Subcutaneous emphysema to the head and neck resulting from a dental crown preparation.

CASE REPORT: A 59-year-old African-American female presents with subcutaneous emphysema to the head and neck region resulting from routine dental treatment with a high speed air-driven handpiece. The patient had a chief complaint of generalized edema, dysphagia and pain to her face and neck. The patient subsequently was admitted to the intensive care unit for airway monitoring. PURPOSE: To alert the dental community about the incidence of iatrogenic subcutaneous emphysema from a routine dental procedure, and how to recognize and manage its occurrence.

Seizure disorders. Update on medical management for the dental clinician. A pacemaker for the brain.

There have been many papers reviewing the dental office assessment and management of the patient with a seizure disorder. This paper will discuss two updated medical management issues which may impact dental office care of the seizure patient. The focus will on the use of a pacemaker-like device connected to the vagus nerve as part of seizure control. Also presented is a Food and Drug Administration (FDA) update on the increasingly recognized medical side effects of antiseizure (antiepileptic) medications which may impact a dentist's interaction with a patient.

Trigeminal neuralgia and radiofrequency.

Trigeminal neuralgia (TN) is a painful neurological disorder often mistaken for pain of dental origin by the patient and dentist. Dentists should be acquainted with TN to differentiate it from orofacial pain and prevent unnecessary tooth extraction. TN pain alleviating modalities are numerous, yet not uniformly effective. Radiofrequency, known for 25 years, is a minimally-invasive outpatient procedure used for TN when drugs are ineffective. The authors evaluate radiofrequency in trigeminal neuralgia patients referred from the dental office.

The airway, breathing and orthodontics.

Dentists need to play a bigger role in managing airway development and craniofacial formation even though the relationship between the airway, breathing and malocclusion remains quite controversial. Certainly the airway, the mode of breathing and craniofacial formation are so interrelated during growth and development that form can follow function and function can follow form. So, it is imperative to normalize form and function as early as possible so that function is optimized for life.

Taste change associated with a dental procedure: case report and review of literature.

Loss or alteration of taste is a rare phenomenon that may be idiopathic or may be caused by head trauma, medication use or systemic and local factors including various invasive dental procedures resulting in nerve damage. We present an unusual case of generalized taste change following an oral surgical procedure. The case is presented to enhance understanding of taste disorders and their relation to a localized traumatic event. Causative factors and management strategies are also reviewed. Taste change, encompassing loss (ageusia) or alteration (dysgeusia) of taste, is a rare phenomenon that m
