Clinical and radiological outcomes of high tibial osteotomy with combined fixator-assisted nailing and subtubercle tibial osteotomy.

OBJECTIVE: The aim of this study was to assess the clinical and radiological results of our high tibial osteotomy technique combining fixator-assisted nailing and subtubercle tibial osteotomy in varus malalignment. METHODS: This was a retrospective study of a consecutive series of 32 knees in 32 patients ('2 follow-up loss' 12 males and 18 females; mean age at the time of operation: 50.6±7.8 (36-62) years) operated on between 2014 and 2016. Radiographic and clinical measurements were assessed pre- and postoperatively. Kolmogorov-Smirnov, paired t and Wilcoxon rank tests were used in the stati

Fassier-Duval Rods are Associated With Superior Probability of Survival Compared With Static Implants in a Cohort of Children With Osteogenesis Imperfecta Deformities.

BACKGROUND: The survival of Fassier-Duval (FD) telescoping rods as compared with static implants in children affected by osteogenesis imperfecta is not well characterized. The purpose of this study was to compare risk of lower extremity implant failure in FD rods versus static implants. METHODS: Data were retrospectively collected from patients with osteogenesis imperfecta who underwent surgical treatment using either FD rods or static implants (Rush rods, flexible nails, or Steinmann pins) between 1995 and 2015. The timing of implant failure was the primary outcome variable of interest. Comp

Short-Term Radiographic Outcome After Distal Chevron Osteotomy for Hallux Valgus Using Intramedullary Plates With an Amended Algorithm for the Surgical Management of Hallux Valgus.

Distal Chevron osteotomy is a well-established surgical procedure for mild to moderate hallux valgus deformity. Many methods have been described for fixation of osteotomy site; secure fixation, enabling large displacement of the metatarsal head, is one of the essentials of this procedure. The purpose of the present study was to evaluate the short-term radiographic outcome of a distal Chevron osteotomy using an intramedullary plate for the correction of hallux valgus deformity. The present study evaluated 37 patients (40 feet) who underwent distal Chevron osteotomy using an intramedullary plate

Beaming in Charcot Arthropathy-Intramedullary Fixation for Complicated Reconstructions: A Cadaveric Study.

In the modern treatment of Charcot neuroarthropathy, beam screw fixation is an alternative to plate and screw fixation. Exposure is minimized for implantation, and this technique supports the longitudinal columns of the foot as a rigid load-sharing construct. A published data review identified a paucity of data regarding metatarsal intramedullary canal morphology relevant to beam screw fixation. The purpose of the present study was to describe metatarsal diaphyseal morphology qualitatively and quantitatively in an effort to provide data that can be used by surgeons when selecting axially based

[Arthroscopic tibiotalocalcaneal arthrodesis].

Tibiotalocalcaneal arthrodesis (TTCA) is indicated for a variety of disorders, including end-stage osteoarthritis, severe deformities and complications after operative interventions on the upper and lower ankle joints. Due to the biomechanical advantages, TTCA is predominantly performed with curved retrograde intramedullary nails allowing compression before locking. Hindfoot arthrodesis is most commonly performed by extensive open surgical approaches. Despite a patient satisfaction rate greater than 80 %, current reviews have reported mean complication rates of more than 50 % with a pronounced

Elbow joint luxation in a 1-month-old foal.

This paper reports on luxation of the elbow joint without concomitant fracture in a 1-month-old foal. Conservative treatment, with closed reduction and full-limb bandaging, including caudal and lateral splints, seemed successful initially, however, failed to provide enough stability and luxation recurred, and open reduction and surgical placement of prosthetic collateral ligaments was required. Luxation of the elbow joint should be considered when acute non-weight bearing forelimb lameness occurs associated with pain and swelling in the area of the elbow in young foals. Closed reduction failed

Mechanical characterization of a totally intramedullary gradual elongation nail.

An intramedullary gradual elongation nail (Albizzia) capable of fulfilling the function of traditional intramedullary nails while providing the gradual, controlled distraction of an external fixator was designed for progressive lengthening of lower limbs. In this study, the biomechanics of the gradual elongation nail were compared with several intramedullary nails: Grosse & Kempf, Russell-Taylor, AO, and Laffay. Bending stiffness, torsional stiffness, ultimate bending strength, and torsional strength were determined using the American Society for Testing and Materials standard F383-73 as a gui

Evolution of the concept of an extensible nail accommodating to normal longitudinal bone growth: clinical considerations and implications.

Based on animal experiments, we observe that the central portion of an epiphysis can be subjected to acute trauma in a low order of its cross-sectional area without interfering with growth. Moreover, the presence of a metallic device of small diameter placed across the center of the distal femoral epiphysis of a growing dog does not interfere with growth. A metal pin with a T-shaped end can be placed across the distal femoral epiphysis of a growing dog with the T-shaped end fixed in the epiphysis without interfering with growth. As growth occurs, migration of the spin or elongation of the femu

[Retrograde nailing of humerus shaft fractures with the unreamed humerus nail. An international multicenter study].

Combined with the new unreamed humeral nail (UHN) (Synthes), the retrograde approach to the endomedullary canal of the humeral shaft promises careful reduction and fixation of humeral shaft fractures. This prospective multicenter study reports and analyses 102 retrograde nailings with the UHN and their operative procedures. Seven patients with pathological fractures have died meanwhile, but 75 patients could be followed up until bone healing. Seventy-three fresh humeral shaft fractures, 12 pseudarthrosis, 3 refractures and 14 pathological fractures have been treated with the UHN. In 98 cases (

Functional Outcome of Subtrochanteric Femoral Fractures Fixation by Proximal Femoral Locking Compression Plate.

Subtrochanteric femoral fractures are one of the common fractures encountered in today's Orthopaedic practice. This area consists of mostly cortical bone with high stress generation thus heal slowly which leads implant failure. The inherent instability of this fracture and forces of the muscles with comminuted medial calcar is giving the fracture a tendency to varus collapse. Intramedullary nails are not suitable for short proximal segment and wide medullary canal while extramedullary implants are associated with higher rate of implant failure. Recently proximal femoral locking compression pla

Tibiotalocalcaneal hindfoot nailing for failed pilon fractures in neuropathic patients: a prospective case series and treatment rationale.

Failed pilon fractures in patients with peripheral neuropathy present a complex reconstructive challenge due to poor bone quality, impaired healing, and high rates of fixation failure. When standard revision fixation is unlikely to succeed, treatment options are limited and amputation is frequently considered. This prospective case series evaluates the role of tibiotalocalcaneal arthrodesis using retrograde hindfoot nailing as a limb-preserving strategy in this difficult subgroup and clarifies patient selection criteria. Between January 2022 and June 2024, fifteen consecutive patients with fai

Outcome analysis after cephalomedullary nail implantation in older adults and elderly patients with per-, sub- or intertrochanteric femur fractures.

OBJECTIVE: Per-, sub-, and intertrochanteric femur fractures are common in older adults and elderly patients. They frequently lead to functional decline and increased dependency. Despite the routine use of cephalomedullary nails (CMN) for stabilization, detailed data on perioperative outcomes in this vulnerable population are scarce. RESULTS: In this retrospective study, we analyzed 401 patients aged ≥ 50 years who underwent CMN fixation for per-, sub-, or intertrochanteric femur fractures at a university medical center between 2019 and 2024. Pre-, intra-, and postoperative parameters, includ

Intraoperative assessment of reduction quality during nail fixation of intertrochanteric fractures.

BACKGROUND: The quality of intertrochanteric fracture reduction has traditionally been evaluated using simple radiographs. However, subjective intraoperative evaluation and efforts to achieve a perfect reduction are essential for optimal outcomes. This study aimed to establish criteria for the intraoperative assessment of the quality of intertrochanteric fracture reduction and also analyzed postoperative outcomes in terms of reduction quality. METHODS: This study included 106 patients who received cephalo-medullary nailing for the treatment of intertrochanteric fractures between March 2012 an

Pediatric diaphyseal forearm fractures: epidemiology and treatment in an urban population during a 10-year period, with special attention to titanium elastic nailing and its complications.

This study aims to describe the epidemiology of pediatric diaphyseal forearm fractures in an urban population and the complications of titanium elastic nailing. The medical records of 456 consecutive fractures were reviewed. The annual incidence was 0.7/1000 children. Eighty-six (19%) fractures in 83 children were titanium elastic nailed. The complication rate was 24%. Dorsal entry to the radius was associated with extensor pollicis longus rupture and radial entry was associated with sensory nerve deficit in three cases each. Seventy-eight (94%) of the operated patients recovered completely. T
