We propose a maximum likelihood estimator (MLE) of the kappa coefficient from a 2 x 2 table when the binary ratings depend on patient and/or clinician effects. We achieve this by expressing the logit of the probability of positive rating as a linear function of the subject-specific and the rater-specific covariates. We investigate the bias and variance of the MLE in small and moderate size samples through Monte Carlo simulation and we provide the sample size calculation to detect departure from the null hypothesis H0: kappa = kappa 0 in the direction of H1: kappa > kappa 0.

In contrast to the great number of etiological models for bulimia nervosa, few conceptions determine the therapeutic practice. Two of these models, the ego-psychological model of reduced impulse-control and the behavioural model focussing the influence of "restrained eating" are empirically compared. We address the problem of testing for independence between X and Y in two situations. In the first we assume that the joint distribution of X and Y is unknown but the observations on X and Y are identifiable. In the second case we assume that the distribution of (X, Y) is exchangeable. Here we consider both when (X, Y) are identifiable and when they are not. We illustrate applications to the testing of independence in DNA databases and in twin studies.

The relation between fetal binocular distance and menstrual age was determined by cross-sectional analysis of 555 normal fetuses (14-40 weeks) using real-time sonography. Mathematical modelling of the data demonstrated that the binocular distance growth curve, similar to the biparietal diameter, is nonlinear. This paper deals with model selection and the estimation of odds ratios from cross-classified frequencies in the presence of extraneous factors. Perinatal mortality in sibships has been examined using the Medical Birth Registry of Norway. Using linear logistic regression, parity specific perinatal mortality in the period 1967-1981 has been analysed, controlling simultaneously for maternal age, year of birth and survival of earlier births.
