INTRODUCTION: Axillo-subclavian arterial injuries are generally associated with penetrating trauma. On rare occasion, blunt mechanisms can cause these injuries in the setting of high-energy trauma, humeral head or neck fractures, and severe glenohumeral dislocations. Glenohumeral dislocations sustained from ground-level falls are generally reduced in the emergency department without associated morbidity.
PRESENTATION OF CASE: An 80-year-old woman presented with an inferior glenohumeral dislocation after a ground-level fall that was found to be complicated by axillary dissection, pseudoaneurys

BACKGROUND: Combined vascular and skeletal injuries are associated with a high limb loss rate. One of the major factors resulting in amputation is frequently because the allowable warm ischemia time for skeletal muscle is exceeded before adequate revascularization.
METHODS: Temporary vascular shunting has been used in selected patients with complete ischemia to minimize the ischemic time of the injured limb, allowing identification of vital structures, thorough debridement, and rigid internal fixation before definitive vascular repair.
RESULTS: Five male and two female patients with a median

Ulnar nerve dysfunction is a well-recognized phenomenon following distal humerus fractures. Its fixed anatomic position predisposes the nerve to injury. Injury can occur at the time of injury, during closed-fracture manipulation, intraoperatively during fracture fixation (when it is routinely identified), or during fracture healing. Intraoperative management varies widely and can include in situ decompression or anterior transposition. This article reviews the literature and presents 24 patient cases. A 38% incidence of late ulnar neuropathy following open reduction and internal fixation is id

An osteocartilaginous exostosis developed in a 4-year-old boy in whom a distally-based lateral supramalleolar adipofascial flap had been used to cover a defect in the dorsum of the foot. The bony exostosis was first noticed four months after the operation. It was excised with no complications and there has been no recurrence after two years follow-up. The exostosis was thought to result from stripping the perichondrium over the epiphyseal plate while the flap was being raised. This unique complication has not to our knowledge been reported after harvest of a flap before.

INTRODUCTION: Distal humerus fractures are rare and challenging to treat. Anatomic reduction of the articular surface and stable osteosynthesis are mandatory for satisfactory results. The transolecranon approach allows superior visualisation of the joint. However, controversy exists regarding how best to fix the osteotomy. The purpose of the present study was to investigate the eligibility of a one-third tubular hook plate for osteosynthesis of olecranon osteotomies in distal humerus type-C fractures.
PATIENTS AND METHODS: A consecutive series of 34 patients who were treated through an olecra

BACKGROUND: Treatment of cubitus varus deformity from a malunited fracture is a challenge. Anatomically accurate correction is the key to obtaining good functional outcomes after corrective osteotomy. The aim of this study was to attempt to increase the accuracy of treatment by use of 3-dimensional (3D) computer-aided design. We describe a novel method for ensuring an accurate osteotomy method in the treatment of cubitus varus deformity in teenagers by means of 3D reconstruction and reverse engineering.
MATERIALS AND METHODS: Between January 2006 and May 2008, 12 male and 6 female patients wi

INTRODUCTION: The elbow is the second most common joint involved in dislocations in the human body, accounting for approximately 20% of all joint dislocations. The precariousness of some health services, in rural and less developed places, contributes to a higher rate of underdiagnosis of these injuries, resulting in a higher prevalence of persistent elbow dislocations. In the surgical management of chronic elbow dislocations, primary ligament repair is frequently not feasible, making reconstruction of the lateral and medial collateral ligament complexes of the elbow usually necessary, often w

BACKGROUND: Concerning surgical management experience with locking plates for proximal humeral fractures has been described with promising results. Though, distinct hardware related complaints after fracture union are reported. Information concerning the outcome after removal of hardware from the proximal humerus is lacking and most studies on hardware removal are focused on the lower extremity. Therefore the aim of this study was to analyze the functional short-term outcome following removal of locking plate fixation of the proximal humerus.
METHODS: Patients undergoing removal of a locking …

OBJECTIVE: Because difficult vaginal delivery is more frequent with macrosomic fetuses, some authors recommend routine caesarean section for the delivery of fetuses >4500 g. The purpose of this study was to evaluate the appropriateness of this recommendation, in particular, to analyze maternal and fetal complications according to the mode of delivery.
METHOD: Maternal and neonatal records of 100 infants with weights of at least 4500 g were identified retrospectively from January 1991 to December 1996. Outcome variables included the mode of delivery and the incidence of maternal and perinatal

A posterior approach to the elbow utilizing an olecranon osteotomy has been shown to provide excellent visualization of the distal humerus articular surface. However, many bony stabilization and fixation methods for the olecranon osteotomy are usually prominent, frequently symptomatic, and often require a second operation for removal. This paper evaluates the use of an innovative device, the olecranon sled, in fixation of olecranon osteotomies for exposure of intra-articular distal humerus fractures and provides follow-up results. A retrospective review of all patients with intra-articular dis

Since war injuries are not predictable, each patient presents as a new and unique case. Before deciding on amputation for upper extremity injuries, it is necessary to evaluate all limb-sparing vascular interventions that will keep the amputation level as distal as possible, in order to improve the patient's quality of life and enable the use of more functional prostheses. While amputation may be a faster and more life-saving option for a surgeon, limb-sparing and mobile joint-preserving surgical procedures should be the preferred choice, provided the patient's overall condition permits. This c

PURPOSE: Cubitus varus is a common triplane deformity in adults associated with supracondylar humeral fractures experienced as a child and consists of varus, extension, and internal rotation components. When corrective osteotomy is indicated, these three components should be measured precisely. This study aimed to evaluate the accuracy of radiographic and physical measurements of cubitus varus deformities in adults compared to values measured on three-dimensional (3-D) bone surface models of the adult bilateral humerus.
METHODS: Three-dimensional bilateral humerus models were developed using …

BACKGROUND: In older patients requiring a total shoulder replacement (TSR) and with an intact rotator cuff, there is currently uncertainty on whether an anatomic TSR (aTSR) or a reverse TSR (rTSR) is best for the patient. This comparison study of same-aged patients aims to assess clinical and radiological outcomes of older patients (≥75 years) who received either an aTSR or an rTSR.
METHODS: Consecutive patients with a minimum age of 75 years who received an aTSR (n = 44) or rTSR (n = 51) were prospectively studied. Pre- and post-operative clinical evaluations included the ASES score, Constan
