Measurement of structural abnormalities in arthritis using radiographic images.

This article reviewed the current methods for scoring radiographic abnormalities in RA with emphasis on reliability. There is disagreement on the expected course of RA with usual treatment and there is no clear resolution of this disagreement although several factors that contribute to this disagreement have been identified. Scoring joint damage has been effective in establishing benefit from drug treatment for nine agents, five within the last 3 years. Scoring will continue to be used to examine the benefit of new agents in the immediate years to come. A panel of experienced readers agreed th

Comparative Analysis of Dorsal Distraction Plating Versus External Fixation in the Management of Intra-Articular Distal Radius Fractures.

INTRODUCTION: Fractures located in close proximity to the lower end of the radius bone, which make up around 16% of fractures requiring emergency treatment require careful treatment to regain mobility in the wrist joint and the fingers. Although classic external fixation (ExFix) methods are widely used, newer procedures such as dorsal distraction plating are showing potential . This study investigates the concepts, debates, and results of several methods so that to determine the most effective fracture treatment. MATERIALS AND METHODS: The experiment will recruit individuals with intricate fr

Joint contractures responsive to immunosuppressive therapy in a girl with childhood-onset systemic sclerosis double-seropositive for rare anti-nucleolar autoantibodies: a case report.

BACKGROUND: Systemic sclerosis (SSc; scleroderma) is an autoimmune connective tissue disease that affects the skin and subcutaneous tissue, in addition to the internal organs of the whole body. Onset in childhood is uncommon; however, both patients with childhood-onset and adult-onset SSc are positive for anti-nuclear antibodies (ANAs).Detection of SSc-related anti-nuclear antibodies is often useful for predicting clinical features, disease course, and outcomes. CASE PRESENTATION: A 5-year-old Japanese female manifested gradually progressive abnormal gait disturbance, regression of motor deve

Infrared thermography in children: a reliable tool for differential diagnosis of peripheral microvascular dysfunction and Raynaud's phenomenon?

BACKGROUND: Infrared Thermography (IRT) has been used for over 30 years in the assessment of Raynaud Phenomenon (RP) and other peripheral microvascular dysfunctions in adults but, to date, very little experience is available on its use in children for this purpose. The first aim of the study was to assess reproducibility of thermographic examination after cold exposure by comparing inter-observer agreement in thermal imaging interpretation. The secondary aim was to evaluate whether IRT is reliable to diagnose and differentiate peripheral circulation disturbances in children. METHODS: Children

Congenital limb deficiency in Japan: a cross-sectional nationwide survey on its epidemiology.

BACKGROUND: Congenital limb deficiency is a rare and intractable disease, which impairs both function and appearance of the limbs. To establish adequate medical care, it is necessary to reveal the actual conditions and problems associated with this disease. However, there have been no extensive epidemiological surveys in Japan addressing this disease. This is the first nationwide epidemiological survey of congenital limb deficiency in this country. METHODS: With the cooperation of epidemiology experts, we performed a two-stage nationwide survey to estimate the number of patients with congenit

Value of ultrasonography for diagnosis of synovitis associated with rheumatoid arthritis.

AIM: To investigate the value of ultrasonography (US) for diagnosing synovitis associated with rheumatoid arthritis (RA). METHOD: Bilateral metacarpophalangeal (MCP), proximal interphalangeal (PIP) II-V and wrist joints of 46 RA patients and 35 healthy controls were evaluated by quantitative and semiquantitative US. Wrists on more severely affected sides of 20 of the 46 patients also underwent magnetic resonance imaging (MRI). The MRI and US results were compared. The US cutoff to distinguish pathology was calculated. The two US methods were compared and the correlation between quantitative m

Upper Extremity Surgery in Hirayama Disease: A Modification of the Current Algorithm.

INTRODUCTION: Hirayama disease (HD) is a rare, nonfamilial, monomelic amyotrophy in which patients present with muscle atrophy and weakness of the forearms and hands, either unilateral or bilateral, and without sensory loss. Current treatment guidelines describe the role of conservative treatments including cervical collars and neurotropic medications, as well as spinal surgery in select patients. Upper extremity surgery has not yet been incorporated into the treatment algorithm of HD. The objective of this study is twofold: to present a case series of HD patients treated with the incorporatio

Superficialis Sling (Flexor Digitorum Superficialis Tenodesis) for Swan Neck Reconstruction.

Swan neck deformity, or hyperextension of the proximal interphalangeal joint, may occur secondary to trauma, rheumatoid arthritis, cerebral palsy, or Ehlers-Danlos syndrome, and can be treated with tenodesis of one slip of the flexor digitorum sublimis tendon. This technique has several variations, differing primarily in the specific location and method that a single slip of the flexor digitorum sublimis tendon is secured, but they all serve to create a static volar restraint against hyperextension. Options include tunneling the tendon through the bone of the proximal phalanx, attaching the te

Distal interphalangeal joint arthrodesis with the reverse fix nail.

PURPOSE: Arthrodesis of distal interphalangeal (DIP) and thumb interphalangeal (IP) joints is a well-established procedure for the treatment of pain, deformity, and instability associated with arthritis and traumatic conditions. Fixation with various headless compression screws has become increasingly common and with favorable outcomes. However, there are still characteristic complications, such as hardware prominence with the distal end of the screw incompletely buried in the distal phalanx. The purpose of this study was to review the outcomes and complications associated with the arthrodesis

Tendon transfer for radial nerve palsy: a single tendon to restore finger extension as well as thumb extension/radial abduction.

Since 1994, the author has been treating irreparable radial nerve palsy with pronator teres to the extensor carpi radialis brevis (for wrist extension) and a single tendon (flexor carpi radialis or ulnaris) transfer to restore finger extension as well as thumb extension/radial abduction. We sought to investigate whether these patients are able to flex the fingers with the thumb in abduction/extension posture. This was a prospective study over a 5-year period, and the results of this transfer in 15 consecutive patients (mean age 28 years) were analyzed. At final follow-up (mean 30 months), all

[Thumb duplication].

Thumb duplication is one of the most frequent congenital abnormality. Treatment is rarely limited to surgical amputation of one of the thumb duplicate. Wassel proposed a classification of seven categories based on the level of skeletal division, the most frequent being the type IV. Factors influencing the surgical result are numerous, including the importance of angular deviation of both duplicates, the joint stability and the duplicate size. In the distal categories (type I and II), when both duplicates are symmetrical, thumb reconstruction should be achieved by fusion of both duplicates on t

Operative treatment of metacarpal and phalangeal shaft fractures.

Diaphyseal fractures of the metacarpals and phalanges are common injuries that can lead to impairment of hand function. The fracture pattern and soft-tissue injury vary with the mechanism of injury. The imbalance of the flexor and extensor forces created by displaced fractures will often produce a secondary angulatory deformity. Nonoperative treatment is indicated for reducible and stable fracture configurations. Irreducible or unstable fracture patterns require open or closed reduction and fixation. Reduction must be assessed in flexion and extension to ensure correct rotatory alignment. Frac
