Humeral shaft nonunions treated with fibular allograft and compression plating.

The present study is a retrospective review of the treatment of 12 humeral shaft nonunions by using an intramedullary allograft with compression plating. The average age of the patients was 61 years (range, 36-82 years). Eight cases involved the proximal shaft, 3 cases were at the mid-diaphyseal level, and 1 case was at the distal one third. Follow-up averaged 30 months (range, 12-96 months). Ten patients (83%) went on to uneventful healing at an average of 3 months after surgery. Two failures involving patients with multiple medical conditions occurred secondary to reinjury. Two cases of post

Technique of using the AO-femoral distractor for femoral intramedullary nailing.

Immediate closed interlocking intramedullary (IM) nailing of femoral shaft fractures in polytraumatized patients using the fracture table may be difficult due to concomitant adjacent injury. We report on the technique of using the AO-femoral distractor for femoral IM nailing and our experience with 56 consecutive cases of IM nailing of the femur. The AO distractor is an alternative to the traction table. Unlike the latter, it does not rely on intact adjacent structures to distract the main fragments. Schanz screws are placed into the femur condyle and into the lesser trochanter. The fracture i

Intramedullary nailing for impending pathological subtrochanteric fractures.

We reviewed ten patients with metastatic carcinoma and impending pathological fractures of the subtrochanteric region of the femur who had prophylactic insertion of the Russell-Taylor reconstruction nail. There were no operative complications or long-term mechanical failures. This reconstruction nail has some technical and biomechanical advantages over other implants and is ideal for the management of such cases.

Treatment of a Large Tibial Non-Union Bone Defect in a Cat Using Xenograft with Canine-Derived Cancellous Bone, Demineralized Bone Matrix, and Autograft.

A 17-month-old domestic short-hair cat was referred due to a non-union in the left tibia. The initial repair, conducted 3 months prior at another animal hospital, involved an intramedullary (IM) pin and wire to address a comminuted fracture. Unfortunately, the wire knot caused a skin tract, resulting in osteomyelitis. Although the wire knot was removed at that hospital, the draining tract persisted, continuously discharging exudate. Upon evaluation, the first surgery was reassessed and revised, involving the removal of the IM pin and the application of external skeletal fixation alongside an a

Incidence of nonunion after surgery of distal femoral fractures using contemporary fixation device: a meta-analysis.

PURPOSE: Nonunion is the most frequent cause of reoperation and is associated with high morbidity after distal femur fracture (DFF). We examined the rates of nonunion requiring reoperation after fixation for DFF using a locking compression plate (LCP) or retrograde intramedullary nail (RIMN). METHODS: We included four studies comparing LCP and RIMN and 38 single-cohort studies reporting LCP or RIMN. In total, 2156 femurs were included and 166 non-unions were detected. We conducted a pair-wise meta-analysis (with a fixed-effects model) on the four comparative studies and a proportional meta-an

Vascularized free fibular bone graft in the management of congenital tibial pseudarthrosis.

Congenital pseudarthrosis of the tibia (CPT) remains one of the most challenging problems confronting the orthopaedic surgeon. The operative results are frequently less than successful; many cases require several surgical procedures, and a significant number of them ending in amputation. The purpose of this study was to access the surgical results, complications, secondary procedures, and long-term results of free vascularized fibular graft (FVFG) in the treatment of congenital pseudarthrosis of the tibia. Between 1992 and 2007, nine patients with CPT were treated consecutively at our clinic w

Impact of limited intramedullary reaming versus unreamed technique in short proximal femoral nail anti-rotation for intertrochanteric fractures: a matched retrospective study.

PURPOSE: Short proximal femoral nail antirotation (PFNA) fixation is commonly performed for intertrochanteric fractures and is typically inserted without reaming. However, limited reaming may be required in cases of a narrow femoral canal, severe femoral bowing, or inadvertent selection of an oversized nail. Unlike long nails, reaming for short PFNA is confined to the isthmus, and its effects have not been well described. This study aimed to evaluate the association between limited reaming during short PFNA fixation and clinical outcomes. METHODS: We conducted a retrospective matched cohort s

Fixation of unstable ankle fractures using a long hindfoot nail.

BACKGROUND: Management of fragility fractures of the ankle is challenging. Non-operative treatment often requires significant periods of reduced weight bearing. Operative fixation is difficult due to osteoporotic bone and poor soft tissues. The aim of this study was to review the initial experience of treating unstable ankle fractures with a retrograde femoral nail used as a long tibiotalocalcaneal nail. PATIENTS AND METHODS: Sixteen patients were treated from January 2014 until November 2017. Mean age was 73 (50-96) and twelve of the sixteen patients were female. All patients had three or mo

A prospective multi-center study of intramedullary nailing vs casting of stable tibial shaft fractures.

BACKGROUND: The purpose of this study was to determine optimal treatment of stable tibial shaft fractures using intramedullary nailing (IMN) or casting. MATERIALS AND METHODS: We performed a multi-center prospective study cohort. Patients with stable tibia shaft fractures meeting Sarmiento's criteria (isolated closed fractures with less than 12 mm of shortening and 10° of angulation) were enrolled prospectively and treated with either a reamed IMN with static interlocking screws or closed reduction followed by long-leg casting. Both groups were weight bearing following surgery. Radiographs we

The effect of autologous concentrated bone-marrow grafting on the healing of femoral shaft non-unions after locked intramedullary nailing.

The aim of this study was to assess the union rates in a series of patients with failed femoral shaft aseptic non-union who were treated with percutaneous concentrated autologous bone marrow grafting. Bone marrow harvesting and cell injection were performed under general anaesthesia in a single surgical procedure. Radiographic union was diagnosed in fractures with a score ≥ 10 according to the radiographic union scale in tibial fractures (RUST) and confirmed by clinical examination. Eight out of 16 patients progressed to consolidation (RUST score ≥ 10). Radiographic evidence of fracture union

Osteonecrosis of distal tibia in open dislocation fractures of the ankle.

INTRODUCTION: Open ankle dislocation fractures are one of the most severe injuries of the ankle. Development of posttraumatic arthrosis is well known. However, there are just a few case reports describing evidence of posttraumatic osteonecrosis (PON) of distal tibia. The pathophysiological mechanism remains unclear and the question of morphologic or personal risk factors cannot be answered. The goal of this study was to evaluate the morphologic characteristics of open dislocated ankle fractures in correlation with the development of PON to facilitate early identification of patients with highe

Evaluation of methods and timing in nail dynamisation for treating delayed healing femoral shaft fractures.

INTRODUCTION: Dynamisation is a simple method for treating femoral shaft fractures (FSFs) in patients with delayed healing after intramedullary nailing. However, only around half of patients with a slow-healing FSF have been successfully treated with this strategy. Although it is thought that dynamisation with a screw preserved in the dynamic locking hole improves treatment outcome in these patients, to date, no studies have demonstrated empirical evidence supporting this technique. In this study, we investigated the effects of dynamisation with and without rotation and length control as well
