Step-cut osteotomy for cubitus varus deformity with application of patient-specific instrument in children: a preliminary report.

PURPOSE: The step-cut osteotomy has been recognized as a valuable approach for addressing cubitus varus deformity, albeit one that necessitates technical proficiency. This study aims to evaluate the efficacy of the modified step-cut osteotomy technique in conjunction with patient-specific instruments by clinical and radiological assessment. METHODS: We conducted a retrospective review of patients who underwent modified step-cut osteotomy with the use of patient-specific instruments in conjunction with Kirschner wires fixation for the correction of cubitus varus deformity between April 2016 an

[Complications of supracondylar fractures of the humerus in children].

Vascular, neurological and bony complications of sopracondylar fractures of the humerus in children are discussed in detail. A series of cases seen and treated at the Orthopaedic Clinic of the University of Padua are reviewed. Most cases were treated conservatively with good results. Neurological and vascular complications cleared completely, functional recovery was most satisfactory with minimal axis deviation after manipulation of the supracondylar area. In conclusion, conservative treatment remains the treatment of choice for this injury. Local conditions or complications can, in a very lim

Efficacy of 3D-printed surgical guides in the correction of pediatric cubitus varus deformity: a systematic review and meta-analysis.

BACKGROUND: Cubitus varus is a frequent post-traumatic deformity in children, typically resulting from malunion of supracondylar humeral fractures. Despite the availability of various osteotomy techniques, achieving precise and reproducible correction remains challenging. Recently, 3-dimensional (3D) printing-guided osteotomy has gained attention for improving surgical accuracy and outcomes. This study aimed to systematically evaluate the clinical effectiveness and safety of 3D-guided corrective osteotomy in pediatric cubitus varus in comparison with conventional surgical techniques. METHODS:…

Cement augmentation for proximal humerus fractures: a meta-analysis of randomized trials and observational studies.

INTRODUCTION: It is unclear if elderly patients treated with plate osteosynthesis for proximal humerus fractures benefit from cement augmentation. This meta-analysis aims to compare cement augmentation to no augmentation regarding healing, complications, and functional results. METHODS: PubMed, Embase, and Cochrane Central Register of Controlled Trials were searched for randomized clinical trials and observational studies. Effect estimates were pooled across studies using random effects models. The primary outcome is overall complication rate. Stratified analyses were performed for types of c

Simple Osteotomy Site Repair Method for the Trans-Olecranon Approach: Tension Band Wiring with Ring Pins.

Background: The trans-olecranon approach is commonly used to treat intra-articular distal humeral fractures (DHFs). We describe an osteotomy site repair technique that is both simple and safe. Methods: We retrospectively reviewed 30 patients with intra-articular DHFs who were treated via olecranon osteotomies repaired by tension band wiring (TBW) with ring pins. Medical records and radiographs were retrospectively assessed in terms of injuries, operative characteristics, clinical outcomes, and complications, as well as any need for hardware removal. Clinical outcomes were evaluated by deriving…

Safe zone for lateral pin placement for external fixation of the distal humerus.

External fixation is a common, efficient technique used for humeral shaft stabilization and elbow fractures. There are reports of radial nerve injuries associated with this procedure. In this study, we investigated the course and variability of the radial nerve along the lateral humerus in relation to the elbow joint to determine a relatively safe zone for lateral pin placement in external fixation. Twenty upper extremities from 10 cadavers were studied. The nerve branches and course of the radial nerve along the lateral humerus were carefully dissected. Straight lines (a, b, and c) were made

[Calcium heparin-induced immunologic thrombocytopenia complicated with venous gangrene of the legs. Report of a clinical case].

An 80-year-old woman was admitted to the Traumatologic Hospital of Florence because of bilateral post-traumatic humeral fractures. Subcutaneous calcium heparin was immediately administered to prevent deep venous thrombosis. Nine days later, the patient was referred to the Internal Medicine Unit because of severe immune-mediated heparin-induced thrombocytopenia and bilateral deep venous thrombosis. Heparin therapy was immediately discontinued, and the patient was switched to warfarin therapy. Diagnosis of heparin-induced thrombocytopenia was confirmed by positivity of anti-heparin-PF4 antibodie…

Evaluation of risk factors for stiffness after distal humerus plating.

PURPOSE: In distal humerus fractures, the goal is to achieve a functional range of motion of 30°-130° which is not always possible. The aims of the study were to evaluate the functional results after distal humerus fracture operation and to investigate the risk factors for stiffness. METHODS: Between 2005 and 2014, 75 patients with the mean age of 37.8 years (17-80) underwent open reduction and plate fixation for distal humerus fractures. Range of motion (ROM), Mayo elbow performance scores, and quick DASH scores were used for functional evaluation. Patients were divided into two groups accor

Shoulder dystocia.

The frequency of shoulder dystocia in different reports has varied, ranging 0.2-3% of all vaginal deliveries. Once a shoulder dystocia occurs, even if all actions are appropriately taken, there is an increased frequency of complications, including third- or fourth-degree perineal lacerations, postpartum hemorrhage, and neonatal brachial plexus palsies. Health care providers have a poor ability to predict shoulder dystocia for most patients and there remains no commonly accepted model to accurately predict this obstetric emergency. Consequently, optimal management of shoulder dystocia requires

Use of an acellular collagen-elastin matrix to support bladder regeneration in a porcine model of peritoneocystoplasty.

BACKGROUND: Proximal humerus fractures with medial calcar disruption present a surgical challenge. Screw cut-out and varus collapse are common complications following fixation. Augmentation of fixation using intramedullary fibular autograft with medialization along the calcar adequately restores the medial column, which is a well-described technique in the literature. We undertook this study to evaluate the results of central intramedullary placement of fibular autograft without medialization. MATERIAL AND METHODS: We conducted this prospective study with 15 patients over 2 years with a minim

Mechanistic-data synergy for predicting fracture load in canine bone with appendicular osteosarcoma.

Canine appendicular osteosarcoma substantially increases the risk of pathological fractures. Limb-sparing approaches such as Stereotactic Body Radiation Therapy (SBRT) can extend and improve the quality of life in large-breed dogs; however, recent clinical studies have reported high post-SBRT fracture rates and significant complications following prophylactic stabilization. While patient-specific Finite Element Analysis (FEA) can estimate pre- and post-stabilization fracture-initiation thresholds, its computational burden limits its clinical applicability. This study presents Machine Learning

Outcomes of modular diaphyseal vs. nonmodular metaphyseal reverse total shoulder arthroplasty stem for 3- and 4-part proximal humerus fractures.

BACKGROUND: Proximal humerus fractures (PHFs) are increasingly prevalent in the aging population, with reverse shoulder arthroplasty (rTSA) emerging as a preferred surgical treatment for complex fracture patterns. Recent advancements have introduced shorter, metaphyseal-fixation humeral stems as alternatives to traditional modular diaphyseal stems. However, limited data exist comparing the clinical outcomes of these designs following rTSA for PHF. METHODS: A retrospective analysis of 99 patients undergoing rTSA for acute three- or four-part PHFs between 2014 and 2024 was conducted. Patients r

Intramedullary Nailing for Lower Limb Polyostotic Fibrous Dysplasia in Children: A Long-term Follow-up Study.

BACKGROUND: In children, intramedullary nailing (IN) has been proposed as the best treatment when the femur and tibia are totally affected by fibrous dysplasia (FD). However, in younger children IN must be repeated to maintain stabilization of the affected skeletal segment during growth. We report the long-term results in a cohort of patients in whom more than two-thirds of cases had IN repeated during growth. METHODS: Twenty-nine femurs and 14 tibias totally affected by FD were treated by IN in 21 patients with polyostotic FD and McCune-Albright syndrome. Thirteen patients with 35 femoral an
