Management of recurrent anastomotic stenosis following radical prostatectomy using holmium laser and steroid injection.

OBJECTIVE: To present our experience with the management of recurrent and resistant anastomotic stenosis following radical prostatectomy (RP) using transurethral laser incision of the stenotic area and injection of steroids. PATIENTS AND METHODS: Between January 1999 and April 2006, we evaluated 24 patients with anastomotic stenosis that would not allow the passage of the flexible cystoscope (17 F). Using the paediatric 7.5 F Olympus scope and a 550-microm fibre holmium laser, deep incisions were cut at the 3 and 9 o'clock positions at the bladder neck, and then triamcinolone was injected at

Post-operative drain output as a predictor of bladder neck contracture following radical prostatectomy.

BACKGROUND: Bladder neck contracture (BNC) following prostatectomy has been reported in 0.5-32% of cases. While the etiology of a BNC is unclear, several factors have been associated with this complication, including blood loss, devascularization of bladder neck tissue, poor mucosal apposition and urinary extravasation. To study the impact of urinary extravasation on BNC formation, we used postoperative drain output as a surrogate measure for anastomotic leakage. METHODS: All patients undergoing a radical retropubic prostatectomy (RRP) or a robotic assisted radical prostatectomy (RARP) from J

Bladder neck contracture after robot-assisted laparoscopic radical prostatectomy: evaluation of incidence and risk factors and impact on urinary function.

BACKGROUND AND PURPOSE: Bladder neck contracture (BNC) after radical prostatectomy has been reported to occur in 5% to 32% of men after open retropubic prostatectomy (RRP) and in 0% to 3% after laparoscopic RRP. Optimal anastomotic closure involves creating a watertight, tension-free anastomosis with well-vascularized, mucosal apposition and correct realignment of the urethra. The cause of BNC is poorly understood; however, it is likely related to multiple factors, including excessive luminal narrowing at the site of reconstruction, local tissue ischemia, failed mucosal apposition, and urinary

Bladder-sparing surgery in locally advanced nonurological pelvic malignancy.

INTRODUCTION: The urinary bladder is commonly involved in pelvic malignancy. The incidence of apparent extension into adjacent organs in locally advanced colorectal malignancy is 5-12%. It is not known with other pelvic malignancy. No guidelines are available for its management. Often a dilemma exists between cystectomy and a bladder-sparing procedure. We studied the validity of bladder-sparing surgery (BSS) in locally advanced nonurological pelvic malignancy. METHODS: Hospital records of patients who underwent BSS along with other surgeries (abdomino-perineal resection, anterior resection, a

Effectiveness of alpha1-adrenergic blockers in boys with low urinary flow rate and urinary incontinence.

BACKGROUND AND PURPOSE: Although alpha blockers have been shown to be effective in treating various types of neuropathic voiding dysfunction in children, the effectiveness of alpha(1) blockers in treating neurologically intact children with voiding dysfunction remains unclear. We investigated the effectiveness of treatment with alpha(1)-adrenergic blockade in boys with low uroflow rate and urinary incontinence. METHODS: The alpha(1) blocker doxazosin (0.5 to 1.0 mg daily) was administered to 16 boys (mean age, 8.9 +/- 3.4 years) with maximum uroflow rate (Qmax) < 15 mL/s and urinary incontine

Recurrence, progression and success in stage Ta grade 3 bladder tumors treated with low dose bacillus Calmette-Guerin instillations.

PURPOSE: Bacillus Calmette-Guerin (BCG) therapy is considered to be an effective prophylactic and therapeutic agent for high risk superficial transitional cell carcinoma of the bladder. Nevertheless, in a select uncommon population of stage Ta grade 3 superficial lamina-free tumors the results of this treatment have not yet been well established. We evaluated recurrence and progression rates, and the success of BCG therapy in a population with stage Ta grade 3 transitional cell carcinoma of the bladder. MATERIALS AND METHODS: Of the 605 patients treated at our institution from 1982 to 1996 fo

Analysis of real-world re-induction outcomes with nadofaragene firadenovec in patients with Bacillus Calmette-Guérin (BCG)-unresponsive non-muscle-invasive bladder cancer.

Nadofaragene firadenovec is an intravesical gene therapy approved for use in patients with Bacillus Calmette-Guérin (BCG)-unresponsive non-muscle-invasive bladder cancer who have carcinoma in situ (CIS) with/without papillary disease (Ta or T1). It is administered every 3 months for as long as the patient maintains a complete response (CR). In the phase 2 and 3 clinical trial program, patients without CR at 3 months were not re-treated with nadofaragene firadenovec; however, many immune therapies do benefit from re-induction of this kind. We report findings from an observational cohort study o

Rare Early Coexistence of Classic Urothelial and Small Cell Carcinoma of the Urinary Bladder.

INTRODUCTION: Small cell carcinoma of the bladder (SCCB) is a rare aggressive cancer, representing less than 1% of bladder cancers. Similar to small cell lung cancer (SCLC), it progresses rapidly, metastasizes early, and has a poor prognosis, complicating treatment. Unlike more common bladder cancers, SCCB often resists localized treatments and responds better to chemotherapy. However, its management generally relies on protocols for SCLC due to limited specific research. CASE PRESENTATION: A 47-year-old male from Bangladesh presented with intermittent painless hematuria, weak urinary stream,
