INTRODUCTION: Dysbaric osteonecrosis, albeit rare, have been reported in patients with decompression sickness. We report a patient with dysbaric osteonecrosis, diagnosed 60 days after presenting with decompression sickness.
CASE PRESENTATION: A 38-year-old, previously fit and healthy male, noted his tank running out of air at approximately 40-50 m while diving, surfaced rapidly before losing consciousness. Upon gaining consciousness, he noted loss of power on all four limbs. He completed 26 sessions of hyperbaric oxygen treatment. Magnetic resonance (MR) of the spine noted T2 abnormality in t

Clinicians commonly observe upper extremity signs and symptoms which result from median nerve entrapment and can develop at multiple sites along this nerve. Median nerve entrapment may occur at the distal humerus when the rarely present ligament of Struthers connects an anomalous bony spur of the humeral shaft to its medial epicondyle. The pronator syndrome refers to compromise of the median nerve in the proximal forearm region. This may result from entrapment between the 2 heads of the pronator teres, between the pronator teres and the flexor digitorum sublimis, or by the lacertus fibrosus ex

Septic arthritis is an infection of the joint space that requires prompt recognition by physicians. In children, the diagnosis of septic arthritis can often be challenging and delayed diagnosis can produce long-term morbidity. Posttraumatic septic arthritis is rarely reported and can be an equally challenging diagnosis. We present a case of a fully immunized 20-month-old boy with Streptococcus pneumoniae posttraumatic septic arthritis without evidence of an open fracture.

Intraoperative distraction aids in the restoration of length, facilitates reduction, assists in maintaining rotational alignment, and provides a temporary stable platform for definitive fixation of acute fractures, malunions, and nonunions. This technique has been described at length in the lower extremity; however, there is a paucity of literature regarding its use in the upper extremity. Distraction is the application of tension across a fracture site. Proximal and distal fixation may be achieved in several ways, with common instrumentation including the use of an external fixator set. Intra

This study was designed to evaluate the results of treatment of joint contractures (6 patients) and dislocations (2 cases) with an Ilizarov device. Six patients with restricting joint contractures were successfully treated and after gradual scar expansion they achieved a significant improvement in their range of motion. One patient with elbow instability after closed dislocation achieved 5 degrees to 130 degrees of motion, while one patient with an open dislocation achieved a range of motion from 15 degrees to 100 degrees. These results suggest that a hinged Ilizarov external fixation frame ca

BACKGROUND: Reverse shoulder arthroplasty (rTSA) is increasingly used as a revision option, but long-term survivorship and patient-reported outcomes (PROs) after revision rTSA remain poorly defined. We hypothesized that revision rTSA for failed rTSA would have inferior survivorship and PROs when compared to revision rTSA for failed anatomic total shoulder arthroplasty (aTSA) or failed hemiarthroplasty (HA). Moreover, we hypothesized that shoulders undergoing secondary revision would have a higher risk of failure when compared to those undergoing first-time revision surgery.
METHODS: This stud

Cubitus varus deformity is common in children. The purpose of this study was to assess the usefulness of computer simulation based on three-dimensional computed tomography (3D-CT) reconstruction with regard to preoperative planning and surgical treatment in children with traumatic cubitus varus deformity. The study included 13 patients with traumatic cubitus varus deformity between January 2012 and January 2015. The original 3D-CT data were transferred to Mimics 10.01 software. Special tools were used for simulation of supracondylar lateral wedge osteotomy of the humerus. Intraoperative elbow a

OBJECTIVE: Supracondylar closing wedge osteotomy is a standard operation for the management of post-traumatic cubitus varus deformity. There are many fixation methods for the broken ends of bone. However, most of these fixation methods are fraught with various complications. To evaluate the methods and functional results of double volume internal fixation for correction of adult post-traumatic cubitus varus deformity.
METHODS: The clinical data were retrospectively reviewed, from 22 cases of adults post-traumatic cubitus varus deformity between June 2007 and December 2010. There were 16 males

We compared two management strategies for the perfused but pulseless hand after stabilisation of a Gartland type III supracondylar fracture. We identified 19 patients, of whom 11 were treated conservatively after closed reduction (group 1). Four required secondary exploration, of whom three had median and/or anterior interosseus nerve palsy at presentation. All four were found to have tethering or entrapment of both nerve and vessel at the fracture site. Only two regained patency of the brachial artery, and one patient has a persistent neurological deficit. In six of the eight patients who wer

PROBLEM: Fractures of the distal humerus are difficult to treat. In elderly patients, diminished bone mineral quality and increased trauma-associated joint destruction may make stable joint reconstruction even more problematic. Furthermore, comorbidities and poor tolerance of joint immobilization might be additional factors which influence elbow function negatively. Until now, disagreement has existed on how to treat these fractures in elderly patients. Recommendations range from conservative treatment to primary total elbow replacement. So far, reports in the literature on whether or not open

PURPOSE: Supracondylar humerus fractures are the most common elbow injuries in children, with non-operative management frequently preferred in Gartland type I and selected type II cases. While biomechanical factors affecting outcomes have been well documented, the influence of socioeconomic status (SES) on functional and radiological results remains insufficiently explored.
METHODS: This retrospective cohort study analyzed 237 children (aged 3-13 years) treated non-operatively for supracondylar humerus fractures between February 2019 and February 2024. Socioeconomic status was determined usin

PURPOSE: Although acute compartment syndrome (ACS) is associated with pediatric supracondylar humerus (SCH) fractures, there are limited data describing its incidence and risk factors. The purpose of our study was to report the local and national incidence of ACS with SCH and floating elbow (concomitant SCH and forearm) fracture patterns and the associated risk factors.
METHODS: We retrospectively queried data for SCH fracture patients over a 4-year period from our institution (a level I pediatric trauma center) and the National Trauma Data Bank (NTDB). Data on demographics, mechanism of inju

Total elbow arthroplasty is the current gold standard of treatment for unreconstructable distal humerus fractures; however, longevity of the implant remains a concern in younger, more active patients. Distal humerus hemiarthroplasty offers an alternative and may allow for more durable results. The authors retrospectively evaluated the short-term clinical outcomes of 10 patients who underwent elbow hemiarthroplasty for distal humerus fractures. This short-term review suggests that distal humerus hemiarthroplasty may be an effective treatment for certain distal humerus fractures. Additional stud

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is widely recognized as an effective treatment for complex proximal humerus fractures (PHFs), providing reliable and predictable outcomes. However, whether glenoid lateralization confers tangible advantages in this setting remains unclear. This study aimed to evaluate the impact of different amounts of glenoid lateralization on clinical and functional outcomes following rTSA for PHFs. To our knowledge, this is the first study to specifically address this question.
METHODS: A total of 132 patients with 3 or 4-part PHFs were treated with rT

BACKGROUND: Posterior shoulder dislocation frequently results in a centrally located impression fracture of the anterior humeral head, known as reverse Hill-Sachs lesion (RHSL). Depending on its size and location, the RHSL can lead to engagement with the posterior glenoid rim and subsequently redislocation of the shoulder joint. The objective of this study was to present the clinical and radiologic outcomes of anatomic reconstruction of the humeral articular surface using arthroscopically assisted disimpaction in patients with acute posterior shoulder dislocation and an engaging RHSL.

BACKGROUND: Several techniques have been adopted during primary reverse shoulder arthroplasty (RSA) to manage glenoid bone defect. Among bone grafts, humeral head autograft is currently the mainstream option. However, autologous humeral heads may be unavailable or inadequate, and allografts may be a viable alternative. The aim of the present study was to evaluate the functional and radiologic outcomes of femoral head allografts for glenoid bone defects in primary RSA.
METHODS: We conducted a retrospective study with prospective data collection enrolling 20 consecutive patients who underwent R
