Epiphyseal fractures of the proximal phalanx of the hallux are rare in pediatric patients. While various management techniques exist, studies detailing outcomes, complications, and pathogenic mechanisms of Salter-Harris (S-H) 3 and S-H 4 fractures in children are limited. This study evaluates the radiographic, clinical, and functional outcomes of pediatric patients with these injuries and identifies potential risk factors for adverse results. A retrospective review was conducted on 11 pediatric patients diagnosed with S-H 3 and S-H 4 fractures. Demographics, injury mechanisms, and treatment mo

Biodegradable polylactic acid (PLA) plastic is considered to be an effective method to solve the global white pollution caused by petroleum-based plastics. Its wide application in disposable tableware and food packaging has led to the risk of human exposure to PLA microplastics (MPs), but its impact on bone health is unclear. Our study investigated the skeletal developmental toxicity of PLA-MPs at environmentally accessible concentrations in adolescent mice and its potential mechanism. Here we demonstrated that PLA-MPs treatment in adolescent mice showed significant skeletal developmental toxi

BACKGROUND: Recent animal studies have suggested that cartilage hypertrophy may occur in Legg-Calvé-Perthes disease (LCPD), potentially influencing femoral head morphology during containment treatment. We aimed to analyze cartilage hypertrophy in children with LCPD using magnetic resonance imaging (MRI) and assess its relationship with long-term femoral head outcomes.
METHODS: Fifty-five patients with unilateral LCPD who underwent conservative containment treatment were evaluated. Cartilage thickness was measured in 10 distinct regions on coronal and sagittal MRI scans obtained during the ear

OBJECTIVE: Ossification variants (OVs) of the femoral condyles are traditionally regarded as benign developmental findings distinct from juvenile osteochondritis dissecans (JOCD). We aimed to characterize the longitudinal MRI behavior of OVs and JOCD lesions in asymptomatic siblings of JOCD patients.
MATERIALS AND METHODS: In this HIPAA-compliant longitudinal pilot study, seven asymptomatic siblings of JOCD patients underwent serial 3 T bilateral knee MRI. Two fellowship-trained musculoskeletal radiologists independently assessed 56 studies for bone marrow edema, lesion location, and MRI-defi

UNLABELLED: Anterior cruciate ligament (ACL) reconstruction in skeletally immature patients represents a significant challenge due to the risk of growth disturbance, including limb length discrepancy (LLD) and angular deformity (AD). Although rare, the consequences of such complications can be substantial. Contributing factors include inadequate clinical and radiological preoperative assessment and insufficient postoperative monitoring.
METHODS: This review synthesizes the current understanding of growth disturbances after paediatric ACL reconstruction, examining their incidence, pathophysiol

PURPOSE: Pediatric distal radius fractures with epiphyseal growth arrest can lead to ulnar positive variance and ulnar carpal abutment. In these cases, where there is an associated distally facing sigmoid (or reverse oblique sigmoid, Tolat type III), an ulna shortening osteotomy can result in incongruity of the distal radioulnar joint (DRUJ). We present a series of patients with a distally facing sigmoid who had undergone an ulna shortening osteotomy and sigmoid notch rotational osteotomy to treat the ulnar carpal abutment and adjust for the incongruity of the DRUJ.
METHODS: A review of patie

Skeletal dysplasias (SD) are rare genetic disorders affecting skeletal development and bone growth. Whereas specific gene mutations have been identified in many, however, the molecular signaling pathways contributing to the phenotype are poorly understood. The C-type natriuretic peptide (CNP) signaling pathway is a driver of normal endochondral bone growth and underlies the impact of several genetic disorders of bone growth, including achondroplasia, the most common form of SD. In this cross-sectional study of 73 children with SD, comprising 7 distinct forms, we have examined the association o

The anterior cruciate ligament (ACL) is a crucial stabilizer of the knee joint, and its injury risk and surgical outcomes are closely linked to femoral and tibial anatomy. This review focuses on current evidence on how skeletal parameters, such as femoral intercondylar notch morphology, tibial slope, and insertion site variations—influence ACL biomechanics. A narrowed or concave femoral notch raises the risk of impingement, while a higher posterior tibial slope makes anterior tibial translation worse, which increases ACL strain. Gender disparities exist, with females exhibiting smaller notch d

Adolescent idiopathic scoliosis (AIS) is a spine deformity whose progression during growth is affected by asymmetrical loads acting on the spine. The conservative brace treatment aims to limit the deformity's progression until the end of skeletal growth. This study's objective was to develop a patient-specific finite element model (FEM) simulating immediate in-brace (IB) correction and subsequent growth modulation over 2 years of treatment. Thirty-five retrospective AIS cases with documented correction over 2 years were analyzed. For each case, a patient-specific FEM was built and IB correctio

To improve quantification of individual responses to bed rest interventions, we analyzed peripheral quantitative computer tomography (pQCT) datasets of the lower leg of 76 participants, who took part in eight different bed rest studies. A newly developed statistical approach differentiated measurement uncertainty U Meas from between-subject-variation (BSV) and within-subject variation (WSV). The results showed that U Meas decreased 59.3-80% over the two decades of bed rest studies (p < 0.01), and that it was higher for muscles than for bones. The reduction of U Meas could be explained by impro

INTRODUCTION: Rickets involves the softening of bones in children and osteopenia with disordered calcification, leading to a higher proportion of osteoid tissue prior to epiphyseal closure in children. Rickets is common in the Middle East, Africa, and Asia. The peak age of prevalence is 3-18 months. Signs include bone tenderness, easy fracture, early bone deformity, delayed closure of fontanelles, and softening of skull bones (craniotabes).
OBJECTIVE: This study assesses knowledge in the AlBaha region about rickets' clinical presentation, causes, complications, and prevention, as well recomme

AIMS: Osteoarthritis (OA) is the most prevalent systemic musculoskeletal disorder, characterized by articular cartilage degeneration and subchondral bone (SCB) sclerosis. Here, we sought to examine the contribution of accelerated growth to OA development using a murine model of excessive longitudinal growth. Suppressor of cytokine signalling 2 (SOCS2) is a negative regulator of growth hormone (GH) signalling, thus mice deficient in SOCS2 (Socs2 -/-) display accelerated bone growth.
METHODS: We examined vulnerability of Socs2 -/- mice to OA following surgical induction of disease (destabilizat

Natriuretic peptide receptor 2 (NPR2) plays a key role in cartilage and bone morphogenesis. The NPR2 gene mutations result in acromesomelic dysplasia, Maroteaux type (AMDM), short stature with nonspecific skeletal abnormalities (SNSK), and epiphyseal chondrodysplasia, Miura type (ECDM). However, the pathogenic mechanism remains unclear. In our study, we identified one de novo (R557C) and six novel variants (G602W, V970F, R767*, R363*, F857S, and Y306S) in five independent Chinese families with familial short stature. Three patients with heterozygous mutations (G602W, V970F, and R767*) were dia

BACKGROUND: The mechanism of body growth in mammals is poorly understood. Here, we investigated the regulatory networks involved in body growth through transcriptomic analysis of pituitary and epiphyseal tissues of smaller sized Debao ponies and Mongolian horses at the juvenile and adult stages.
RESULTS: We found that growth hormone receptor (GHR) was expressed at low levels in long bones, although growth hormone (GH) was highly expressed in Debao ponies compared with Mongolian horses. Moreover, significant downregulated of the GHR pathway components m-RAS and ATF3 was found in juvenile ponie

Severe prolonged hypothyroidism due to Hashimoto thyroiditis may lead to rapid pubertal progression and compromised adult height after initiation of levothyroxine (LT4) therapy. There are no reports of aromatase inhibitor use to augment height in these patients. We describe a patient with severe hypothyroidism and growth failure who experienced rapid pubertal and bone age maturation on initiation of LT4 therapy. Anastrozole was added after 2 years to delay epiphyseal fusion. A boy aged 12 years and 1 month presented to the endocrine clinic with short stature and a markedly delayed bone age of

An excess of growth hormone (GH) results in accelerated growth and in childhood, the clinical manifestation is gigantism. When GH excess has its onset after epiphyseal fusion at puberty, the overgrowth of soft tissue and bone results in acromegaly. Persistent GH excess in gigantism also causes acromegalic features that become evident in the adult years. The causes of GH excess are primarily lesions in the pituitary, which is the main source of GH. In this review, we provide an update on the clinical, radiological and pathologic features of the various types of pituitary neuroendocrine tumors (
