Long-Term Clinical and Radiological Results of Hemi-Hamate Arthroplasty for Proximal Interphalangeal Fracture Dislocation.

PURPOSE: This study aimed to assess the long-term clinical and radiologic results after hemi-hamate arthroplasty in patients with acute or chronic irreparable fractures of the base of the middle phalanx. METHODS: Between 2010 and 2014, hemi-hamate arthroplasties were performed on 12 patients with a mean age of 29.5 (SD 4.9) years. The patients included acute cases (treated within 6 weeks of the fracture) and chronic cases (treated after 6 weeks). We assessed the cases at a mean follow-up of 10.7 years. We recorded visual analog scale (VAS) scores, Quick Disabilities of the Arm, Shoulder, and

The Syringe External Fixator: Short- and Medium-Term Functional Outcomes From This Inexpensive and Customizable Treatment for Comminuted Intra-Articular Fractures of the Hand.

PURPOSE: Comminuted intra-articular fractures and fracture dislocations of the metacarpophalangeal (MCP) and interphalangeal joints are challenging. Dynamic external fixation, permitting early joint motion while still minimizing forces across the healing joint, can result in acceptable postoperative active range of motion (AROM). However, some fractures are not initially stable enough for early dynamic motion; further, many available dynamic external fixation systems are costly and cumbersome. We reviewed our experience using an external fixator made from a 1-mL syringe and K-wires and report

An alternative technique for the management of phalangeal enchondromas with pathologic fractures.

PURPOSE: Enchondroma of the hand with a pathologic fracture is generally treated by tumor curettage and bone grafting after the fracture has healed. However, delayed surgery postpones definitive diagnosis and prolongs the period of disability. We have treated pathologic fractures in a single stage through a modified lateral surgical approach with curettage of the tumor and stabilization using injectable calcium sulfate cement. The aim of this study was to report the outcomes of treatment with this material and the modified approach. METHODS: Between 2006 and 2010, we enrolled 8 patients with

Incidence and treatment of complications, suboptimal outcomes, and functional deficiencies after pollicization.

PURPOSE: To evaluate the incidence and treatment of complications, suboptimal outcomes, and functional deficiencies after pollicization, and the need for additional surgical procedures. METHODS: A total of 73 index finger pollicizations performed by a single surgeon were identified. We retrospectively evaluated all available patient records for perioperative complications, suboptimal outcomes, and functional deficiencies of the pollicized digit. RESULTS: There were 8 complications in the perioperative period (including 3 cases of venous congestion, 4 cases of marginal necrosis, and 1 infecti

Reconstruction for Wassel type III radial polydactyly with two digits equal in size.

PURPOSE: To report the surgical outcomes for Wassel type III radial polydactyly thumbs of equal or nearly equal size treated by excising the radial thumb with augmentation. METHODS: We have reconstructed 15 cases of Wassel type III radial polydactyly, in which the duplicated digits were equal or almost equal in size, by ablation of a radial digit. The distal articular surface of the radial proximal phalanx was preserved in order to maintain stability of the interphalangeal joint. The stability of the retained thumb was further augmented by tendon repositioning, restoration of the radial colla

Thumb hypoplasia.

Hypoplasia of the thumb refers to a spectrum of clinical abnormalities ranging from a slightly small digit to complete absence (or aplasia) of the thumb unit. As a component of radial dysplasia, thumb hypoplasia can be either an isolated entity or seen in conjunction with other elements of radial longitudinal deficiency. Treatment of this condition initially involves identifying and addressing co-morbid anomalies in other body systems. The severity of thumb hypoplasia is then graded so that appropriate treatment methods can be instituted. The goal of treatment is ultimately to provide the chil

Fractional flexor tendon lengthening for advanced metacarpophalangeal flexion contracture in rheumatoid hands.

This technical report discusses a subgroup of rheumatoid patients who have minimal ulnar drift but a severe fixed metacarpophalangeal joint flexion contracture for whom conventional metacarpophalangeal joint arthroplasty alone was insufficient to correct the deformity. We describe a surgical technique to deal with this clinical problem that uses fractional flexor tendon lengthening in the forearm to correct the severe flexion deformity at the metacarpophalangeal joint.

[A case of Marshall-Smith syndrome].

Marshall-Smith syndrome (MSS) is a rare clinical disorder, characterized by accelerated skeletal maturation, facial anomalies, failure to thrive and death in early infancy due to respiratory complications. We experienced the difficult airway case with MSS. A 4-year-old boy underwent tenosynovectomy for the snapping finger. Although his upper airway was diagnosed as almost normal on the fiber-optic laryngoscope examination, he frequently showed pharyngeal collapse during sleep. Anesthesia was induced and maintained with N2O-O2-sevoflurane using oral-airway without muscle relaxants. But, when we

Hypocomplementemic urticarial vasculitis, jaccoud's arthropathy, valvular heart disease, and reversible tracheal stenosis: a surfeit of syndromes.

We describe a patient who, during 29 years of observation, manifested polyarthralgia and polyarthritis leading to progressive deformity of the joints of hands and feet (without loss of cartilage or erosion of bone); persistent urticaria made worse by cold and accompanied by hypocomplementemia; and progressive cardiac valvular disease with mitral and aortic stenosis and regurgitation. In 1996, she developed subglottic tracheal stenosis that resolved by the end of 1997 without a change in treatment, which has consisted of low dose azathioprine, glucocorticoid, and nonsteroidal antiinflammatory d

Microsurgical toe to thumb transplantation for traumatic thumb loss.

Toe-to-thumb transfer was performed on 13 patients with traumatic thumb loss, using microvascular technique. Of those, 9 patients had industrial injury, 3 patients had a sharp cut injury and the remaining one had animal bite injury. The toes used for the transfer were the great toe(5), and the second toe(8). There were twelve successes, and one partial success. Ten patients have been followed-up for more than 12 months, and they are reviewed in detail. Total active motion after great toe and second toe transfer was 45 degrees and 68 degrees respectively. Static two-point discrimination was 10-

Use of a distraction plate for distal radial fractures with metaphyseal and diaphyseal comminution.

BACKGROUND: Distal radial fractures with extensive comminution involving the metaphyseal-diaphyseal junction present a major treatment dilemma. Of particular difficulty are those fractures involving the articular surface. One approach is to apply a dorsal 3.5-mm plate extra-articularly from the radius to the third metacarpal, stabilizing the diaphysis and maintaining distraction across the radiocarpal joint. METHODS: Twenty-two patients treated with a distraction plate for a comminuted distal radial fracture were included in the study. With use of three limited incisions, a 3.5-mm ASIF plate

Clinical Outcomes of Closed Reduction and External Fixation for Isolated Second Metacarpal Base Fracture-Dislocations.

OBJECTIVE: The isolated second metacarpal base fracture-dislocations in adults are rare, and the traditional main treatments include closed reduction with the plaster or splint and open reduction with internal fixation (ORIF). However, closed reduction with the plaster or splint is not solid enough and ORIF can damage the surrounding important tissues. The purpose of this study was to explore the clinical outcomes of closed reduction and external fixation (CREF) for the treatment of isolated second metacarpal base fracture-dislocations. METHODS: Ten patients who suffered isolated second metac

Surgery for Pediatric Trigger Finger.

BACKGROUND: Pediatric trigger finger (PTF) is an uncommon condition that is 10 times less common than trigger thumb. The Quinnell grade is utilized to quantify the extent of the triggering on a 4-point scale (0 = normal movement, 1 = uneven movement, 2 = actively correctable triggering, 3 = passively correctable triggering, and 4 = fixed deformity)1. Less extensive triggering can be treated nonoperatively with use of monitoring or splinting; however, the reported resolution rates are low, with only 30% of PTF cases treated nonoperatively achieving complete resolution1. Splinting has also been

Alpha and beta event-related synchronization as neural markers of self-other processing during sensorimotor coordination.

Interpersonal synchrony requires continuously balancing internally generated timing predictions with externally driven adjustments to a partner's behavior. Neural oscillations in alpha and beta bands have been implicated in prediction processes during joint action, yet their role in self-other integration remains unclear, particularly across varying levels of partner adaptivity. Here, we examined event-related desynchronization and synchronization (ERD/ERS) in the alpha (8-12 Hz) and beta (15-25 Hz) bands during a sensorimotor synchronization task. 47 participants performed a finger-tapping ta
