The implications of revised WHO classification (2008) of chronic myeloid neoplasms.

Myeloid disorders constitute a subgroup of hematological malignancies of myeloid lineage. In the revised 2008 WHO classification system, the nomenclature for myeloproliferative entities has been changed from "chronic myeloproliferative diseases" to "myeloproliferative neoplasms", the definition criteria has changed, and and the MDS/MPN has emerged as a well defined entity the subgroup formerly known as "myelodysplastic/myeloproliferative diseases" has changed to "myelodysplastic/myeloproliferative neoplasms". The World Health Organization classification of myeloid neoplasms relies on the assoc…

Modified Mallampati classification as a clinical predictor of peroral esophagogastroduodenoscopy tolerance.

BACKGROUND: Unsedated esophagogastroduodenoscopy (EGD) is simpler and safer than sedated EGD; however, approximately 40% of patients cannot tolerate it. Early identification of patients likely to poorly tolerate unsedated EGD is valuable for improving compliance. The modified Mallampati classification (MMC) has been used to evaluate difficult tracheal intubation and laryngoscope insertion. We tried to assess the efficacy of MMC to predict the tolerance of EGD in unsedated patients. METHODS: Two hundred patients who underwent an unsedated diagnostic EGD were recruited. They were stratified acc…

Reclassification of SIDS cases--a need for adjustment of the San Diego classification?

A study was undertaken reclassifying cases of sudden infant death syndrome (SIDS) taken from two geographically separate locations utilizing the San Diego definition with subclassifications. One hundred twenty-eight infant cases were examined from files at Forensic Science South Australia in Adelaide, SA, Australia over a 7.5-year period from July 1999 to January 2007. Thirty-one cases (24%) had initially been diagnosed as SIDS and 30 (23%) as undetermined while 67 (52%) had an explainable cause of death. After reclassification, the number of SIDS cases had increased to 49 of the 128 cases, no…

[A fast classification method for petroleum products based on the Raman spectroscopy].

A fast and effective method for classification of petroleum products based on Raman spectroscopy is proposed. A knowledge base composed by Raman spectra of training samples, intra-class feature spectra and intra-class thresholds of all classes was firstly established. Then, correlation coefficients between the test sample and the intra-class feature spectra were calculated. If the maximal correlation coefficient of the test sample is larger than or equal to the corresponding intra-class threshold, the test sample is determined to belong to the corresponding class. For 96 petroleum product samp…

Muscle abnormalities of the chest in Poland's syndrome: variations and proposal for a classification.

PURPOSES: Poland's syndrome (PS) is a rare congenital malformation, which combines anomalies of the chest and the homolateral upper limb. The purposes of the paper are to study the chest musculoskeletal malformations of the syndrome and propose a classification for the thoracic anomalies through our experience and taking into account the literature. METHODS: We reviewed ten patients diagnosed with PS. All of them suffered from anomalies of the thorax and the ipsilateral upper extremity. We proceeded to scan the thorax with 3-D CT for better imaging of the structures and examination of the var…

[Progress in classification and treatment for TNF receptor-associated periodic syndrome].

TNF receptor-associated periodic syndrome (TRAPS) is an autosomal dominant autoinflammatory disorder characterized by recurrent febrile attacks. TRAPS is associated with mutation in the gene encoding TNF Receptor I (TNFRI) and seven mutations have been reported in Japan. Molecular modeling experiments indicate that the mutant TNFRI accumulates intracellularly in the endoplasmic reticulum due to misfolding and activates MAP kinase (MAPK) through induction of mitochondrial reactive oxygen species production. MAPK activation is further enhanced by the stimulation through toll-like receptor, resul

Arthroscopic disease classification and interventions as an adjunct in the treatment of acetabular dysplasia.

BACKGROUND: Treatment of hip dysplasia has focused on corrective osteotomy surgery, while hip arthroscopy remains controversial. Improved understanding of intra-articular disease patterns associated with hip dysplasia will help delineate the role of arthroscopy as an adjunct to osteotomy surgery for dysplastic hips. PURPOSE: The authors set out to describe the intra-articular disease patterns of patients undergoing combined hip arthroscopy and periacetabular osteotomy for the treatment of symptomatic acetabular dysplasia with associated mechanical symptoms. Secondly, they wanted to identify t

Rising trends in cholangiocarcinoma: is the ICD classification system misleading us?

BACKGROUND & AIMS: Cholangiocarcinomas (CC) can be sub-divided into intrahepatic (IHCC) or extrahepatic (EHCC). Hilar or 'Klatskin' tumours are anatomically extrahepatic. Most international studies, also from the UK, report increasing IHCC and decreasing EHCC incidence. The second edition of the International Classification of Diseases for Oncology (ICD-O-2) assigned 'Klatskin' tumours a unique histology code (8162/3), but this was cross-referenced to the topography code for intrahepatic (IHBD) rather than extrahepatic bile duct tumours (EHBD). Under the third ICD-O edition, 'Klatskin' tumours…

Evaluation of the traditional and revised WHO classifications of Dengue disease severity.

Dengue is a major public health problem worldwide and continues to increase in incidence. Dengue virus (DENV) infection leads to a range of outcomes, including subclinical infection, undifferentiated febrile illness, Dengue Fever (DF), life-threatening syndromes with fluid loss and hypotensive shock, or other severe manifestations such as bleeding and organ failure. The long-standing World Health Organization (WHO) dengue classification and management scheme was recently revised, replacing DF, Dengue Hemorrhagic Fever (DHF), and Dengue Shock Syndrome (DSS) with Dengue without Warning Signs, De…

[TNM classification of breast cancer: changes and comments on the 7th edition].

The 7th edition of the TNM classification includes only minor changes in the main TNM categories for breast cancer. Only ductal and lobular carcinoma in situ (DCIS, LCIS), and isolated Paget's disease of the nipple are classified as pTis, but not precursor lesions such as atypical ductal or lobular hyperplasia (ADH, ALH). AJCC emphasizes that microscopic measurement is the most accurate and preferred method to determine pT in small invasive cancers and stresses the importance of strict adherence to criteria for T4 cancers. For better distinction from micrometastases in regional lymph nodes, sm…

Characterization of complex renal cysts: a critical evaluation of the Bosniak classification.

OBJECTIVE: Complex renal cysts represent a major clinical problem, since it is often difficult to exclude malignancy. The Bosniak classification system, based on computed tomography (CT), is widely used to categorize cystic renal lesions. The aim of this study was to evaluate critically available data on the Bosniak classification. MATERIAL AND METHODS: All publications from an Entrez Pubmed search were reviewed, focusing on clinical applicability and the use of imaging modalities other than CT to categorize complex renal cysts. RESULTS: Fifteen retrospective studies were found. Most series …

Developmental biology and classification of congenital anomalies of the hand and upper extremity.

Recent investigations into the mechanism of limb development have clarified the roles of several molecules, their pathways, and interactions. Characterization of the molecular pathways that orchestrate limb development has provided insight into the etiology of many limb malformations. In this review, we describe how the insights from developmental biology are related to clinically relevant anomalies and the current classification schemes used to define, categorize, and communicate patterns of upper limb malformations. We advocate an updated classification scheme for upper limb anomalies that i…
