Angiographic CT with intravenous administration of contrast medium is a noninvasive option for follow-up after intracranial stenting.

Intracranial angioplasty and stenting (ICAS) is a therapeutic option in symptomatic intracranial atherosclerotic disease. Adequate follow-up examination is necessary to exclude in-stent restenosis. Conventional intraarterial digital subtraction angiography (ia-DSA) is the current gold standard, but it is an invasive technique and carries the risk of neurological complications. Angiographic CT (ACT) is a new technique that provides a volume dataset of the highest spatial resolution and high contrast resolution derived from a rotational acquisition of a c-arm-mounted flat-panel detector. The fea

[Evaluation of steal phenomena by measuring the steal index using transcranial Doppler ultrasound in intracranial arteriovenous malformation].

OBJECTIVE: The authors previously deduced a formula for calculating the steal index (SI) for evaluation of the steal phenomena during the development of the collateral circulation resulting from unilateral carotid artery occlusion or stenosis. In this study the authors examine the application of SI in evaluating the steal phenomena of cerebral blood flow in cases of intracranial arteriovenous malformation (AVM). METHODS: The clinical data of 16 cases of AVM confirmed by both digital subtraction angiography (DSA) and transcranial Doppler (TCD) ultrasound were analyzed retrospectively. The rati

Prospective study of the effectiveness and durability of carotid endarterectomy.

In a series of 252 consecutive patients who underwent 282 carotid endarterectomies, we conducted clinical and angiographic follow-up for 2 to 6 years (mean, 3.2 years). Digital subtraction angiography (DSA) was done postoperatively in 95% of cases. Clinical follow-up was achieved in 97% of cases, and DSA follow-up was obtained in 66% of cases. The overall group had a 1% operative minor morbidity (three cases of minimal new neurologic deficit), no major morbidity, and a 0.7% mortality (one death from stroke and one from myocardial infarction). Complications correlated well with the patient's pr

Efficacy and safety of artificial intelligence-assisted image-guided navigation in complex endovascular aortic repair: a systematic review and meta-analysis.

BACKGROUND: Complex endovascular aortic repair exposes patients and operators to substantial radiation, contrast, and long procedures. Artificial intelligence (AI)-assisted image-guided navigation, based on cloud-based three-dimensional image fusion, has been proposed to improve intraoperative guidance, but its comparative benefit is uncertain. METHODS: We searched MEDLINE, Embase, the Cochrane Library, and Web of Science (last search July 2025) for studies comparing AI-assisted image-guided navigation with conventional fluoroscopy and digital subtraction angiography during complex endovascul

Multimodal therapeutic efficacy model for predicting early treatment response to TACE-HAIC combined with immune checkpoint inhibitors and tyrosine kinase inhibitors in unresectable hepatocellular carcinoma.

BACKGROUND: For patients with unresectable hepatocellular carcinoma (uHCC), the novel regimen of combining transarterial chemoembolization (TACE) with hepatic arterial infusion chemotherapy (HAIC), supplemented by immune checkpoint inhibitors (ICIs) and tyrosine kinase inhibitors (TKIs), has brought new hope for treatment. However, individual differences are significant. Some patients do not benefit from the treatment but instead bear the toxicity of the drugs and the economic burden. Accurate prediction of early treatment responses is key to optimizing strategies. METHODS: A total of 205 pat

Transcranial Doppler in diagnosing and monitoring reversible cerebral vasoconstriction syndrome: a systematic review.

INTRODUCTION: Digital subtraction angiography is considered the reference standard for diagnosing reversible cerebral vasoconstriction syndrome (RCVS); however, its invasive nature limits routine application. Transcranial Doppler ultrasonography (TCD) represents a noninvasive and bedside-accessible alternative, but its diagnostic and monitoring value in RCVS remains uncertain. EVIDENCE ACQUISITION: A systematic review was conducted in accordance with PRISMA 2020 guidelines. Searches were performed in PubMed, Embase, Cochrane Library, Scopus, and Web of Science up to January 2024. Eligible stu

Comparison of general anesthesia versus no anesthesia in elective transjugular intrahepatic portosystemic shunt (TIPS): Procedural and hemodynamic parameters.

Transjugular intrahepatic portosystemic shunt (TIPS) is an established treatment for complications of portal hypertension; yet the influence of anesthesia modality on procedural performance and hemodynamics remains insufficiently characterized. This retrospective single-center study compared radiation exposure, procedural parameters, and portal hemodynamics between procedures performed with and without general anesthesia (GA). A total of 84 patients were identified, of whom 62 were age- and sex-matched into two equal groups: group 1 (GA, n = 31) and group 2 (no GA, n = 31). Evaluated parameter

Discordance between CECT and angiographic findings in severe acute pancreatitis-related hemorrhage: implications for interventional management.

BACKGROUND: Hemorrhagic complications in severe acute pancreatitis (SAP) carry high mortality and demand timely intervention. Contrast-enhanced computed tomography (CECT) is routinely performed before digital subtraction angiography (DSA) to guide management decisions, yet its predictive accuracy for angiographic findings and interventional success remains poorly defined. MATERIALS AND METHODS: We conducted a retrospective analysis of 69 bleeding episodes in 56 SAP patients with clinical suspicion of active hemorrhage who underwent both CECT and subsequent DSA. Imaging findings were graded on

Focal Cerebral Arteriopathy in a Young Adult.

Focal cerebral arteriopathy (FCA) is a rare, monophasic stenosis of the distal internal carotid artery (ICA) or proximal middle cerebral artery that primarily affects children but is occasionally seen in adults. It is unclear what causes FCA in adults, which complicates diagnosis and management. The case presented highlights diagnostic challenges using multimodal imaging, supporting conservative management, and the need for adult-specific treatment criteria. A 38-year-old woman with a history of breast cancer, migraines, and prior pulmonary embolism presented with a sudden-onset thunderclap he

Retrieval of a fractured PICC fragment following umbilical venous insertion in a preterm infant: a case report.

BACKGROUND: Due to the immaturity of their organs, preterm infants often require central venous access to ensure effective treatment and nutritional support. Although umbilical vein catheterization is a commonly used method, rare but serious complications such as catheter fracture and migration can occur during the procedure, posing significant challenges and high risks to manage. METHODS: This report describes a case of a male preterm infant with a gestational age of 32 weeks who required a peripherally inserted central catheter (PICC device inserted via the umbilical vein as an off-label ap

The blister sign on non-contrast CT for detection of blood blister-like aneurysms in acute subarachnoid hemorrhage.

PURPOSE: Blood blister-like aneurysms (BBLAs) are rare, fragile lesions associated with a high risk of early rebleeding, yet they are frequently occult on initial non-invasive imaging. Early identification is therefore critical. This study introduces a novel non-contrast CT (NCCT) marker - the blister sign -for early recognition of BBLAs in acute subarachnoid hemorrhage (SAH). METHODS: We conducted a retrospective multicenter cohort study across five tertiary neurovascular centers, including consecutive patients presenting with non-traumatic SAH in 2023. The blister sign was defined on NCCT a

Middle cerebral artery fenestration presenting with cerebral ischemia: a case report and review of the literature.

BACKGROUND: Middle cerebral artery (MCA) fenestration is a rare vascular variant that can mimic stenosis. Its association with cerebral ischemia remains controversial. METHOD: We report the case of a 53-year-old man with a history of transient ischemic attack (TIA), initially misdiagnosed with left MCA occlusion, who presented with sleep disturbances due to fear of stroke recurrence. A definitive diagnosis of MCA fenestration was established by digital subtraction angiography (DSA) and high-resolution magnetic resonance vessel-wall imaging (HRMR-VWI). Perfusion-weighted imaging (PWI) was empl

Percutaneous transluminal angioplasty for vertebral artery stenosis due to primary central nervous system vasculitis: A case report.

We report a rare case of high-grade vertebral artery stenosis caused by biopsy-proven primary central nervous system vasculitis, successfully treated with percutaneous transluminal angioplasty. Primary central nervous system vasculitis with large-vessel involvement is uncommon, and reports of effective endovascular intervention remain limited. This case is notable for 3 reasons: (1) the diagnosis was histologically confirmed, (2) standard immunosuppressive therapy failed to halt disease progression, and (3) endovascular revascularization resulted in durable recovery without complications. A 60

Sliding balloon-assisted thrombectomy combined with aspiration and intrasinus urokinase thrombolysis for the treatment of hemorrhagic cerebral venous sinus thrombosis: experience of 10 patients.

BACKGROUND: Cerebral venous sinus thrombosis (CVST) is an unusual cause of stroke. Currently, standard anticoagulant therapy does not have satisfactory efficacy for treating some cases of hemorrhagic CVST. Therefore, we explored the efficacy and safety of the combination of sliding balloon-assisted thrombectomy with aspiration and intrasinus urokinase thrombolysis for the treatment of CVST patients with intracranial hemorrhage (ICH). METHODS: We retrospectively analyzed the clinical, imaging and follow-up data of 10 CVST patients with ICH who underwent sliding balloon-assisted thrombectomy co
