The Impact of COVID-19 on Acute Surgeries in England Among the Under-25s: A Retrospective Study of 61,360 Appendicitis and 15,850 Testicular Torsion Admissions.

BACKGROUND: Little is known about how COVID-19 impacted acute surgical activity for children and young people (CYP) across England. Appendicitis and testicular torsion are common surgical conditions where treatment delays can lead to avoidable complications. We undertook a retrospective national cohort study. PRIMARY AIM: To describe monthly acute surgical activity in CYP during the COVID-19 pandemic. Secondary aim: To investigate evidence of delayed diagnosis and adverse outcomes, describing variations by age and socioeconomic deprivation. METHODS: Acute hospital admissions with appendiciti

Alvarado Scores Predict Additive Value of Magnetic Resonance Imaging in Workup of Suspected Appendicitis in Children.

BACKGROUND: The presentation of appendicitis in pediatrics is variable, and diagnostic imaging is often used. Magnetic resonance imaging (MR … culated from the electronic medical record. Primary outcomes were pathology-confirmed appendicitis, appendectomy, and perforation. RESULTS: AS was determined for 352 patients out of 463 who met inclusion criteria (76%). Sixty-two percent had an NDUS, and 45% of these patients received MRI. Patients with high-risk AS were significantly more likely to have MRI …

Safety and feasibility of laparoscopic surgery for appendiceal mucocele: a multicenter study.

BACKGROUND: Although laparoscopic appendectomy has been widely performed since 1987, concerns over potential spillage of mucus into the peritoneal cavity during laparoscopic manipulation have prevented the use of laparoscopic surgery (LS) for appendiceal mucocele. The purpose of the present study was to evaluate the safety, feasibility, and short-term perioperative outcomes of LS for appendiceal mucocele. METHODS: A retrospective review was performed to identify patients diagnosed with appendiceal mucocele based on their imaging studies and who underwent surgery at one of six Hallym-Universit…

Concurrent presentation of appendicitis and acute cholecystitis: diagnosis of rare occurrence.

A 67-year-old woman presented with a 2-day history of central abdominal pain migrating to the right upper and lower abdomen. On examination she was normothermic but tachycardic. Inflammatory markers were noted to be elevated with a white cell count of 18.5×10(9)/L and C reactive protein of 265 mg/L. A CT scan revealed dual pathology of appendicitis and acute cholecystitis, which was confirmed intraoperatively and histologically.

Population level analysis of diagnostic laparoscopy versus normal appendicectomy for acute lower abdominal pain.

BACKGROUND: There is virtually no evidence to guide surgeons and patients when faced with an absence of pathology for acute lower abdominal pain. This study aimed to compare diagnostic laparoscopy alone to laparoscopic removal of a normal appendix in patients with acute lower abdominal pain but an absence of pathology. METHOD: Retrospective analysis of routinely collected hospital data from all English acute hospitals performing emergency appendicectomy between 01/04/2002 and 31/03/2012. Patients admitted as emergencies with lower abdominal pain undergoing diagnostic laparoscopy (with no othe

Additional procedures performed during elective colon surgery and their adverse impact on postoperative outcomes.

BACKGROUND: The need for additional procedures during a segmental elective colectomy is considered to lead to increased postoperative morbid … tomy, such as splenic flexure mobilization and endoscopy or a relatively minor procedure such as an appendectomy. Group 3 included patients that had additional procedures performed along with a segmental colectomy. Serious morbidity, overall morbidity, return to the operating room, and death were calculated and compared for each group. RESULTS: There were 25, …

Amyand's hernia: 10 years' experience.

BACKGROUND: Amyand's hernia is an atypical groin hernia which contains the vermiform appendix. The aim of this study was to review a single institution's experience in the clinical presentation, management and prognostic factors of this rare hernia. METHODS: The authors reviewed records of all patients undergoing hernia surgery from 1996 to 2006 at their institution, a tertiary care, University-affiliated hospital. RESULTS: Twelve patients (six men) with a median age of 88 years (range 60-97) were included. Six presented with right inguinal hernias and six presented with right femoral hernia

Routine surgical pathology in general surgery.

BACKGROUND: Although pathological analysis provides the definitive diagnosis for most resection specimens, recent evidence suggests that such analysis may be omitted for certain routine samples. This was a retrospective analysis of the value of routine histopathological examination performed in daily general surgical practice. METHODS: All specimens from routine appendicectomies, cholecystectomies, haemorrhoidectomies and inguinal hernia repairs performed between 1993 and 2002 were included. The analysis included a comparison of histological and macroscopic diagnoses, review of preoperative a

[Laparoscopic therapy of appendicovesical fistula -- two case reports].

Two cases of appendicovesical fistula are reported. One case was caused by Crohn's disease, the second case by a chronical appendicitis. The guiding symptoms in both cases were coprosuria and pneumaturia, with chronical infection of the urinary tract. No diagnostic imaging method was able to detect the fistula. The correct diagnosis was finally made by laparoscopy followed by laparoscopical appendicectomy and fistula closure. The clinical symptoms alone seem to be sufficient for indication to operate. Thus, an early laparoscopic operation significantly can shorten the length of disease.

Initial nonoperative management for periappendiceal abscess.

PURPOSE: Our goal was to compare initial operative and nonoperative management for periappendiceal abscess complicating appendicitis. METHODS: This study is a retrospective review of 155 consecutive patients with appendicitis complicated by periappendiceal abscess treated between 1992 and 1998. Eighty-eight patients were treated initially nonoperatively, and 67 patients were treated operatively. All patients had localized abdominal tenderness and either computed tomography or intraoperative documentation of an abscess. RESULTS: Our patient population consisted of 107 males and 48 females, wi

Right lower quadrant pain and suspected appendicitis: nonfocused appendiceal CT--review of 100 cases.

PURPOSE: To determine the value of standard, nonfocused computed tomography (CT) in examining patients with right lower quadrant (RLQ) pain and suspected appendicitis. MATERIALS AND METHODS: The CT scans and medical records of 100 consecutive patients who presented to the emergency department with RLQ pain and were clinically suspected of having appendicitis were retrospectively reviewed. Helical CT of both the abdomen and pelvis was performed at 7-mm increments after oral and intravenous contrast material administration. CT scans were evaluated for the presence of appendiceal or other diseas

Amyand's hernia: prospective CT diagnosis.

We report two cases of Amyand's hernia, which is the development of acute appendicitis within an inguinal hernia. Both patients were clinically thought to have incarcerated inguinal hernias, but were correctly prospectively diagnosed as having Amyand's hernia on the basis of preoperative computed tomography (CT) examinations. Our cases again show the utility of CT of the acute abdomen and pelvis in revealing a previously unsuspected diagnosis and rapidly triaging patients to the appropriate management.

[Laparoscopic adhesiolysis in therapy of chronic abdominal pain].

Over a period of 14 months 16 patients with chronic abdominal pain were subjected to diagnostic laparoscopy and laparoscopic adhesiolysis. Selective criteria for this operation included previous surgery to the abdomen, clinical symptoms for more than six months and non-conclusive findings through other diagnostic procedures. Operations where an appendicectomy in addition to the adhesiolysis was carried out, were excluded from the study. All patients were requested to quantify their pain by way of a visual analogue scale preoperatively, two days postoperatively and 4 to 18 months postoperativel
