stratification remains crucial, particularly in cases with blood culture-negative endocarditis. Methods: Eleven native heart valves obtained from IE and calcific aortic valve disease (CAVD) patients were analyzed. Immunohistochemical analysis of a pan-leukocyte marker (CD45), macrophage marker (CD68), T-lymphocyte marker (CD3), B-lymphocyte marker (CD19), neutrophil myeloperoxid

Opportunistic CT Screening: Clinical Applications, Technical Foundations, and Best Practices.

Opportunistic CT screening refers to the extraction of clinically relevant imaging biomarkers from CT examinations routinely performed for unrelated primary indications. This screening does not require additional radiation exposure, imaging time, or patient burden. Advances in automated image analysis and artificial intelligence (AI) have substantially expanded the feasibility of opportunistic CT screening across a range of conditions, including low bone mineral density, hepatic steatosis, sarcopenia, adiposity, and vascular calcifications. These biomarkers are increasingly being recognized as…

Purinergic signaling modulates endothelial-to-mesenchymal transition in human aortic valve endothelial cells.

Endothelial-to-mesenchymal transition (EndMT) is a phenotypic switch in which endothelial cells acquire mesenchymal characteristics, involving both functional and morphological changes. While EndMT is essential for cardiac development, its aberrant activation contributes to adult cardiovascular pathologies, including calcific aortic valve disease (CAVD). Dysregulation of ectonucleotidases-membrane-bound enzymes that regulate extracellular ATP and adenosine metabolism-has been implicated in such diseases. Altered extracellular nucleotide signaling influences valvular interstitial cell (VIC) deg

Report of a case of renal hilum Castleman disease complicated with Paget's disease of the breast.

Renal hilar Castleman disease is a rare lymphoproliferative disorder characterized by non-specific radiological manifestations. It is easily misdiagnosed as primary renal or adrenal malignant tumors in clinical practice, which may consequently lead to unnecessary radical nephrectomy. A 51-year-old female with a history of right breast cancer treated with mastectomy and adjuvant endocrine therapy 7 years prior presented with right nipple erosion. Biopsy confirmed Paget's disease, and preoperative staging with contrast-enhanced computed tomography (CT) incidentally revealed a solitary soft-tissu

Sex-dependent divergence between skeletal and cardiac responses to phosphate load in experimental CKD.

Chronic kidney disease (CKD) induces a progressive decline in kidney function, leading to increased inorganic phosphate (Pi) load resulting in higher fibroblast growth factor 23 (FGF23) and cardiovascular complications including vascular calcifications (VC). Parallel rise of parathyroid hormone (PTH) increases bone resorption and release of calcium and Pi. These mineral and bone disorders (MBD) are central to the abnormalities observed in CKD. CKD has higher prevalence in women but higher cardiovascular impact on men, with increased risk of progressing to late stages. Here we characterize CKD-…

Standard and advanced echocardiographic study of patients with Paget's disease of bone: Evidence of a pagetic heart disease?

BACKGROUND: Paget's disease of the bone (PDB) is a metabolic bone disorder involving one or more skeletal sites. Cardiovascular diseases (CVDs) have been described in patients with PDB but have not been systematically analysed. OBJECTIVES: This study aimed to compare standard and advanced (speckle-tracking) echocardiographic parameters measured in patients with PDB and controls matched for age, weight, height and history of hypertension but without metabolic bone disorders. METHODS: This multicentre case-control study included all patients with PDB referred to the Federico II and Siena Unive

Cardiac Myxoma With Gamna-Gandy Bodies in a Nicaraguan Patient: Leveraging Advanced Imaging to Reduce Mortality Risk.

Myxomas are the most frequent primary cardiac neoplasms, presenting with diverse clinical manifestations depending on their size and location due to obstructive effects on blood flow. In patients with pre-existing structural heart disease, their nonspecific symptoms can closely mimic acute heart failure, valvular disease, or even pulmonary thromboembolism, leading to significant diagnostic challenges. We describe the case of a 59-year-old Hispanic male patient with hypertensive heart disease with preserved ejection fraction and paroxysmal atrial fibrillation, poorly adherent to treatment. He i

Secondary parkinsonism associated with focal brain lesions.

Focal imaging abnormalities in patients with parkinsonism suggest secondary etiology and require a distinctive clinical approach to diagnosis and treatment. We review different entities presenting as secondary parkinsonism associated with structural brain lesions, with emphasis on the clinical course and neuroimaging findings. Secondary parkinsonism may be due to vascular causes, hydrocephalus, space-occupying lesions, metabolic causes (including acquired hepatocerebral degeneration, diabetic uremic encephalopathy, basal ganglia calcifications, osmotic demyelination syndrome), hypoxic-ischaemi

Prevalence of lipoprotein(a) measurement in patients with or at risk of cardiovascular disease.

INTRODUCTION: Lipoprotein(a) [Lp(a)] is a genetically determined independent risk factor for atherosclerotic cardiovascular disease (ASCVD) and calcific aortic valve disease. Despite recommendations from professional societies in the cardiovascular field, the awareness of elevated Lp(a) as a risk factor and screening for Lp(a) are suspected to be low. METHODS: We conducted a retrospective, observational case control study of patient charts from January 1, 2017 to June 19, 2022. The primary aims were 1) to describe the proportion of patients at the healthcare network's primary care and cardiol

RailTracking: A Novel Technique to Overcome Difficult Anatomy During Transradial Approach.

OBJECTIVE: We aim to investigate the safety and efficacy of a new technique, "RailTracking," in the management of challenging transradial routes during percutaneous coronary interventions (PCI). BACKGROUND: The transradial access (TRA) currently represents the access site of choice in PCI, but complex anatomy could lead to complications and access-site crossover. The assisted-tracking techniques described in the past (such as balloon-assisted tracking and pigtail-assisted tracking) are based on the concept of a "guiding tapered tip" to improve trackability. The RailTracking technique creates
