[Description of a series of cases of hypnic headache. Differentiation between sexes].

INTRODUCTION: Hypnic headache is an infrequent type of primary headache characterised by appearing almost exclusively during sleep and by waking the patient up. The pain is dull (generally bilateral), is not associated to autonomic signs and usually appears from the age of 50 onwards. PATIENTS AND METHODS: A 10-year prospective study was conducted which describes the patients with hypnic headache who were attended in a specialised headache clinic. Data collected include demographic variables, the characteristics of the pain and response to treatment. Data from males and from females were comp

Developmental plasticity of GABAergic neurotransmission to brainstem motoneurons.

KEY POINTS: Critical homeostatic behaviours such as suckling, swallowing and breathing depend on the precise control of tongue muscle activity. Perinatal nicotine exposure has multiple effects on baseline inhibitory GABAergic neurotransmission to hypoglossal motoneurons (XIIMNs), consistent with homeostatic compensations directed at maintaining normal motoneuron output. Developmental nicotine exposure (DNE) alters how GABAergic neurotransmission is modulated by acute activation of nicotinic acetylcholine receptors, which may provide insight into mechanisms by which nicotine exposure alters mot

Effects of slow oscillatory HD-tDCS on creative cognition and prefrontal cortex activity: A multimodal EEG-fNIRS study.

Creativity relies on the dynamic interaction between the default mode (DMN) and frontoparietal control (FPCN) networks, supported by controlled attentional processing and associative strategies. Slow wave activity (SWA: 0.1- 1 Hz) is a critical component of network reorganization during non-REM sleep; while slow oscillatory tDCS (so-tDCS) at 0.75 Hz has been shown to acutely modulate SWA to enhance cognition. This study employed a novel concurrent EEG-fNIRS multimodal imaging approach to investigate whether so-tDCS at 0.75 Hz over the bilateral dorsolateral prefrontal cortex (DLPFC) and medial

Machine Learning for Diagnosis and Differentiation of Central Disorders of Hypersomnolence: A Systematic Review.

BACKGROUND AND PURPOSE: Central disorders of hypersomnolence (CDH) are, except for Narcolepsy Type 1 (NT1), difficult to diagnose and manage because of overlapping features and the lack of reliable biomarkers. Machine learning (ML) has the potential to improve diagnosis by detecting subtle physiological patterns and distinguishing between CDH subtypes. This review systematically explores current ML applications in CDH, assesses their limitations, and suggests future directions. METHODS: Following PRISMA guidelines, MEDLINE, Embase, PsycINFO, IEEE Xplore, CINAHL, Web of Science, and Google Sch

Association of cholesterol and glycemic state biomarkers with phenotypic variation and Parkinson's disease progression: The Oxford Discovery cohort.

BackgroundParkinson's disease (PD) has marked phenotypic variability. Increased lipids have been suggested as being neuroprotective whilst hyperglycemia may increase α-synuclein aggregation.ObjectiveWe have tested whether high total cholesterol and high-density lipoprotein cholesterol (HDL-C) and low levels of fructosamine are associated with better PD phenotypes and predict less rapid progressionMethodsNon-fasting serum HDL-C, total cholesterol, and fructosamine were measured at baseline in 866 patients with early PD (median duration, 0.96; IQR, 0.43-1.98 years) from the Oxford Discovery coho

Developmentally unique cerebellar processing prioritizes self-over other-generated movements.

Animals must distinguish the sensory consequences of self-generated movements (reafference) from those of other-generated movements (exafference). Only self-generated movements entail the production of motor copies (i.e., corollary discharges), which are compared with reafference in the cerebellum to compute predictive or internal models of movement. Internal models emerge gradually over the first three postnatal weeks in rats through a process that is not yet fully understood. Previously, we demonstrated in postnatal day (P) P8 and P12 rats that precerebellar nuclei convey corollary discharge

Motor and non-motor features in Parkinson's Disease patients carrying GBA gene mutations.

BACKGROUND: Mutations of the Glucocerebrosidase (GBA) gene are the most common genetic risk factor yet discovered for Parkinson's Disease (PD), being found in about 5-14% of Caucasian patients. OBJECTIVE: We aimed to assess motor and non-motor symptoms (NMS) in patients with GBA-related PD (GBA-PD) in comparison with idiopathic PD (iPD) subjects using standardized and validated scales. METHODS: Eleven (4 M, 7 F) patients with GBA-PD and 22 iPD patients, selected from the same cohort and matched for gender, age, and disease duration, were enrolled. The disease severity was assessed by Unified

The clinical phenotype of psychiatric-onset prodromal dementia with Lewy bodies: a scoping review.

BACKGROUND: Recent consensus research criteria have identified a 'psychiatric onset' form of prodromal dementia with Lewy bodies (DLB) characterised by prominent late-onset psychiatric symptoms. Although recognised as important to raise the index of diagnostic suspicion, evidence regarding this cohort was deemed too limited to impose formal criteria. We reviewed the published literature on psychiatric-onset DLB to identify key clinical characteristics and evidence gaps to progress our understanding of this entity. METHODS: Medline, PubMed and Embase were searched for relevant articles contain

Measuring anxiety in Lewy Body Disease - Which scale to choose?

BACKGROUND: Anxiety is among the most prevalent mood disorders in Lewy Body Disease (LBD) (i.e., Parkinson's disease (PD), Dementia with Lewy bodies DLB), and those at-risk for developing LBD (e.g. isolated REM Sleep Behaviour Disorder (iRBD)). Yet, there is little consensus on which clinical scale best evaluates anxiety across synuclein-based diseases. OBJECTIVE: This study compared the convergent validity of commonly used anxiety scales across PD, DLB and iRBD patients. METHODS: Anxiety was assessed using the Hospital Anxiety and Depression Scale (HADS-A), State-Trait Anxiety Inventory (ST

Clinical features of autopsy-confirmed multiple system atrophy in the Mayo Clinic Florida brain bank.

BACKGROUND: Multiple system atrophy (MSA) presents with various combinations of autonomic dysfunction, parkinsonism, and cerebellar ataxia. Although clinical diagnostic criteria have been widely used, the sensitivity and specificity are suboptimal. This study aims to provide evidence supporting the revision of the current diagnostic criteria for MSA. METHODS: Medical records of 171 patients with autopsy-confirmed MSA in the Mayo Clinic brain bank were reviewed with regard to their clinical features and diagnoses. Pathologic features, including concomitant pathologies (i.e., Alzheimer-related

Impaired metabolic brain networks associated with neurotransmission systems in the α-synuclein spectrum.

INTRODUCTION: While the involvement of multiple neurotransmitter systems in α-synucleinopathies is reported, a comprehensive study on their metabolic connectivity reconfiguration in the preclinical and clinical disease-spectrum is lacking. We aimed to investigate shared and disease-specific neural vulnerabilities of the nigro-striato-cortical dopaminergic, noradrenergic and cholinergic networks within the α-synuclein-spectrum, by means of metabolic connectivity approach. METHODS: We collected 34 polysomnography-confirmed isolated REM sleep behaviour disorder (iRBD) subjects, 29 idiopathic Par
