BACKGROUND: Neuroinflammation, measured using [11C](R)-PK11195 positron emission tomography (PET), has been reported in isolated rapid-eye-movement sleep behavior disorder (iRBD), but its temporal progression is unknown.
OBJECTIVE: The aim was to assess longitudinal progression of neuroinflammation in iRBD patients and its relationship with phenoconversion into Parkinson's disease (PD) or dementia with Lewy bodies (DLB).
METHODS: Sixteen iRBD patients received longitudinal [11C](R)-PK11195 PET scans over 3 years and were followed up clinically for 8 years to record phenoconversion into PD an

INTRODUCTION: Neutrophil and lymphocyte levels and their ratio, the neutrophil-lymphocyte ratio (NLR), in the peripheral blood provide accessible indices of systemic inflammatory activity. In this study, we evaluated whether these blood markers are linked to motor symptoms and cognitive performance during the prodromal stage of Parkinson's disease (PD).
METHODS: We analyzed data from the prodromal cohort of the Parkinson's Progression Markers Initiative, which included 1,269 individuals with prodromal PD features (isolated REM sleep behavior disorder [RBD]: n = 116, isolated hyposmia: n = 570

Rapid eye movement sleep behavior disorder (RBD) is a common condition found in more than 50% of the patients with Parkinson's disease (PD). Molecular imaging shows that PD with RBD (PD-RBD+) have lower striatal dopamine transporter activity within the caudate and putamen relative to PD without RBD (PD-RBD-). However, the characterization of the extra-striatal dopamine within the mesocortical and mesolimbic pathways remains unknown. We aim to elucidate this with PET imaging in 15 PD-RBD+ and 15 PD-RBD- patients, while having 15 age-matched healthy controls (HC). Each participant underwent a si

IMPORTANCE: There is increasing evidence that Parkinson disease (PD) is heterogeneous in its clinical presentation and prognosis. Defining subtypes of PD is needed to better understand underlying mechanisms, predict disease course, and eventually design more efficient personalized management strategies.
OBJECTIVES: To identify clinical subtypes of PD, compare the prognosis and progression rate between PD phenotypes, and compare the ability to predict prognosis in our subtypes and those from previously published clustering solutions.
DESIGN, SETTING, AND PARTICIPANTS: Prospective cohort study

This paper reviews current evidence in support of dysthymia as a sub-affective disorder that precedes major affective episodes, often by more than a decade. In cases beginning in childhood or adolescence, dysthymia is associated with high familial rates of mood disorders, and a recurrent pattern of superimposed major depression. At least two trait-like markers, sleep electro-encephalographic and thyroid axis abnormalities-similar to those in major affective disorder-have been reported. These data indicate a common pathophysiological substrate for both dysthymia and major depressive illness. Al

Background: Gaucher disease (GD) is a rare lysosomal storage disorder caused by mutations in the GBA1 gene. Traditionally, GD is classified into three subtypes based on the severity of neurological involvement; however, overlapping clinical features increasingly suggest a continuum of phenotypes rather than distinct categories. In this prospective observational cohort study, we conducted a multidisciplinary assessment of patients with GD to identify and monitor neurological, cognitive, auditory, and visual impairments. BACKGROUND: Established assessment scales used for Parkinson's disease (PD) have several limitations in tracking symptom progression and fluctuation. Both research and commercial-grade wearables show potential in improving these assessments. However, it is not known whether pervasive and affordable devices can deliver reliable data, suitable for designing open-source unobtrusive around-the-clock assessments. Our aim is to investigate the usefulness of the research-grade wristband Empatica E4, commercial-grade smartwatch Fitbit Sense, and the Oura ring, for PD research.
METHOD: The study inclu

OBJECTIVE: Despite the relatively high prevalence of Lewy body disease (LBD) in over middle-aged people, physicians and patients show poor awareness of early symptoms. We examined the clinical utility of a simple checklist for screening for the early symptoms of LBD.
METHODS: We administered a 7-item screening checklist assessing the five representative early symptoms of LBD (dizziness, constipation, anosmia, REM sleep behavior disorder, and depression: the acronym is "DCARD") to 450 consecutive individuals who visited the memory clinic at our university hospital. The aim of this retrospectiv

OBJECTIVES: To investigate the prevalence and risk factors for probable rapid eye movement (REM) sleep behavior disorder (pRBD) in Chinese patients with essential tremor (ET).
METHODS: In this cross-sectional study, 391 ET patients underwent assessments of sociodemographics, lifestyles, environmental exposures, medical histories, and motor/nonmotor symptoms. pRBD status was evaluated using the RBD screening questionnaire (RBDSQ). Risk factors for pRBD in ET patients were identified via multivariable logistic regression.
RESULTS: Fifty-six patients (14.3 %) met the criteria for pRBD, with a m

BACKGROUND: α-Synuclein seed amplification assay on cerebrospinal fluid (CSF-αSyn-SAA) has shown high accuracy for Parkinson's disease (PD) diagnosis. The analysis of CSF-αSyn-SAA parameters may provide useful insight to dissect the heterogeneity of synucleinopathies.
OBJECTIVE: To assess differences in CSF-αSyn-SAA amplification parameters in participants with PD stratified by rapid eye movement (REM) sleep behavior disorder (RBD), dysautonomia, GBA, and LRRK2 variants.
METHODS: Clinical and CSF-αSyn-SAA data from the Parkinson's Progression Marker Initiative dataset were used. CSF-αSyn-SAA

OBJECTIVE: Rapid eye movement sleep behavior disorder (RBD) is often underdiagnosed among people living with mental disorders. The present study aimed to investigate the prevalence of probable RBD (pRBD) and its associated factors among middle-aged and older adults in a psychiatric outpatient clinic.
METHODS: We conducted a cross-sectional survey among 2907 people aged 45-80 years who visited the outpatient clinic between March 1 and August 31, 2022 in a psychiatric hospital. A cutoff score ≥5 on the RBD Screening Questionnaire (RBDSQ) was used to indicate the presence of probable RBD (pRBD).

BACKGROUND: Isolated rapid eye movement sleep behavior disorder (iRBD) serves as a prodromal phase of Parkinson's disease (PD) and dementia with Lewy bodies (DLB). Blunted tachycardia (BT) during postural changes indicates neurogenic orthostatic hypotension, a marker of autonomic dysfunction. We aimed to investigate whether BT is associated with cardiac sympathetic neurogenic denervation. Additionally, we conducted a preliminary short-term follow-up to examine the potential prognostic significance of BT regarding phenoconversion and mortality.
METHODS: Forty-three patients with iRBD at Shiga

BACKGROUND: The histological feature of Parkinson's disease is the presence of intraneuronal aggregates of phosphorylated α-synuclein (αSyn). In patients with Parkinson's disease, deposits of αSyn are found in the autonomic nerve fibres of the submandibular gland. Since patients with idiopathic rapid-eye-movement sleep behaviour disorder (IRBD) can develop Parkinson's disease and other synucleinopathies, we investigated whether αSyn deposits could also be detected in their submandibular gland nerve fibres.
METHODS: We did a case-control study at the Hospital Clinic de Barcelona (Barcelona, Sp

OBJECTIVE: To provide a 16-year update from the authors' 1996 report documenting a 38% conversion from idiopathic rapid eye movement sleep behavior disorder (iRBD) to a parkinsonian disorder at a mean interval of nearly 13 years after the onset of iRBD in a series of 29 males > or =50 years old.
METHODS: The methods of evaluation, diagnosis and follow-up were previously described in the 1996 report. All patients had video-polysomnography (vPSG) confirmed RBD.
RESULTS: 80.8% (21/26) of patients who were initially diagnosed with iRBD eventually developed parkinsonism/dementia (three of the ori

Recently, low levels of a newly identified neuropeptide, hypocretin 1, were described in the cerebrospinal fluid of patients with narcolepsy. This neurochemical finding furthers our understanding of this enigmatic sleep disorder typically characterized by excessive daytime sleepiness, cataplexy, sleep paralysis, and hypnagogic hallucinations. Narcolepsy appears to be fundamentally related to abnormally regulated rapid eye movement sleep. The diagnosis of this disorder remains challenging because of multiple other conditions that can cause daytime sleepiness and the difficulties in recognizing

BACKGROUND: Infections classified as group 1 biological carcinogens include the helminthiases caused by Schistosoma haematobium and Opisthorchis viverrini. The molecular mediators underlying the infection with these parasites and cancer remain unclear. Although carcinogenesis is a multistep process, we have postulated that these parasites release metabolites including oxysterols and estrogen-like metabolites that interact with host cell DNA. How and why the parasite produce/excrete these metabolites remain unclear. A gene encoding a CYP enzyme was identified in schistosomes and opisthorchiids.

Fetal behavior was studied in 17 cases of fetal abnormalities including nonimmune hydrops fetalis and in 11 cases of intrauterine growth retardation (IUGR) in comparison to 30 healthy fetuses. Breathing movements, rapid eye movements and trunk movements were observed ultrasonically and analyzed using a computer-assisted system. The cases with central nervous system anomalies showed a markedly different behavioral pattern. In the fetal abnormalities without chromosomal anomalies, the cases without major behavioral disorders were cured by neonatal treatment. IUGR cases with decreased trunk, brea
