The majority of research on distal radius fractures consists of retrospective, descriptive studies of patients with unstable fractures requiring fixation. The purpose of this investigation was to report on impairments in flexibility, grip strength, and motor control and on the presence of swelling and atrophy immediately after cast immobilization of closed reductions of simple distal radius fractures. Sixteen adult subjects from Kaiser Permanente Medical Center, San Francisco, entered the study, and 13 completed it. At the initial evaluation, upper extremity ranges of motion, grip strength, fo

BACKGROUND: Blood flow restriction therapy (BFRT) has gained recognition in recent years as an adjunct to rehabilitation to maximize results for patients with poor strength and endurance. However, literature on the effects of BFRT for conditions afflicting the upper extremity is limited. The purpose of this study was to analyze the available literature regarding the role of BFRT in upper extremity conditions and evaluate its influence on functional and patient reported outcomes.
METHODS: PubMed, Embase, and Cochrane Trials were searched in July 2024. Only randomized controlled trials (RCTs) w

Bone tumors of the upper limb are a common cause of bone pain and pathological fractures in both old and young populations. Surgical reconstruction and limb salvage have become valid options for these patients despite this kind of surgery being challenging due to the need for wide bone resection and the involvement of surrounding soft tissues. Computer-assisted technology helps the surgeon in pre-operative planning and in designing customized implants. The aim of this study was to investigate the surgical outcomes and complications of custom-made prostheses in oncologic reconstruction of the u

INTRODUCTION: Perilunate dislocations are rare high-energy injuries which may often have the potential to cause lifelong disability of the wrist if not addressed optimally. Hence, early recognition, diagnosis, and intervention are of paramount importance in the restoring function and prevention of morbidity. Lunate dislocations are the fourth and last stage of perilunate dislocations being extremely rare, with volar dislocations representing <3% of perilunate dislocations.
CASE REPORT: A 24-year-old man suffered from an alleged history of fall from a bike on an outstretched hand following whi

BACKGROUND: Pathologies of the long head of biceps tendon (LHB) can cause anterior shoulder pain. Surgical treatment includes tenotomy and tenodesis (TD), however TD causes fewer cosmetic deformities and less cramping of the biceps muscle. To date, numerous techniques for TD have been described but the "gold standard" has not yet been established.
OBJECTIVES: The purpose of this study is to evaluate the functional and cosmetic outcome following subpectoral biceps TD with an interference screw and a cortical-button (STLHB-IC) after 1 year.

BACKGROUND: Stress shielding of the proximal humerus represents an increasing challenge for the implantation of total shoulder arthroplasty (TSA). The literature on this topic is heterogeneous, and many studies analysing its clinical impact are underpowered, justifying a systematic review of current evidence.
MATERIALS AND METHODS: On 1 September 2024, the Cochrane Library, PubMed, and Web of Science were searched for studies on stress shielding in shoulder arthroplasty. All studies meeting inclusion criteria were included in the qualitative review. Clinical significance was assessed through

BACKGROUND: Osteochondritis dissecans of the capitellum is a focal injury to the articular cartilage and subchondral bone observed in young athletes. This can cause activity-limiting pain, intra-articular loose bodies, and limited range of motion.
INDICATIONS: Stable lesions identified early with minimal symptoms can be managed nonoperatively. Indications for surgical management include unstable lesions, loose bodies, mechanical symptoms, and stable lesions that failed 6 months of nonoperative treatment. A range of surgical options have been advocated-including arthroscopic versus open loose

INTRODUCTION: While tuberosity reinsertion significantly improves the short-term functional outcomes of reverse shoulder arthroplasty done for proximal humerus fracture, we do not know how well these results hold over the long term. The objective of this study was to analyze the effect of tuberosity reinsertion on the quality of life of patients and the functional outcomes of the operated limb after a minimum follow-up of 5 years.
METHODS: Sixty-two patients were included. Their mean age at the final review was 79 ± 10 years. The Katz and Lawton scales, Constant score, DASH and SSV were colle

BACKGROUND: There remains a paucity of studies examining the conversion of failed hemiarthroplasty (HA) to reverse total shoulder arthroplasty (RTSA). Therefore, the purpose of this study was to examine a large series of revision HA to RTSA.
METHODS: A population of 157 patients who underwent conversion of a failed HA to a revision RTSA from 2006 through 2014 were included. The mean follow-up was 49 months (range, 24-121 months). The indications for revision surgery included instability with rotator cuff insufficiency (n = 127) and glenoid wear (n = 30); instability and glenoid wear were asso

BACKGROUND: Patients suffering from a pathological fracture or painful bony lesion because of metastatic bone disease often benefit from a total joint replacement. However, these are large operations in patients who are often weak. We examined the patient survival and complication rates after total joint replacement as the treatment for bone metastasis or hematological diseases of the extremities.
PATIENTS AND METHODS: 130 patients (mean age 64 (30-85) years, 76 females) received 140 joint replacements due to skeletal metastases (n = 114) or hematological disease (n = 16) during the period 20

Physical trauma is generally accepted as a possible factor in the pathogenesis of rheumatoid arthritis. In the last ten years, there have been a few rare case reports of physical trauma precipitating psoriasic arthritis. We observed two such cases following an occupational accident discovered one and a half year and two and a half year after onset of the first clinical manifestations. In the first case, a 43-year-old man had a fracture of the right calcaneus in March 1991. He was treated with nailing and also required emergency surgery of the posterior tibial artery. The tibiotarsal joint was

BACKGROUND: While reverse shoulder arthroplasty (rTSA) is commonly utilized for rotator cuff tear arthropathy, indications have expanded to include, primary glenohumeral osteoarthritis (GHOA) with intact cuff. The presence of an intact cuff may influence outcomes after rTSA because preserved cuff musculature can contribute to shoulder stability and force which could potentially improve postoperative function and reduce complication rates. However, studies have reported contradictory results on whether or not an intact cuff would provide better outcomes in patients receiving an rTSA.

INTRODUCTION: The Grammont-style reverse shoulder arthroplasty (RSA) relies on medialization and distalization of the shoulder center of rotation. Lateralized designs have recently gained popularity. The amount of lateralization, however, remains a controversial topic. The purpose of this study was to correlate the change in humeral offset (HO) with outcomes and complications following RSA. We hypothesized that a lateralized HO following RSA would be associated with improved range of motion (ROM), better patient-reported outcomes (PROs), and fewer complications.
