Accurate and reliable forecasting models are critical for guiding public health responses and policy decisions during pandemics such as COVID-19. Retrospective evaluation of forecasting performance provides an essential framework for assessing and improving epidemic prediction methods. In this study, we used COVID-19 wastewater data from CDC's National Wastewater Surveillance System to generate sequential weekly retrospective out-of-sample forecasts for the United States from March 2022 through September 2024, both at the national level and for four major regions (Northeast, Midwest, South, an

BACKGROUND: Transfusion thresholds in upper gastrointestinal bleeding are debated; hemoglobin cutoffs of 70-80 g/L are widely cited yet inconsistently applied. Common risk scores offer limited individualized guidance and rarely provide calibrated, interpretable predictions for transfusion decisions.
OBJECTIVE: This study aimed to develop and validate a two-stage, clinically constrained gradient-boosting framework (Medically Constrained Gradient Boosting [MCGB]) that predicts transfusion need and estimates transfusion dose with quantified uncertainty and to implement a prototype recommendation

Motion sickness is expected to become more prevalent in road transportation as passengers increasingly engage in non-driving-related activities under automated driving. This study quantified the relative contributions of exposure time (discrete trial index), physiological signals, and vibration-derived features to motion sickness prediction under controlled road vibration. Thirty-nine participants were exposed to four road profiles reproduced on a motion simulator while electrocardiography (ECG), galvanic skin response (GSR), triaxial head acceleration, and motion sickness ratings were recorde

BACKGROUND: Efficiently identifying patients with human immunodeficiency virus (HIV) using administrative health care data (e.g., claims) can facilitate research on their quality of care and health outcomes. No prior study has validated the use of only ICD-10-CM HIV diagnosis codes to identify patients with HIV.
METHODS: We validated HIV diagnosis codes among women enrolled in a large U.S. integrated health care system during 2010-2020. We examined HIV diagnosis code-based algorithms that varied by type, frequency, and timing of the codes in patients' claims data. We calculated the positive p

International Classification of Diseases, 10th Revision codes for rheumatic heart disease (RHD) include valvular heart disease of unspecified origin, limiting their usefulness for estimating RHD burden. A cross-disciplinary national consultation developed an algorithm to improve RHD identification in hospital data. The algorithm has been operationalised and piloted. The algorithm developed categorised 32% of RHD-coded patients as probable/possible RHD. We outline a series of research initiatives to improve identification of RHD in administrative data thereby contributing to monitoring the RHD

Accurate estimation of wound age is a critical issue in forensic practice. However, injured tissues in real-world cases are often affected by postmortem changes, which may reduce the reliability of conventional laboratory methods. This study established a skin contusion model in SD rats, collected skin tissue samples under different wound ages (0-48 h) and postmortem intervals (0 h and 24 h), utilized ATR-FTIR to obtain molecular spectral information, and combined it with 10 machine learning algorithms to construct predictive models. The results demonstrated that 24 h postmortem changes affect

BACKGROUND: Predictive models in surgery promise to improve clinical care by anticipating complications, guiding decision-making, and supporting personalized treatment strategies. Although their potential to enhance outcomes and efficiency is substantial, their integration into clinical practice also raises profound ethical challenges.
ETHICAL FRAMEWORK: These challenges span the entire lifecycle of predictive models from data collection and development to validation and clinical use. They touch upon patient privacy, algorithmic bias, transparency, and the shifting responsibilities of clinici

BACKGROUND: Scleroderma is a serious chronic autoimmune disease in which a patient's disease state manifests in several irregularly spaced longitudinal measures of lung, heart, skin, and other organ systems. Threshold crossings of pulmonary and cardiac measures indicate potentially life-threatening key clinical events including interstitial lung disease (ILD), cardiomyopathy, and pulmonary hypertension (PH). The statistical challenge is to accurately and precisely predict these events by using all of the clinical history for the patient at hand and for a reference population of patients.

BACKGROUND: The recent failure of clinical trials to treat Alzheimer's disease (AD) indicates that the current approach of modifying disease is either wrong or is too late to be efficient. Mild cognitive impairment (MCI) denotes the phase between the preclinical phase and clinical overt dementia. AD mouse models that overexpress human amyloid-β (Aβ) are used to study disease pathogenesis and to conduct drug development/testing. However, there is no direct correlation between the Aβ deposition, the age of onset, and the severity of cognitive dysfunction.
OBJECTIVE: To detect and predict MCI wh

Fetal electrocardiogram (fECG) monitoring is crucial for assessing fetal condition during pregnancy. However, current fECG extraction algorithms are not suitable for wearable devices due to their high computational cost and multi-channel signal requirement. The paper introduces a novel and efficient algorithm called Adaptive Improved Permutation Entropy (AIPE), which can extract fetal QRS from a single-channel abdominal ECG (aECG). The proposed algorithm is robust and computationally efficient, making it a reliable and effective solution for wearable devices. To evaluate the performance of the

Clinically relevant cardiovascular system metrics can either be directly measured (e.g., systolic and diastolic pressure) or estimated (e.g., arterial compliance and resistance) from arterial blood pressure waveforms. Accurate characterization of such information requires identification of each individual arterial pressure pulse. This task is difficult due to variability in pulse morphology over time and throughout the arterial tree. While various pulse detection methods have been developed, many rely on arbitrary rules and thresholds or are computationally complex. We propose a simple real-ti

Diagnosis of sleep-disordered breathing is based on the presence of an abnormal breathing pattern during sleep. In this study, an algorithm was developed for the offline breath-to-breath analysis of the nocturnal respiratory recordings. For that purpose, respiratory signals (nasal airway pressure, thoracic and abdominal movements) were divided into half waves using period amplitude analysis. Individual breaths were characterized by the parameters of the half waves (duration, amplitude, and slope). These values can be used to discriminate between normal and abnormal breaths. This algorithm was

Rotational 3-D shear wave elasticity imaging (3D-SWEI) offers many opportunities for the development of biomechanical biomarkers of muscle health. However, methods for material parameter reconstruction to date have involved manual processing steps, which are impractical for clinical implementations. In this work, we describe and optimize algorithms for the automatic processing of 3D-SWEI skeletal muscle data and explore the most important factors affecting the success of automatic estimation of the shear wave speed (SWS) along and across the fibers. We compare the results of several automatic

INTRODUCTION: Community health workers (CHWs) help patients navigate community resources. CHW programs can improve health outcomes and reduce healthcare utilization, but identifying eligible patients is challenging. We developed an electronic health record (EHR)-based algorithm trained to predict referrals to a CHW program. Our objective was to evaluate whether an algorithm trained on historical referrals identifies a subgroup of patients that differs from the subgroup identified through questionnaire-based health-related social needs (HRSN) screening alone.
METHODS: This analysis used data f
